REM sleep episodes during the maintenance of wakefulness test in patients with sleep apnea syndrome and patients with narcolepsy.

Twelve patients with sleep apnea, 12 narcoleptic patients, and 10 controls were given 20-min opportunities to remain awake while sitting comfortably. Test sessions were administered at 10:00, 12:00, 14:00, 16:00, and 18:00. Apneic and narcoleptic subjects were less capable of maintaining wakefulness than controls. Patients with sleep apnea had an average of 1.4 daytime rapid eye movement (REM) episodes with the peak incidence at 14:00. Narcoleptics also had sleep onset REM periods (mean of 2.7), whereas none of the controls had REM episodes during the daytime testing. Narcoleptic and control g…

Conventional and novel hypoxemia-related indices for obstructive sleep apnea severity and incident hypertension: a prospective cohort study.

BACKGROUND AND OBJECTIVE: Obstructive sleep apnea (OSA) is associated with hypertension (HTN), but the apnea-hypopnea index (AHI) and oxygen desaturation index (ODI) may not fully characterize nocturnal hypoxemia relevant to HTN development. We evaluated the associations of the sleep breathing impairment index (SBII) and hypoxic burden (HB) with incident HTN and compared their discriminative performance with AHI and ODI. METHODS: This prospective study included 601 participants with OSA (AHI ≥ 15 events/h) from Sleep Heart Health Study who were free of HTN at baseline. SBII, HB, AHI and ODI w…

Beyond apnea: Early caffeine therapy and the risk of acute kidney injury in very low birth weight infants.

IntroductionAcute kidney injury is a frequent but often underrecognized complication in very low birth weight preterm infants. In addition to its established respiratory benefits, caffeine has been suggested to have potential renal-protective effects. The aim of this study was to evaluate the association between early caffeine administration and the incidence of acute kidney injury in preterm infants with a birth weight of 1250g or less.MethodsThis retrospective cohort study included 156 preterm infants with a gestational age of 30 weeks or less and a birth weight of 1250g or less. Infants wer

Single-cell and transcriptomic profiling reveal stemness-driven immune evasion in obstructive sleep apnea (OSA) associated lung cancer.

Obstructive sleep apnea (OSA) is characterized by recurrent intermittent hypoxia (IH) and has been increasingly associated with lung cancer incidence and mortality. However, how IH-related biological programs relate to immune remodeling, stemness-associated phenotypes, and therapeutic resistance in lung cancer remains incompletely understood. We integrated single-cell RNA sequencing data from IH-exposed murine lung tissues (GSE301350) with bulk transcriptomic datasets from TCGA-LUAD and GSE31210 to examine hypoxia-associated cellular and transcriptional patterns. Stemness was quantified using …

White matter microstructure differences in obstructive sleep apnea severity groups assessed by diffusion tensor metrics and biophysical modeling.

Obstructive sleep apnea (OSA) is a complex condition characterized by repeated episodes of upper airway collapse during sleep, leading to chronic intermittent hypoxia. Diffusion magnetic resonance imaging (dMRI) techniques offer sensitivity to white matter (WM) microstructure changes. 150 individuals from a community-based study underwent one-night nocturnal polysomnography (NPSG), cognitive assessments, and brain structural MRI. Gaussian and non-Gaussian diffusion signal changes in WM tracts were quantified with diffusion tensor metrics as (DTI) and Diffusion Kurtosis Imaging (DKI), respectiv…

METTL14 Regulates Adipose Tissue Macrophage Polarization via JAK2/STAT3 Pathway to Attenuate Obstructive Sleep Apnea-Related Systemic Inflammation.

The role of m6A RNA methylation in obstructive sleep apnea (OSA)-related metabolic dysfunction and systemic inflammation is unknown. We aimed to identify the key m6A regulator involved in OSA-induced adipose tissue inflammation and explore the underlying mechanisms. The expression of m6A methylation regulators was measured and their associations with systemic inflammation indicators were analyzed in patients with OSA. A mouse model of OSA was established with high-fat diet feeding and chronic intermittent hypoxia (CIH) treatment. The histological alterations of epididymal white adipose tissue …

Machine learning for scalable obstructive sleep apnea risk screening using digital phenotyping from wearable devices and clinical scales.

Obstructive sleep apnea (OSA) is common yet frequently underdiagnosed, partly because overnight polysomnography (PSG) is logistically burdensome and access to specialized testing is limited. We aimed to develop machine-learning models for OSA risk screening using multimodal digital phenotyping from consumer-grade wearable devices, smartphone-based assessments, and clinical scales. We enrolled 338 participants and collected data over four weeks. After preprocessing, 107 features were derived from wearable-derived physiological and activity measures, smartphone-based records, and questionnaire-b…

Artificial Intelligence Diagnosis of Obstructive Sleep Apnea Using Overnight Pulse Oximetry: A Systematic Review and Bayesian Meta-Analysis.

BACKGROUND: Obstructive sleep apnea (OSA) affects 38% of the population, yet over 90% of cases remain undiagnosed. The gold standard for diagnosis, polysomnography, requires specialized equipment and trained personnel, making it inaccessible in primary care and acute settings. With artificial intelligence (AI) advancements, oximetry-based AI models have emerged as potential alternatives for OSA diagnosis. OBJECTIVE: This meta-analysis aims to evaluate the diagnostic accuracy of AI models trained on pulse oximetry readings in diagnosing OSA. METHODS: A systematic search was conducted across M…

Understanding gut microbiota dysbiosis as a plausible link between obstructive sleep apnea (OSA), viral infections, and lifestyle diseases.

Obstructive sleep apnea (OSA) is a multifactorial disorder which is influenced by intermittent hypoxia, sleep fragmentation, and systemic inflammation. Recent evidence suggests that lifestyle diseases and viral infections further exacerbate OSA severity through common inflammatory and metabolic pathways. Parallelly, gut dysbiosis has gained recognition as a key mediator which links respiratory, metabolic, and infectious disease processes via the gut-lung axis. This review explores the convergent role of gut microbial dysbiosis across OSA, lifestyle-associated comorbidities such as obesity, dia…

Risk of new-onset obstructive sleep apnea up to 4.5 years after COVID-19 in the urban population.

Obstructive sleep apnea (OSA) is linked to cardiovascular, metabolic, and cognitive morbidity. Although COVID-19 has been associated with long-term respiratory and neurological sequelae, its role in precipitating new-onset OSA remains unclear. We evaluated whether SARS-CoV-2 infection increases risk of developing OSA up to 4.5 years post-infection and how risk varies by hospitalization status, demographics, comorbidities, and vaccination status. This retrospective cohort study used electronic health records from the Montefiore Health System in the Bronx. Adults tested for SARS-CoV-2 between Ma…

An EEMD-derived slow-wave activity metric improves association with obstructive sleep apnea severity beyond conventional N3 staging.

STUDY OBJECTIVES: Conventional sleep staging relies on visual criteria, overlooking the contiguous transitions of sleep configuration and individual variability of brainwaves. Consequently, whether conventional N3 sleep metrics sufficiently capture sleep architecture alterations associated with heterogeneous sleep disorders such as obstructive sleep apnea (OSA) remains uncertain. Our objective was to determine whether slow-wave sleep (SWS) derived from an elevated slow-wave activity (SWA)-based approach exhibits stronger associations with the apnea-hypopnea index (AHI) than conventional N3 sle…

Sex differences in sleep architecture and nocturnal oxygenation across obstructive sleep apnea severity: an observational hospital-based study.

BACKGROUND: Sex differences in obstructive sleep apnea (OSA) may extend beyond the apnea-hypopnea index (AHI), the standard measure of OSA severity, and involve differences in sleep architecture and nocturnal oxygenation. We compared these characteristics between women and men across OSA severity categories in a large Chinese clinical cohort. METHODS: This single-center, hospital-based observational study included adults referred for overnight polysomnography at a sleep center in Fujian, China. OSA severity was classified as mild, moderate, or severe according to AHI. Outcomes included total …

Disease-stage-specific immunometabolic remodeling in pediatric obstructive sleep apnea: a single-cell transcriptomic atlas of adenoid tissue.

Hypertrophied adenoids in children can impair breathing and lead to obstructive sleep apnea (OSA), often accompanied by abnormal growth and weakened stamina and immunity. However, the cause of the pathological transformation in these originally immune-enhancing lymphoid tissues remains unclear. Our study provides the first single cell transcriptomic and immune repertoire atlas of adenoids from normal snoring to mild, moderate, and severe OSA, and identified markedly asynchronous functional modules, transcriptional regulatory networks and intercellular communications during the progression of O…
