Improvement of severe alopecia areata in an adolescent patient on upadacitinib.

Recently, alopecia areata (AA) treatment via the Janus kinase (JAK)-signal transducer and activator of transcription pathway has been reported. However, as baricitinib, a JAK1/2 inhibitor is only approved for adult patients, children, and adolescent patients still lack treatment options. We present a case that showed improvement of severe AA in an adolescent patient on upadacitinib, which has been approved by the Food and Drug Administration (FDA) for use in patients with rheumatoid disease or atopic dermatitis (AD) in children aged 12 years or older and weighing 40 kg or more. Herein, we sugg

Myeloid dendritic cells are increased in the lesional skin and associated with pruritus in patients with prurigo nodularis.

The number of dermal mDCs (CD11c+ cells) was increased with itch intensity (r = 0.886, p = 0.003) using immunofluorescence (IF). On IF, CD11c+ mDCs expressed IL-31 in lesional PN skin. Fluorescence in situ hybridization combined with IF also confirmed IL-31 mRNA expression by mDCs in PN lesion. Higher population of colocalization of CD11c+ mDCs expressing IL-31 mRNA were more than CD68+ macrophages and CD3+ T cells in consecutive sections of PN skin lesion. Psychopathological symptoms in dermatology: A basic approach toward psychocutaneous disorders.

Dermatological conditions impact not only an individual's physical body but also their psychological health. Similar to how cutaneous conditions can affect one's psychological health, worsening psychological conditions can exacerbate or even induce dermatological conditions. There are four common psychiatric pathologies typically found in dermatology practices: depressive symptoms, anxiety symptoms, obsessive-compulsive disorder behaviors, and psychosis. Common cutaneous disorders associated with these psychopathological symptoms include, but are not limited to, psoriasis, acne vulgaris, atopi

Janus Kinase Inhibitors: A Review of Their Application in the Vitiligo.

The small-molecular inhibitors targeted JAks (JAK inhibitors) could modulate the cytokines- mediated signaling via the JAK-STAT pathway, which plays an important role in immune regulation and cell proliferation. The JAK inhibitors were previously designed and synthesized to treat diseases involving the hematologic and immune system. Increasing evidence shows that they are quite effective in Atopic Dermatitis (AD), Alopecia Areata (AA), psoriasis, vitiligo, and other autoimmuneinduced dermatologic conditions. Currently, many JAK inhibitors possessing anti-vitiligo activity are being investigate

Ultraviolet A1 phototherapy for the treatment of localized scleroderma.

Ultraviolet (UV)A1 phototherapy is effective for T-cell-mediated skin diseases such as atopic dermatitis and mast cell-mediated skin diseases such as mastocytoma. UVA1 phototherapy is also effective against the sclerotic lesions of systemic sclerosis and morphea. Currently, in Japan, access to UVA1 phototherapy is limited because the UVA1 phototherapy device has not yet been approved. On the basis of our experience, we report three patients with localized scleroderma who responded successfully to UVA1 phototherapy. Efficacy was assessed by histological analysis and elastography. UVA1 successfu

[The skin and rheumatism].

The skin is commonly affected in chronic inflammatory disorders and may act as a visual marker for internal or systemic inflammation. Frequent inflammatory skin diseases, like psoriasis and atopic dermatitis (AD), are associated with rheumatic and inflammatory bowel diseases. Metabolic, mental and cardiovascular comorbidity are frequent consequences of chronic inflammation. Further intersections between skin and joints are connective tissue diseases (collagenoses) and can be observed in complex diseases, e.g. systemic lupus erythematosus. Clinically, these diseases range from predominant cutan

Unchecked immunity: a unique case of sequential immune-related adverse events with Pembrolizumab.

BACKGROUND: Immune checkpoint inhibition has dramatically transformed the treatment of malignant melanoma. With increasing use, their unique spectrum of immune-mediated toxicity has become apparent. CASE PRESENTATION: We describe a case of sequential immune-related adverse events (irAEs) in a patient with metastatic melanoma treated with single-agent anti-programmed cell death-1 (PD-1) therapy, pembrolizumab. Although numerous cases of irAEs have been reported, sequential multi-organ involvement, including progressive atopic dermatitis, vitiligo, autoimmune nephritis, autoimmune hepatitis, an

Vitamin D in allergic disease: shedding light on a complex problem.

Vitamin D is unique among nutritional factors because the intake of this special vitamin represents the sum of vitamin D obtained from diet, nutritional supplements, and endogenous production after exposure to sunlight. The current recommended nutritional intake requirements reflect needs based on its established role in calcium absorption and bone health. However, recent studies have revealed that vitamin D has important functions in the immune system and might influence the course of immune-mediated disorders, including atopic dermatitis and asthma. This review discusses the scientific ratio

Resistant itchy lesions in a young man.

Case study A man, 18 years of age, presented with a 6-month history of a widespread itchy rash, more intense at night, which had appeared 1 month after returning from a holiday to the Dominican Republic. The rash initially appeared as white patches affecting the fingers. The patient reported unprotected heterosexual intercourse while on holiday. There was no previous or family history of atopic dermatitis, however, the patient's sister had recently begun to experience similar symptoms. Clinical examination revealed scattered excoriated papules over the trunk (Figure 1), limbs and hands (Figure

Procedures and drugs in pediatric dermatology: iatrogenic risks and situations of concern.

Over the past several decades, improved technologies used in the care of hospitalized and outpatient pediatric populations have resulted in a decreased but still significant number of iatrogenic injuries. Children at the highest risk for cutaneous injury include those with the most immature skin barriers, such as neonates younger than 32 weeks of gestational age. Additional risk factors include low birth weight, increased length of hospital stay, and indwelling instrumentation. Also at risk are older children with compromised skin barriers owing to infectious disease (staphylococcal scalded sk

[What's new in dermatological treatments?].

This article aims to identify and collect publications related to treatments in dermatology published during the last 12 months (October 2007 - October 2008). The large number of such papers leads to a choice "marked" by the author decision. Randomized clinical trials, cohorts, metaanalysis, guidelines have been selected in priority. However, non randomized studies, even informative clinical cases have been included. Overall, 26 articles have been selected concerning different aspects of therapeutics in dermatology and involving the following disorders : psoriasis, hypereosinophilic syndrome,

Adverse events occurring after smallpox vaccination.

We reviewed the literature on adverse events reported to occur after smallpox vaccination. Nearly one-half of the United States population is vaccinia-naïve and may be at risk for development of serious adverse events. We describe the clinical features of postvaccinial central nervous system disease, progressive vaccinia, eczema vaccinatum, accidental implantations, "generalized vaccinia," and the common erythematous and/or urticarial rashes. In the 1960s, death occurred approximately once in every million primary vaccinations, with fatalities resulting from progressive vaccinia, postvaccinial

Spontaneous histamine release from washed leukocytes and whole blood in atopic and non-atopic individuals.

The ability of basophils from patients with allergic rhinitis, extrinsic asthma, chronic idiopathic urticaria and atopic dermatitis to release histamine spontaneously in vitro was studied. Spontaneous release in the presence and absence of 30% D2O was investigated from both washed leukocytes and whole blood. Compared with controls histamine release (HR) from washed leukocytes was significantly enhanced in allergic rhinitis patients only, whereas in the other groups only a certain percentage of patients was found to have high spontaneous HR. In whole blood experiments HR in all groups was withi

Emerging roles of lipocalin-2-mediated neuroimmune interactions in chronic pain and itch.

Neuroimmune interactions serve as a core regulatory node of chronic pain and pruritus and a key target for clinical intervention. Lipocalin-2 (LCN2), a member of the lipocalin superfamily, is a multifunctional protein widely expressed in various tissues and cells. LCN2 exerts diverse biological effects by regulating iron metabolism, mediating inflammatory responses, and participating in signal transduction pathways. In recent years, accumulating evidence has indicated that LCN2 plays a crucial role in the pathogenesis of chronic pain and pruritus through neuroinflammation, neuron-glia interact

Remission of chronic actinic dermatitis on baricitinib: A case report.

Chronic actinic dermatitis (CAD) is an immune-mediated photodermatosis characterised by eczematous, pruritic changes to sun-exposed skin. The pathophysiology of CAD is poorly understood, with current explanations including a hypersensitivity reaction and cross-reactivity to contact allergens. The disease is often refractory to immunosuppressive treatment and has a marked impact on patient quality of life. Janus kinase inhibitors (JAKi) are a novel class of small molecules licenced for the management of certain inflammatory conditions, including atopic dermatitis We present the case of a 69-yea

Analysis of gene expression profiles of multiple skin diseases identifies a conserved signature of disrupted homeostasis.

Triggers of skin disease pathogenesis vary, but events associated with the elicitation of a lesion share many features in common. Our objective was to examine gene expression patterns in skin disease to develop a molecular signature of disruption of cutaneous homeostasis. Gene expression data from common inflammatory skin diseases (eg psoriasis, atopic dermatitis, seborrhoeic dermatitis and acne) and a novel statistical algorithm were used to define a unifying molecular signature referred to as the "unhealthy skin signature" (USS). Using a pattern-matching algorithm, analysis of public data re

The pH of the skin surface and its impact on the barrier function.

The 'acid mantle' of the stratum corneum seems to be important for both permeability barrier formation and cutaneous antimicrobial defense. However, the origin of the acidic pH, measurable on the skin surface, remains conjectural. Passive and active influencing factors have been proposed, e.g. eccrine and sebaceous secretions as well as proton pumps. In recent years, numerous investigations have been published focusing on the changes in the pH of the deeper layers of the stratum corneum, as well as on the influence of physiological and pathological factors. The pH of the skin follows a sharp g
