Factors related to intra-tendinous morphology of Achilles tendon in runners.

The purpose of this study was to determine and explore factors (age, sex, anthropometry, running and injury/pain history, tendon gross morphology, neovascularization, ankle range of motion, and ankle plantarflexor muscle endurance) related to intra-tendinous morphological alterations of the Achilles tendon in runners. An intra-tendinous morphological change was defined as collagen fiber disorganization detected by a low peak spatial frequency radius (PSFR) obtained from spatial frequency analysis (SFA) techniques in sonography. Ninety-one runners (53 males and 38 females; 37.9 ± 11.6 years) wi

Conservative versus surgical treatment for displaced fracture of the medial process of the calcaneal tuberosity.

PURPOSE: To compare the outcome following conservative or surgical treatment for displaced fracture of the medial process of the calcaneal tuberosity. METHODS: 14 men and 4 women aged 20 to 44 years chose to undergo conservative (9 feet) or surgical (10 feet) treatment by a single surgeon for closed displaced fracture of the medial process of the calcaneal tuberosity. The injury mechanism was a fall from a height of <1.5 m; the mean time from injury to treatment was 3 (range, 1-7) days. Conservative treatment comprised immobilisation in a plaster cast. Surgical treatment involved fixation wit

Safe Zone for Neural Structures in Medial Displacement Calcaneal Osteotomy: A Cadaveric and Radiographic Investigation.

BACKGROUND: We aimed to define reference lines on standard lateral ankle radiographs that could be used intraoperatively to minimize iatrogenic nerve injury risk in medial displacement calcaneal osteotomy. METHODS: Forty cadaveric specimens were used. In 20 specimens, the sural, medial plantar (MP), and lateral plantar (LP) nerves were sutured to radiopaque wire, and a lateral ankle radiograph was obtained. On the radiograph, a line was drawn from the posterior superior apex of the calcaneal tuberosity to the origin of the plantar fascia and labeled as the "landmark line." A parallel line was

Calcaneocuboid joint involvement in calcaneal fractures.

BACKGROUND: The reported incidence of calcaneocuboid joint (CCJ) involvement in calcaneal fractures varies considerably. It is largely unknown to what extent CCJ involvement accounts for outcome in these fractures. Therefore, the goal of this study was to analyze the incidence and effects of CCJ involvement in calcaneal fractures. METHODS: The clinical records of 106 patients, treated between 2000 and 2004, were reviewed for fracture classification, injury mechanism, surgical treatment, and complications. In a prospective cross-sectional study, 44 patients were assessed clinically (SF-36 Amer

Functional outcome after peroneal nerve injury.

The objective of this study was to describe muscle strength, ankle-foot orthosis (AFO) use, walking ability, participation and quality of life in patients with peroneal nerve injury. A historic cohort study (n=27) was performed with a median follow-up time of 61 months (inter quartile range 37-91). Muscle strength was assessed using the Medical Research Council scale. Perceived walking ability was assessed with the Walking Questionnaire. AFO use and problems in participation were assessed with a structured interview. The RAND-36 Health Survey was used to evaluate health-related quality of life

Complete avulsion of spinal cord and cauda equina: A case report.

Pure ligamentous flexion-distraction injuries of the lumbar spine are relatively rare and even less commonly associated with neurologic compromise. They are largely related to the use of lap belt restrains during motor vehicle accidents. We report a 19-year-old female backseat passenger wearing a lap belt who was involved in a head-on collision. On admission she was paraplegic, with a T12 sensory level and no motor and sensory function of S4-S5 (American Spinal Injury Association (ASIA) A). Plain X-ray and computerized axial tomography (CAT) spine showed a flexion-distraction injury at the L3-

Bilateral peroneus longus tendon rupture through a bipartite os peroneum.

Peroneus longus rupture with associated involvement of the os peroneum is an uncommon injury, and a small number of cases have been reported. Several mechanisms of injury have been suggested, but the most accepted theory is due to an inversion force placed on a cavovarus foot type. The sesamoid often becomes the focal point of the mechanical stresses and may fracture. Although the purpose of the sesamoid is to protect the tendon from rupture, the os peroneum may actually encourage fatigue (tear/rupture) under certain circumstances. Because this injury occurs at the cuboid notch, primary repair

Effects of direction and gender on lower limb biomechanics during forward lunges in badminton players: a preliminary analysis.

BACKGROUND: The lunge is a critical movement in badminton, facilitating rapid and efficient court coverage and enabling players to promptly reach the hitting position. While previous studies have examined the influence of lunge direction on lower limb biomechanics, the effects of gender have yet to be explored. Therefore, the purpose of this study was to investigate the combined effects of direction and gender on lower limb biomechanics during forehand and backhand forward lunges in amateur badminton players. METHODS: 16 male and 16 female amateur players were recruited. Kinematic and kinetic

Regular Nonsteroidal Anti-Inflammatory Drug Use Increases Stress Fracture Risk in the General Population: A Retrospective Case-Control Study.

Previous studies have shown that the use of nonsteroidal anti-inflammatory drugs (NSAIDs) is associated with increased stress fracture risk. This phenomenon has been studied predominantly in high-activity individuals, so data regarding the general population are limited despite the substantial economic and resource burden of stress fracture injuries within the general US population. Furthermore, our preclinical studies demonstrate that regular use of NSAIDs also diminishes the intrinsic ability of bone to resist fracture. To determine the association of regular NSAID use with stress fractures

Lumbosacral plexopathy due to pelvic hematoma after extracorporeal membrane oxygenation: A case report.

RATIONALE: Peripheral nerve injury related to vascular complications associated with extracorporeal membrane oxygenation (ECMO) is perhaps underappreciated. Compared to the well-described central nervous system complications of ECMO, brachial plexopathy and lumbosacral plexopathy have rarely been reported. We report this case to heighten awareness of lumbosacral plexus injury due to pelvic hematoma formation after ECMO. PATIENT CONCERNS: A 53-year-old woman developed a large pelvic hematoma with significant mass effect on intrapelvic structures after receiving lifesaving venoarterial ECMO for

Functional passive range of motion of individuals with chronic cervical spinal cord injury.

OBJECTIVE: Functional passive range of motion (PROM) requirements for individuals with cervical spinal cord injury (SCI) are clinically accepted despite limited evidence defining the specific PROM needed to perform functional tasks. The objective of this investigation was to better define the minimum PROM needed for individuals with cervical SCI to achieve optimal functional ability, and as a secondary outcome gather self-reported standardized functional data via the Spinal Cord Independence Measure-III (SCIM-III), and the Spinal Cord Injury Functional Index (SCI-FI). DESIGN: Observational co

Rehabilitation and long-term course of nontraumatic myelopathy associated with surfing.

A nontraumatic spinal cord injury related to surfing is called surfer's myelopathy. The case of a 26-yr-old man who became paraplegic after surfing without apparent traumatic events is described. Physical examination revealed a spinal cord injury at T12 according to the American Spinal Injury Association Impairment Scale A. The initial magnetic resonance image revealed a fusiform swelling of the spinal cord from T7-8 to the conus, which was hyperintense on T2-weighted images. After 6 mos of rehabilitation, the patient was followed for more than 1 yr after onset. He became able to walk with kne

Rare case of medial subtalar dislocation from a low-velocity mechanism.

BACKGROUND: Trauma to the foot and ankle from inversion injury accounts for a considerable number of Emergency Department (ED) visits. Acute subtalar dislocations are rare injuries most commonly caused by high-velocity mechanisms that can produce long-term complications if not reduced promptly. The subtleties of the dislocation on plain films can be easily overlooked in a fast-paced environment like the ED, especially if the patient presents with an atypical mechanism. OBJECTIVES: The epidemiology, pathophysiology, plain X-ray findings, management, and prognosis of medial subtalar dislocation

Brain-derived neurotrophic factor and peripheral nerve regeneration: a functional evaluation.

The potential benefit of brain-derived neurotrophic factor (BDNF) on motor-nerve regeneration after transection injury in 24 adult rats was evaluated after entubulation repair. Gait analysis for ankle angle and tension transduction device (TTD) strain-gauge measurements yielded functional evaluation of regeneration. The BDNF (15 mg/mL) or phosphate buffered saline (control) was injected into the silicone elastomer (Silastic) channel. Gait analysis performed 0, 2, 4, 6, 10, and 12 weeks after injury demonstrated a significant difference between uninjured and injured legs of 23 and 43 degrees, r

The Effects of External Lower Limb Weight or Pressure Application on Human Knee Joint Proprioception in Resting and Fatigue Conditions: A Randomized Trial.

Objectives: Knee proprioception is affected by many biomechanical and physiological factors. Often, during training, rehabilitation, or specific sport requirements, weight or pressure is applied to the foot. However, it is not clear if such applications affect knee proprioceptive acuity. This study examined whether the application of an external weight (3 kg) or of pressure (120 mmHg) at a level above the ankle joint would affect knee proprioception in an open kinematic chain movement in resting or fatigue conditions. Methods: Participants included active young men (n = 7) and women (n = 15),

Short-Term Outcome of Tension Band Wiring for Medial Malleolar Fractures.

INTRODUCTION: The structural integrity of the medial malleolus is critical for maintaining the stability of the ankle mortise. While various internal fixation strategies exist, tension band wiring (TBW) leverages the principle of dynamic compression by converting tensile forces into compressive forces during early mobilization. This technique remains highly relevant in clinical practice, particularly for addressing small fracture fragments and transverse patterns. MATERIALS AND METHODS: This prospective observational study evaluated 20 skeletally mature patients presenting with closed Herscov
