Postoperative gas gangrene.

Gas gangrene is a rare complication of elective surgery. Caused by several species of Clostridia, it is an acute, highly invasive necrotizing infection of muscle and fascia often leading to coma and death. Gas gangrene should be suspected if the patient has unremitting pain, high temperature, tachycardia, and prostration. Edema, brown watery exudate emanating from he wound, evidence of gas in tissues, and gram-positive rods on smear support the clinical diagnosis. Rapid surgical decompression and debridement is the basis of treatment. Adjuvant antibiotic, hyperbaric oxygen, antitoxin, local, a

Old burns and pregnancy.

Burns rarely cause problems in subsequently occurring pregnancies. Occasionally abdominal wall scarring is so dense and extensive that consequent maternal pain in the second half of pregnancy demands surgical decompression. This does not usually precipitate labour and the resultant raw areas should be covered with split skin autografts. It is exceptional for burns to destroy the entire breast disc; residual breast mound development occurs during puberty, and hypertrophy and engorgement during and after pregnancy. Although absence of the nipple-areola complex precludes breast feeding, distortio

[Primary phlebitis of the arm].

Primary phlebitis or sprain phlebitis in the upper member are caused by a compressive mechanism in the thoraco-brachial passage. Thrombosis most often originates in the area of the cost-clavicular hinge, and spreads towards the extremity of the member. In this series, 19 cases were treated: 8 recent cases of phlebitis, in which results were all too often imperfect (5 failures), 11 post-phlebitic syndromes, in which there were 9 good results. The surgical procedure which we therefore recommend aims at the decompression of the collateral circulation flowing around the thrombosed area: this shoul

[Acute tuberculous meningitis in children. A report on 4 cases (author's transl)].

Four children were treated for acute tuberculous meningitis. One child died and 2 others were left with severe neurological sequellae. The diagnostic and clinical signs of tuberculous meningitis are reviewed. Treatment includes the administration of an association of INH-rifampicine and ethambutol orally, or INH-ethambutol-ethionamide intravenously when oral administration is impossible. Intrathecal injections of rifamycine SV can be given for acute forms. Corticoids have only one indication: intracranial hypertension with cerebral edema, which requires surgical decompression if no improvement

The craniotubular bone modeling disorders: a neurosurgical introduction to rare skeletal dysplasias with cranial nerve compression.

The craniotubular bone modeling disorders are a group of skeletal dysplasias which involve predominantly the skull and long bones. The cranial involvement includes both external facial deformities and internal bony overgrowth about the cranial foramina and fissures. The latter commonly leads to cranial nerve compression, particularly of the optic nerve. Craniotomy and cranial nerve decompression can give substantial benefit to selected patients with these diseases but the severe bony abnormalities create technical problems which prevent surgery in some extreme cases. These disorders are geneti

Anaesthetic and surgical management of gastric perforation secondary to a diving incident: a case report.

Gastric perforation secondary to barotrauma is a rare surgical condition which may manifest as an acute abdomen and potentially lead to complications such as pneumoperitoneum. A 50-year-old, healthy, experienced diving instructor was transported to our emergency department for an acute abdomen and severe dyspnoea after a diving incident. Clinical suspicion combined with computed tomography scanning lead to the diagnosis of linear rupture of the stomach. Exsufflation of the abdominal cavity was performed in the emergency department and then the patient was sent to the operating room for emergen

Pedicle Screw Fixation for Hemiplegia and Bilateral Hypoesthesia Due to C2-C3 Spinal Fracture Dislocation: A Case Report.

CASE: A 48-year-old man fell from a tree and presented to the emergency department with right-sided full hemiplegia and C3 bilateral hypoesthesia. Imaging was remarkable for a C2-C3 fracture-dislocation. The patient was effectively managed surgically with a posterior decompression and 4-level posterior cervical fixation/fusion that included pedicle screws in the axis fixation and lateral mass screws. The reduction/fixation remained stable, and the patient regained full lower extremity function and demonstrated functional upper-extremity recovery at three-year follow-up. CONCLUSIONS: C2-C3 fra

Scoliosis and spina bifida contributing to strangulation of a hiatus hernia: a case report.

Hiatus hernia is defined as a trans-diaphragmatic protrusion of the intra-abdominal contents through the oesophageal hiatus into the mediastinum. Surgical repair is indicated in symptomatic patients, with some patients presenting emergently with strangulation and ischaemia of hernial contents. In this situation, emergent decompression and surgical repair is indicated. Although it has been suggested previously that kyphoscoliosis may contribute to development and progression of hiatus hernia, there are no published reports of obstruction being caused by severe spinal deformity. A 67-year-old ma

Mal-union of sustentaculum tali fracture with talo-calcaneal coalition leading to tarsal tunnel syndrome: A case report.

Displaced isolated fractures of sustentaculum tali are rare. Inadequate treatment of these injuries can rarely lead to non-union or mal-union and in most cases are treated non-surgically. We report a unique case of undiagnosed mal-union of sustentaculum tali in the setting of underlying tarsal coalition that resulted in symptoms of tarsal tunnel. Osteotomy and excision of the mal-united fragment and coalition along with decompression of the tarsal tunnel was performed. The patient had immediate improvement in pain and the paraesthesia recovered by the end of 6 weeks post-operatively. The Foot

Update on Shunt Surgery.

BACKGROUND: Bleeding from esophagogastric varices is a life-threatening complication from portal hypertension. It occurs in 15% of patients and has a mortality rate of 20-35%. SUMMARY: The primary therapy for variceal bleeding is medical. In cases of recurrent bleeding, a definitive therapy is required. In cases of parenchymal decompensation, liver transplantation is the causal therapy, but if liver function is preserved, portal decompression is the therapy of choice. The use of the transjugular intrahepatic portosystemic shunt (TIPS) has achieved widespread acceptance, although evidence for

A rare and severe case of pronator teres syndrome.

We present the case of a patient with severe symptoms of proximal forearm median nerve neuropathy. Over the course of 5 years his condition progressed to encompass rare features of combined pronator teres syndrome (PTS) and anterior interosseous nerve syndrome (AINS). The aetiology was found to be pronator teres compression and was managed successfully by surgical decompression. Proximal forearm median nerve compression should be considered as a continuum with two classic endpoints. At one end of the spectrum pure PTS presents with solely or mainly sensory symptoms, whereas at the other end AI

Update on traumatic acute spinal cord injury. Part 2.

The aim of treatment in acute traumatic spinal cord injury is to preserve residual neurologic function, avoid secondary injury, and restore spinal alignment and stability. In this second part of the review, we describe the management of spinal cord injury focusing on issues related to short-term respiratory management, where the preservation of diaphragmatic function is a priority, with prediction of the duration of mechanical ventilation and the need for tracheostomy. Surgical assessment of spinal injuries based on updated criteria is discussed, taking into account that although the type of i

Acute transverse colon volvulus with secondary gastric isquemia. Case report.

Acute colonic volvulus accounts for 10% of all intestinal obstructions being the transverse colon volvulus an exceptional localization (2-4%). Late diagnosis is made as there are no pathognomonic clinical or radiological findings for this pathology. We present the case of an 81 year-old male with acute transverse colon volvulus that involved the gastric antrum causing irreversible ischemia. Subtotal gastrectomy, subtotal colectomy and reconstruction with Y en Roux gastrojejunostomy and ileosigmoid anastomosis was performed given the good overall status of the patient. [Nerve injuries after elbow luxation fractures in childhood : Indication and timing for surgical revision].

BACKGROUND: Supracondylar humerus fractures are very common in pediatric populations. In cases of dislocated fractures, closed reduction and percutaneous K‑wire fixation is recommended. Initially, 10-16 % of the patients also present lesion of the ulnar nerve as well as median nerve palsy in 6.4% and radial nerve palsy in 2.6 % of cases, respectively. METHOD: We present the case of a 10-year-old boy with a dislocation of the elbow and fracture of the medial epicondyle. After closed reduction and K‑wire fixation from medial, he presents a median nerve palsy. That a lesion of the ulnar nerve is

[Degenerative adult scoliosis].

Adult scoliosis is a complex three-dimensional rotational deformity of the spine, resulting from the progressive degeneration of the vertebral elements in middle age, in a previously straight spine; a Cobb angle greater than 10° in the coronal plane, which also alters the sagittal and axial planes. It originates an asymmetrical degenerative disc and facet joint, creating asymmetrical loads and subsequently deformity. The main symptom is axial, radicular pain and neurological deficit. Conservative treatment includes drugs and physical therapy. The epidural injections and facet for selectively b

Grave's disease with transverse and sigmoid sinus thrombosis needing surgical intervention.

Thrombosis of venous sinuses associated with thyrotoxicosis is rare, and isolated transverse and sigmoid sinus thrombosis is rarer and reported only once previously. We present a case of Graves disease, who suffered unilateral sigmoid and transverse sinus thrombosis with intracranial hemorrhage. A 42-year-old female, a diagnosed case of Graves disease, presented to us with headache, drowsiness, and hemiparesis. Computed Tomography revealed a large right temporo-parieto-occipital venous infarct. The patient needed surgical intervention in the form of decompressive craniotomy following which she

Gastric necrosis complicating acute gastric dilatation after Nissen fundoplication.

A woman in her 70s was referred for the management of complex intestinal fistulation, which had developed as a postoperative complication of laparoscopic Nissen fundoplication. Surgical treatment of a jejunocolocutaneous fistula and formation of a double-barrelled ileocolostomy was undertaken uneventfully. Routine postoperative nasogastric intubation was not undertaken. On the seventh postoperative day she had nausea and hiccupping. A chest x-ray demonstrated a massively dilated stomach which was decompressed with a nasogastric tube. The patient sustained a cardiac arrest and was resuscitated.

Intussusception and colonic ischemia in portal hypertension: a case report.

Intestinal intussusception is a relatively uncommon occurrence in adults in comparison to pediatric patients. While the management of intussusception in children is frequently decompression of the involved segment, adults often require surgical resection secondary to frequent association with neoplastic lead points. A less common reason for surgical removal of an intussuscepted segment in adults is the development of ischemic colitis. The authors present an unusual case of adult intussusception with associated ischemic colitis in a patient with portal hypertension awaiting liver transplantatio

Craniocervical instability in an infant with partial sacral agenesis.

UNLABELLED: Sacral agenesis is an uncommon condition characterised by total or partial absence of the sacrum. The association of this condition with craniocervical junction abnormalities is extremely rare. CASE REPORT: We describe a 3-year-old girl who, at the age of 8 months, while being investigated for short stature, was found to have significant craniocervical instability associated with anomalies of the odontoid peg. In addition to this she had partial sacral agenesis but genetic tests showed a normal karyotype. Due to the inherent difficulty of surgical fixation and immobilisation in an
