Targeting tumor necrosis factor-alpha as a potential therapy in inflammatory skin diseases.

Tumor necrosis factor-a (TNFalpha) plays an important role in the initiation and persistence of inflammation in a variety of skin disorders. The availability of selective TNFalpha blockers has provided new therapeutic opportunities. TNFa inhibitors have been employed with success in treating psoriasis in controlled trials, although further studies are warranted to better define which subgroup of patients can benefit most from this treatment and modalities of combination therapy. In addition, an increasing number of case reports indicates that anti-TNFalpha agents are of value in the treatment

New developments in contraceptive drugs for use by women.

Since the oral contraceptive pill was first marketed in 1959, advances in contraceptive drugs for women have been limited to variations on the theme of oestrogen in combination with progestogen or progestogen alone. Alterations to the pill regimen, new progestogens and new delivery systems have increased choice but while these advances significantly improve acceptability, side effects and risks remain essentially unchanged. New ideas are in short supply. Antiprogesterones offer a new approach with a variety of dose regimens but their development has been seriously hampered by the politics asso

Towards mitochondrial gene therapy: DQAsomes as a strategy.

Mitochondrial dysfunction is a cause, or major contributing factor in the development, of degenerative diseases, aging, cancer, many cases of Alzheimer's and Parkinson's disease and Type II diabetes (D. C. Wallace, Science 283, 1482-1488, 1999). Despite major advances in understanding mtDNA defects at the genetic and biochemical level, there is no satisfactory treatment for the vast majority of patients available. Objective limitations of conventional biochemical treatment for patients with defects of mtDNA warrant the exploration of gene therapeutic approaches. However, mitochondrial gene the

A compartmental model for the ocular pharmacokinetics of cyclosporine in rabbits.

Studies were conducted in rabbits to determine the ocular distribution and elimination of cyclosporine, with the objective of developing a comprehensive pharmacokinetic model. Following a bolus dose into the anterior chamber, drug levels were measured in the aqueous humor, cornea, iris/ciliary body, lens, sclera, and conjunctiva. Cyclosporine was rapidly eliminated from the aqueous, but drug levels in ocular tissues persisted for in excess of 48 hours, particularly in the cornea and iris/ciliary body. The terminal elimination half life from these tissues was 45 hr and 30 hr, respectively, prov

Pharmacokinetics of 5-aminosalicylic acid from controlled-release capsules in man.

One gram single dose of Pentasa controlled-release capsules was administered to 24 healthy volunteers under fasting condition. Mean plasma 5-aminosalicylic acid (5-ASA) and acetyl 5-ASA concentrations peaked at 0.53 microgram.ml-1 and 1.33 micrograms.ml-1 from 3 to 4 hours following dosing, respectively. The half-lives of both compounds could not be determined as absorption of 5-ASA was continuous throughout the gastrointestinal tract. An average of 29.4% (CV: 27%) of the dose was excreted in the urine primarily as acetyl 5-ASA. Up to 91.1% of the dose was released from the capsules. Forty per

Tumor Targeting and Drug Delivery by Anthrax Toxin.

Anthrax toxin is a potent tripartite protein toxin from Bacillus anthracis. It is one of the two virulence factors and causes the disease anthrax. The receptor-binding component of the toxin, protective antigen, needs to be cleaved by furin-like proteases to be activated and to deliver the enzymatic moieties lethal factor and edema factor to the cytosol of cells. Alteration of the protease cleavage site allows the activation of the toxin selectively in response to the presence of tumor-associated proteases. This initial idea of re-targeting anthrax toxin to tumor cells was further elaborated i

Nano-Reagents in Acute Pancreatitis: Diagnostic, Therapeutic, and Theranostic Advances.

This narrative review synthesizes the latest advances in nano-diagnostic and therapeutic reagents for acute pancreatitis (AP), based on peer-reviewed experimental and preclinical studies published in recent years. AP endangers life through self-digestion of pancreatic tissue and a cascading systemic inflammatory reaction. In clinical practice, the existing diagnostic and therapeutic tools are limited by low sensitivity and insufficient targeting, so it is difficult to achieve the ideal diagnosis and treatment effect of AP. However, the emerging nanotechnology is expected to overcome these draw

Development and evaluation of madecassoside-loaded microemulsion for enhanced topical drug delivery.

Madecassoside, a major triterpenoid glycoside from Centella asiatica, has potential value for dermatological applications; however, its poor stratum corneum permeability is due to high hydrophilicity and large molecular size. This work focused on designing a microemulsion carrier to enhance the topical delivery of madecassoside. Formulation regions were first screened using pseudo-ternary phase diagrams, followed by preparation of madecassoside-loaded systems with different oil phases, surfactant HLB values, cosurfactant compositions, and drug loadings. The resulting formulations were characte

Traditional Chinese medicine derived exosome-like nanovesicles in wound repair and skin regeneration.

Skin injuries, including acute wounds, burns, and chronic ulcers, pose significant clinical challenges due to their potential to cause delayed healing and functional impairment. Exosome-like nanovesicles (ELNVs) derived from traditional Chinese medicinal (TCM) herbs have recently emerged as promising natural agents for skin repair and regeneration. These nanoscale vesicles combine the structural advantages of plant-derived delivery systems with the inherent pharmacological activities of TCM phytochemicals, offering dual roles as both bioactive agents and therapeutic carriers. An Oral Biomimetic Hydrogel for Colon-Localized and Tumor-Targeted Sonodynamic Therapy of Colorectal Cancer.

Oral drug delivery systems show great promise for colorectal cancer (CRC) therapy but are limited by poor gastrointestinal stability, insufficient colon localization, and low tumor targeting. Here, we report an oral biomimetic hydrogel platform, SAM/Au-TiO2@Gel, for colon-localized and tumor-targeted sonodynamic therapy of CRC. Au deposition enhances the sonodynamic reactive oxygen species (ROS) generation capability of TiO2, while Staphylococcus aureus membrane (SAM) coating improves tumor targeting and uptake. Inulin hydrogel enables stable oral delivery and prolonged colonic retention. SAM/…

Orexin pathway modulation in narcolepsy: from Autoimmune mechanisms to nanotechnology enabled therapeutic delivery.

Narcolepsy is a long-term brain disorder caused by disrupted orexin signaling. Orexin neurons sit in the hypothalamus and control the sleep-wake cycle; damage to these neurons leads to narcolepsy. Early evidence pointed to an autoimmune attack on orexin neurons as the main cause. Current treatments - stimulants, antidepressants, sodium oxybate, and scheduled naps - ease symptoms but do not stop the disease, and they often cause side effects such as anxiety, insomnia, and heart problems. Most of these drugs also cannot cross the blood-brain barrier (BBB) to act directly on the hypothalamus. Thi

Treatment strategies for leptomeningeal disease in patients with breast cancer.

Leptomeningeal disease (LMD) associated with breast cancer (BC), characterized by the invasion of metastatic BC cells into the leptomeninges and cerebrospinal fluid, poses a significant clinical challenge. Current management strategies are not curative but rather aim to slow the rapid clinical decline associated with LMD, each with its own set of limitations. For instance, systemic chemotherapy faces delivery barriers while intrathecal administration directly targets the site of disease but struggles with uneven drug distribution, toxicity, and limited efficacy. Radiation therapy, including wh
