BACKGROUND: Tuberculosis is a serious, and unfortunately common, infectious disease caused by the organism Mycobacterium tuberculosis. The incidence, mortality, and physical/ophthalmologic effects are well documented. Although most common ophthalmologic findings include optic neuropathy and uveitis, cranial nerve palsies may also present secondary to intracranial inflammation.
CASE REPORT: A 17-year-old Somalian girl was referred to the eye clinic with the chief concern of intermittent diplopia and headaches. Although monocular visual acuity was excellent, ocular positioning tests confirmed a…

Juvenile idiopathic arthritis is a term that encompasses all forms of arthritis that begin before the age of 16 years, persist for more than 6 weeks and are of unknown cause. Dry synovitis is still not completely understood nor commonly described. It is associated with juvenile idiopathic arthritis and must be considered in patients with minimal swelling but pain and stiffness along with flexion contractures as well as other evidence of an inflammatory process (lab changes and/or other symptoms, such as uveitis or rash), and often follow a destructive course. The authors present a case of a br…

Wegener's granulomatosis mainly affects the upper respiratory tract, the lungs and the kidneys but many other organs and tissues may be affected, as well including any eye structures. Ocular and orbital structures involvement is frequently encountered, sometimes inaugural, with or without obvious systemic manifestations, sometimes leading to visual loss. The most frequent ophthalmic lesions are inflammatory orbital disease, (necrotising) scleritis with or without keratopathy, episcleritis, conjunctivitis and even conjunctival ulcer, nasolacrimal ducts obstruction, uveitis, retinal vasculitis a…

Extra-intestinal manifestations occur in at least 25% of Crohn's disease patients. Some extra-intestinal manifestations, such as erythema nodusum and peripheral arthropathy, will wax and wane in keeping with bowel inflammation. The more severe cutaneous ulcerations, uveitis, and axial arthropathy may precede bowel disease or persist after it subsides. Screening may be appropriate for eye disease and for osteoporosis to prevent complications. Medical management for extra-intestinal manifestations is similar to treatment for the bowel symptoms of Crohn's disease, with corticosteroids the mainsta…

The first case of postoperative endophthalmitis after cataract surgery caused by Aspergillus ustus, a species that has only rarely been implicated in human disease, is described. Six weeks after cataract surgery, a 67-year-old medically controlled diabetic patient presented with uveitis, mild ciliary injection and ocular discomfort. Anterior chamber paracentesis, vitreous tap and finally complete vitrectomy with removal of the capsular bag including the intraocular lens were performed and several sets of culture yielded A. ustus. Despite vigorous systemic (itraconazole and caspofungin) and int…

PURPOSE: To report a case of cytomegalovirus (CMV) retinitis after placement of a fluocinolone acetonide (Retisert) implant.
DESIGN: Interventional case report.
METHODS: Retrospective chart review.
RESULTS: A 65-year-old man with a history of Adamantiades-Behcet disease and bilateral recurrent uveitis that was unresponsive to systemic corticosteroid-sparing immunosuppressive therapy developed clinical evidence of CMV retinitis after receiving his second intravitreal Retisert implant in the left eye, while on no systemic immunosuppression. He did not develop CMV retinitis in the right eye de…

PURPOSE: To bring in the attention of the medical personnel a relatively new discovered disease with serious consequences, that is often mis … diagnosed and consequently untreated. Its ocular involvement may vary from a mild conjunctivitis to uveitis, optic neuritis and sometimes lead to blindness. The disease is endemic in Romania. If it is correctly diagnosed it can be successfully treated with specific antibiotics administrated orally or i.v.

This case report describes a rare manifestation of syphilis. An HIV-positive patient with severe immunosuppression presented with rapid loss of vision due to eye involvement of Treponema pallidum infection. The ophthalmologic examination on admittance showed a chorioretinitis, an uveitis, and a swollen optical disc of both eyes. Reconstitution of vision after application of intravenous penicillin occurred within 3 weeks. Despite low HIV prevalence in central Europe, behaviors that promote transmission of HIV and other sexually transmitted diseases tend to increase. The clinical presentation of…

Relapsing polychondritis is a rare disease of unknown etiology causing systematic inflammation of the whole cartilaginous tissues. We report a patient with relapsing polychondritis preceded by inner ear involvement. A 68-year-old female visited our hospital for a chief complaint of sudden bilateral hearing loss. On pure-tone audiometry, sensorineural hearing loss, 47dB on the right and 51dB on the left, was observed. Later, bilateral auricular chondritis and uveitis developed. On biopsy of the auricular cartilage, perichondritis was observed. The patient was diagnosed with relapsing polychondr…

DESIGN: Interventional case report.
METHODS: To rule out intraocular tumor, ultrasound biomicroscopy was performed in a patient referred for a 2-day history of uveitis with elevated intraocular pressure, dark hypopyon, and pigment dispersion in the anterior chamber.

Behçet's disease is an inflammatory process of unknown origin, which usually presents with recurrent oral ulcers, genital aphthae, uveitis and cutaneous lesions. However, a wide variety of clinical manifestations have been reported, and virtually any organ system may be affected, showing central nervous system, joints, blood vessels or gastrointestinal tract involvement. Therapeutic approach remains complex, and varies in basis of the affected organs. Complex aphthosis may respond to topical therapy, colchicine and dapsone. If this therapy does not result in adequate disease control, thalidomi…

PURPOSE: This study aimed to identify the main ocular complications of leprosy in Cameroon.
PATIENTS AND METHODS: It is a prospective cross-sectional study which took place from July 1998 to January 1999 in five leprosaria in Cameroon. The ophthalmological examination of all patients was performed by the same team.
RESULTS: Of the 218 patients examined, 60.1% were males and 39.9% females. 72.5% had a paucibacillary leprosy and 27.5% a multibacillary form. 77.5% of patients had at least one ocular lesion and 38.3% of eyes had visual acuity < or = 1/10. Madarosis and anterior uveitis were more…

Loteprednol etabonate (LE) is a corticosteroid designed using the "soft drug" concept of Bodor. LE has been extensively evaluated as a treatment for ophthalmic inflammatory conditions. LE is administered as a sterile eye drop suspension and is commercially available as either a 0.5% or a 0.2% suspension. Lotemax (0.5% LE) has been demonstrated as effective in reducing the signs and symptoms of giant papillary conjunctivitis (GPC), acute anterior uveitis and inflammation following cataract extraction with intraocular lens (IOL) implantation. It is also effective for the prophylaxis of seasonal …
