High burden of viridans streptococcal endocarditis linked to multidrug resistance and regional risk factors: a molecular epidemiological report from Khyber Pakhtunkhwa, Pakistan.

BACKGROUND: Infective endocarditis (IE) is a life-threatening condition caused by Viridans Group Streptococci (VGS) in low and middle-income countries. Data on epidemiology and molecular resistance mechanisms in the high-risk resource-limited setting of Khyber Pakhtunkhwa (KP), Pakistan, are scarce. This study aimed to determine the prevalence, risk factors, and molecular analysis of antimicrobial resistance in VGS isolated from IE patients in KP. METHODS: A cross-sectional study was done on 350 IE suspected patients admitted to tertiary care hospitals in KP. Blood cultures were performed and

Implementation of a Multidisciplinary Eye Clinic within a Cancer Center for Patients at Risk of Ocular Graft versus Host Disease.

OBJECTIVE: Ocular graft-versus-host disease (oGVHD) affects approximately 50% of patients undergoing hematopoietic stem cell transplantation (HSCT). Guidelines for monitoring patients for oGVHD development are not clearly established. An oGVHD clinic at the University of Maryland Greenebaum Comprehensive Cancer Center (UMGCCC) was established to evaluate HSCT patients at risk of developing oGVHD. The objective of this study was to examine whether an integrated eye clinic corresponded with lower odds of developing oGVHD within 2 years post-HSCT. DESIGN: A single-center retrospective longitudin

Plasmids of the multidrug-resistant Citrobacter portucalensis KOS1-1 strain isolated from a wastewater treatment plant harbor antibiotic resistance genes and gene clusters involved in carbon metabolism.

The growing interest in multidrug-resistant (MDR) Citrobacter species stems from their epidemiological significance and their potential to harbor antibiotic resistance genes (ARGs), with mobile genetic elements playing a central role in their dissemination. Wastewater treatment plants play an important role in the formation of such MDR strains due to the high rate of horizontal gene transfer in these environments. In this study, we isolated the MDR strain KOS1-1 of Citrobacter portucalensis from wastewater, sequenced its genome, and characterized its ARG content and plasmid profile. This strai

Intracameral Cefuroxime Use in Cataract Surgery After Penicillin Allergy Reclassification.

IMPORTANCE: Among patients who have a penicillin allergy label (a history of any reaction to penicillin in their medical records), many cataract surgeries are performed yearly. Less than 1% of these patients have a true allergy to penicillin. The evidence supports limited concern regarding cephalosporin cross-reactivity; however, the guidance for surgeons regarding antibiotic selection in these patients remains unclear. OBJECTIVE: To examine rates of perioperative antibiotic selection after penicillin allergy label reclassification vs before reclassification. DESIGN, SETTING, AND PARTICIPANT

Ornamental birds: hidden carriers of potentially virulent and antimicrobial-resistant Enterococcus faecalis in Bangladesh.

Enterococcus faecalis, a nosocomial pathogen adept at acquiring virulence and resistance genes, may be silently carried by ornamental birds in proximity to humans, posing a health risk. Two hundred ornamental bird samples (feces = 120, cage feed = 60, and store feed = 20) were collected and processed to detect E. faecalis isolates and determine their biofilm-forming capabilities, virulence, and antimicrobial resistance profiles. In PCR, 44 (22%) samples were found positive for E. faecalis, with the highest occurrence in cage feed samples (18/60), followed by feces (25/120) and stored feed (1/2

Assessment of oral carriage rate of fusobacterium nucleatum among postnatal mothers - An observational study.

BACKGROUND: Preterm birth is defined as childbirth before the 37th week of gestational age, which is highly prevalent worldwide, with 5-18% of births. Fusobacterium nucleatum, an anaerobic oral commensal, is found to be one of the most abundant ubiquitous species in the oral cavity, especially in the subgingival plaque and saliva. This pathogen is strongly associated with intrauterine environment colonization, specifically in amniotic fluid, in up to 70% of pregnant women. It causes adverse pregnancy outcomes like preterm birth, preeclampsia, stillbirth, etc. The potential connection between p

Pediatric Cervical Lymphadenitis: Etiology, Clinical Presentation, and Antimicrobial Resistance.

Background: Cervical lymphadenitis is prevalent in children. Several viruses and bacteria can cause cervical lymphadenitis. Staphylococcus aureus and Streptococcus pyogenes are known to predominate as bacterial causes. Choosing the effective antibiotic regimen to treat cervical lymphadenitis is difficult because of temporal and geographical variations in its etiologies and antibiotic resistance. We aim to elucidate the etiologies, treatment, and outcomes of cervical lymphadenitis in children in Iran. Methods: A total of 113 patients admitted to the Children's Medical Center in Iran were includ

Canine Staphylococcaceae circulating in a Kenyan animal shelter.

Animal shelters, especially in resource-poor countries, bring together pets from different regions and with different backgrounds. The crowding of such animals often results in infectious diseases, such as respiratory infections. This study characterized Staphylococcaceae from diseased and apparently healthy dogs housed in an animal shelter in Kenya, to determine their antibiotic resistance profiles, their genetic relatedness, and the presence of dominant clones. Therefore, bacteria were collected from all 167 dogs present in the shelter in June 2015 and screened for Staphylococcaceae using st
