Spinal metastases in breast cancer: single center experience.

OBJECTIVE: Metastases to the spine are a common manifestation of breast cancer leading to considerable reduction in the patient's quality of life. Physicians must consider the different treatments available to decrease pain, reduce tumor burden, and ensure spinal stability to prevent neurological compromises. The first objective of this study is to analyze the epidemiology and outcomes of patients with spinal metastases from breast cancer and describe changes over time in these lesions. The second objective is to establish the current treatment of spinal metastases in this type of cancer. MET

Detection and survival of colorectal cancer from a 2 week wait service.

AIM: To determine detection strategies for colorectal cancer (CRC) and to analyse subsequent survival from a 2 week wait (2WW) service. METHOD: Retrospective analysis of 2WW and hospital CRC databases from January 2006 to July 2009. Survival was assessed using Kaplan Meier survival curves and Cox's proportional hazard models. Proximal cancers were those proximal to and including the splenic flexure. RESULTS: From 1725 patients seen in a 2WW clinic, 108 were identified with CRC. Median follow-up in survivors was 514 days (interquartile range 160-788 days). Of 23 patients investigated for iron

Interventional Pain Procedures: A Narrative Review Focusing on Safety and Complications. Part 3 - Sympathetic and Ganglion-Targeted Techniques.

Interventional procedures that target the sympathetic nervous system and specific autonomic and sensory ganglia are widely used for refractory visceral, neuropathic, and cancer-related pain. They can provide substantial, often durable analgesia but carry a small yet clinically important risk of serious adverse events. This is the third part of our narrative-review series on the safety of interventional pain procedures, following Part 1 (spinal pain injections) and Part 2 (procedures for back pain), both published in the Journal of Pain Research. It is a structured but non-systematic narrative

Amelioration of Chemotherapy Induced Neuropathic Pain using Novel Nicotinic Acid Derivatives with possible HCN channel binding ability.

One of the major debilitating side effects of cancer chemotherapy is neuropathic pain, which results from abnormal neural signaling and significantly diminishes patients' quality of life. Paclitaxel (PT), a widely used chemotherapeutic agent, induces peripheral nerve degeneration, leading to the development of painful neuropathy. In this study, PT was used to establish a mouse model of chemotherapy-induced peripheral neuropathy. Hyperpolarization-activated cyclic nucleotide-gated (HCN) channels play a key role in regulating neuronal pacemaker activity. The HCN current (Ih) promotes repetitive

Pharmacological evaluation of physcion as a TRPV1 inhibitor with multimodal analgesic efficacy in experimental pain models.

BACKGROUND: Pain serves as a vital protective mechanism triggered by tissue damage. While NSAIDs and opioids offer relief, their prolonged usage is hindered by adverse effects. Developing analgesics with fewer side effects is crucial for effective pain treatment. The TRPV1 channel is a key target for pain relief, with its inhibitors effectively reducing hyperalgesia in animals. This research utilized virtual screening to identify TRPV1-selective natural compounds for potent analgesic properties. RESULTS: The physcion exhibited the notable affinity for TRPV1 compared to the compounds examined.

Continuous Epidural Versus Non-Epidural Pain Management After Minimally Invasive Esophagectomy: A Real-Life, High-Case-Load Center Experience.

Background/Objectives: Esophagectomy is a key component of esophageal cancer treatment, with minimally invasive esophagectomy (MIE) increasingly replacing open esophagectomy (OE). Effective postoperative pain management can be achieved through various analgesic modalities. This study compares the efficacy of thoracic epidural anesthesia (TEA) with non-TEA methods in managing postoperative pain following MIE. Methods: A retrospective review was conducted on 110 patients who underwent MIE between 2018 and 2023. 1. TEA vs. 2. intravenous patient-controlled analgesia (PCA) alone vs. 3. transversus

Negative pressure dressings and the impact on pain management on thoracic incisions in patients undergoing oesophagectomy: a pilot study.

BACKGROUND: Pain control is recognised as a crucial post-operative measure for patients undergoing oesophagectomy with a thoracotomy incision for oesophageal cancer, where ineffective breathing due to pain is directly correlated with increased morbidity. The analgesic benefits of negative pressure wound therapy (NPWT) appear to be a relatively new and emerging finding. This pilot study aims to investigate the effects of NPWT on post-operative pain control and determine the feasibility of a larger trial. METHOD: Ten consecutive patients undergoing oesophagectomy were prospectively enrolled to

Iatrogenic Side Effects of Pain Therapies.

Pain regimens, particularly for chronic cancer and noncancer pain, must balance the important analgesic benefits against potential risks. Many effective and frequently used pain control regimens are associated with iatrogenic adverse events. Interventional procedures can be associated with nerve injuries, vascular injuries, trauma to the spinal cord, and epidural abscesses. Although rare, these adverse events are potentially catastrophic. Pharmacologic remedies for pain must also consider potential side effects that can occur even at therapeutic doses of over-the-counter remedies such as parac…

Animal Venom Peptides Cause Antinociceptive Effects by Voltage-gated Calcium Channels Activity Blockage.

Pain is a complex phenomenon that is usually unpleasant and aversive. It can range widely in intensity, quality, and duration and has diverse pathophysiologic mechanisms and meanings. Voltage-gated sodium and calcium channels are essential to transmitting painful stimuli from the periphery until the dorsal horn of the spinal cord. Thus, blocking voltage-gated calcium channels (VGCCs) can effectively control pain refractory to treatments currently used in the clinic, such as cancer and neuropathic pain. VGCCs blockers isolated of cobra Naja naja kaouthia (α-cobratoxin), spider Agelenopsis apert
