Does the use of fentanyl in epidural solutions for postthoracotomy pain management in neonates affect surgical outcome?

BACKGROUND/PURPOSE: Continuous epidural analgesia is routinely used to manage pain in infants undergoing resection of a congenital cystic adenomatoid malformation (CCAM) of the lung. Our aim was to determine if there is a difference in the length of stay (LOS), supplemental analgesic requirements, pain control, and the incidence of adverse respiratory events in infants receiving the 2 standard epidural solutions commonly used: bupivacaine 0.1% and bupivacaine 0.1% with fentanyl 2 to 5 microg/mL. METHODS: We retrospectively reviewed the charts of infants who received epidural infusions contain

Successful repair of tracheal dehiscence after heart-lung transplantation.

Tracheal dehiscence remains one of the most feared complications after heart-lung transplantation because of its nearly universal fatality rate. Drawing on experiences from the tracheal reconstruction and the lung transplantation literature, we report the successful repair of tracheal dehiscence, after heart-lung transplantation, with an intercostal muscle flap.

Anatomic repair of transposition of great arteries with ventricular septal defect and aortic arch obstruction. One-stage versus two-stage procedure.

Between September 1, 1982, and March 1, 1992, 40 patients underwent anatomic repair of transposition of the great arteries, ventricular septal defect, and aortic arch obstruction. In group I, 26 patients (65%) underwent repair in a two-stage procedure, phases A and B. Phase A included repair of the aortic arch obstruction with (16 patients) or without (10 patients) pulmonary artery banding through a left thoracotomy (mean age 18.7 +/- 23.4 days). There were three deaths and three reoperations. Phase B included an arterial switch operation with closure of the ventricular septal defect (mean age

Large Carinal Chondrosarcoma: Resection of the Main Carina, Distal Trachea, and Orifices of the Mainstem Bronchi and Construction of a New Carina.

BACKGROUND Chondrosarcoma of the trachea is an extremely rare malignancy, and tumors located at the main carina are even rarer. Carinal resection and subsequent reconstruction of a new carina constitute a demanding surgical procedure that requires extensive expertise and close collaboration between the anesthetic and surgical teams. In this case report, we aim to highlight the rarity of chondrosarcoma of the main carina and emphasize the critical role of surgical intervention despite the complexity of the procedure. Furthermore, we aim to share our clinical experience and contribute an additio

Fatal arrhythmia associated with myocardial contusion caused by a horse kick: A forensic autopsy case.

BACKGROUND: Horse-related injuries often involve high-energy blunt trauma that can be fatal. In cases of chest trauma associated with horse kicks, death is typically attributed to structural cardiac injuries such as cardiac rupture or tamponade. Fatal arrhythmia associated with myocardial contusion following a horse kick has rarely been reported in the forensic literature. CASE PRESENTATION: A 48-year-old man was kicked in the left lower chest by the right hind limb of a Thoroughbred horse while handling the animal. The patient collapsed immediately after the impact. Emergency medical personn

Survival after severe blunt cardiac injury: Right atrial rupture.

BACKGROUND: Blunt cardiac rupture is a rare but life-threatening injury that imparts extremely high mortality. Most patients who experience cardiac rupture die at the scene, but timely response in diagnosis and treatment can improve the survivability of this injury. PURPOSE: To describe a case of blunt cardiac injury with right atrial rupture after involvement in a high-speed motor vehicle collision. Early resuscitation with blood products and timely operative treatment were pivotal for the survival of this patient. Follow up with echocardiography revealed a rare presentation of inter-atrial

Morgagni hernia associated with ipsilateral lateral chest wall defect: a case report of transdiaphragmatic intercostal hernia in an adult.

INTRODUCTION AND IMPORTANCE: Transdiaphragmatic intercostal hernias (TDIHs) are rare and typically posttraumatic. The coexistence of a TDIH with congenital anomalies, such as a Morgagni hernia (MH) and chest wall defect, is exceptionally uncommon. This report details a unique case of MH associated with an ipsilateral intercostal hernia in an adult, underscoring key diagnostic and surgical challenges. CASE PRESENTATION: A 71-year-old man with multiple comorbidities presented with acute abdominal pain and a right-sided chest wall bulge. Computed tomography revealed a large right lateral diaphra

Comparing thoracoscopic anatomical lesion resection and lobectomy in children with congenital lung malformations: a multicenter retrospective cohort study.

BACKGROUND: The efficacy of thoracoscopic anatomical lesion resection (TALR) for children with congenital lung malformations (CLMs) remains controversial. This multicenter cohort study aimed to compare the perioperative outcomes of TALR and thoracoscopic lobectomy (TL) in pediatric CLM patients. METHODS: Clinical data of 1176 children with CLMs who underwent TALR (n = 692) or TL (n = 484) in 6 Chinese hospitals from January 2020 to December 2024 were analyzed. Propensity score matching (PSM) was used to adjust for confounding factors, and the effect of indocyanine green (ICG) navigation in th

A congenital shunt between the right costocervical artery, left coronary artery, and left pulmonary artery in a dog.

A one-year-old neutered male Kooikerhondje dog, weighing 12 kg, was referred for evaluation of a heart murmur without clinical signs and mosaic blood flow within the main pulmonary artery detected by transthoracic echocardiography. On day 0, transthoracic echocardiography revealed left heart enlargement, continuous mosaic flow within the main pulmonary artery, and a shunt vessel entering the left pulmonary artery. Electrocardiography (ECG)-gated non-helical computed tomography angiography (CTA) using a 320-row multidetector computed tomography scanner demonstrated two anomalous vessels that co

Usefulness of Transparent Illumination Device in Esophageal Atresia for the Detection of the Distal Esophagus.

INTRODUCTION: Pure esophageal atresia without a tracheoesophageal fistula (Gross type A) is technically demanding, particularly with respect to intraoperative identification of the distal esophageal pouch. Biologically transparent illumination (BTI) devices have been used to visualize nasogastric tubes in pediatric patients; however, their application during esophageal reconstruction has not been reported. CASE PRESENTATION: A male neonate with prenatally suspected esophageal atresia was diagnosed postnatally with Gross type A esophageal atresia. After gastrostomy and serial bougienage of bot

Review of minimally invasive coronary artery bypass grafting.

OBJECTIVES: Minimally invasive coronary artery bypass grafting (CABG), defined broadly as surgical revascularization via any sternotomy-sparing approach. Here, we provide an overview of minimally invasive CABG targeted to cardiologists, cardiac surgeons and other clinicians involved in the care of patients with coronary artery disease (CAD). METHODS: A narrative review of the literature on minimally invasive CABG was performed. RESULTS: Minimally invasive CABG was first described over 20 years ago, yet uptake has been slow and available data are limited. The most common iteration of minimall

Boerhaave syndrome complicated by ruptured esophageal varices: a case report.

BACKGROUND: Boerhaave syndrome is a life-threatening condition caused by a spontaneous full-thickness rupture of the esophagus, typically following forceful vomiting. Esophageal variceal rupture is another potentially fatal condition commonly observed in patients with liver cirrhosis. These two conditions are usually reported independently, and their coexistence is extremely rare. Managing this combination is particularly challenging because of the need for both infection control and hemostasis in the presence of portal hypertension. CASE PRESENTATION: We report the case of a 51-year-old man

High-dose glucocorticoids in the treatment of postoperative pain after video-assisted thoracoscopic surgery-protocol for systematic review and meta-analysis.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is widely used in lung cancer surgery, as this technique causes less pain and faster recovery than open thoracotomy. However, significant postoperative pain persists in a number of patients, often leading to increased opioid use and opioid-related adverse events in addition to prolonged admission times. Perioperatively administered glucocorticoids have been demonstrated effective in reducing pain after other types of surgeries, but the effect in VATS remains unclear. METHODS: This systematic review will assess the impact of glucocorticoi

Precise diagnosis and effective management of highly vascular anterior mediastinal paragangliomas: A case report.

INTRODUCTION: Mediastinal paragangliomas are rare neoplasms arising from extra-adrenal neural crest cells, presenting as either functional or nonfunctional tumors. Clinical manifestations range from catecholamine-related symptoms to physical compression effects. Accurate recognition of these tumors is crucial for diagnosis and management due to their rarity and association with vital mediastinal structures. PRESENTATION OF CASE: We report the case of a 53-year-old Syrian male presenting with progressive dyspnea, fatigue, and weakness. Imaging revealed a highly vascular anterior mediastinal ma

Damage control resuscitation: how it's done and where we can improve. A view of the Brazilian reality according to trauma professionals.

INTRODUCTION: Hemorrhage is the leading cause of preventable deaths in trauma patients, resulting in 1.5 million deaths annually worldwide. Traditional trauma assessment follows the ABC (airway, breathing, circulation) sequence; evidence suggests the CAB (circulation, airway, breathing) approach to maintain perfusion and prevent hypotension. Damage Control Resuscitation (DCR), derived from military protocols, focuses on early hemorrhage control and volume replacement to combat the "diamond of death" (hypothermia, hypocalcemia, acidosis, coagulopathy). This study evaluates the implementation of
