Peripheral nervous system in gyrate atrophy of the choroid and retina with hyperornithinemia.

OBJECTIVE: To evaluate peripheral nervous system involvement in gyrate atrophy of the choroid and retina with hyperornithinemia (GA). BACKGROUND: GA is an inborn error of amino acid metabolism caused by mutations in the enzyme ornithine aminotransferase. Patients with GA have hyperornithinemia, progressive centripetal loss of vision, minor CNS abnormalities, and type II muscle fiber atrophy with accumulation of tubular aggregates. The authors previously showed that muscle and brain creatine stores are depleted in the patients with GA. METHODS: The authors searched evidence of peripheral nerv

Neuronal ceroid lipofuscinosis: detection of atypical forms.

The neuronal ceroid lipofuscinoses (NCL) are progressive neurodegenerative diseases occurring in infancy and adulthood. Atypical forms of these diseases have been described and are particularly represented in the late-infantile and juvenile onset groups. Recent progress in biochemistry and molecular genetics has identified some of these variants as separate disease entities while disclosing the phenotypic variability of some classic forms. We report the results of a retrospective analysis performed on a series of 27 NCL patients, 15 of which were atypical as to clinical and/or pathological fin

Effects of muscle relaxants on EEG, ABR and EMG in rabbits.

The effects of baclofen and pancuronium bromide on evoked electromyogram (EMG), cortical electroencephalogram (EEG) and auditory brainstem responses (ABR) were studied in pentobarbital anesthetized normal rabbits. Evoked EMG was measured in the gastrocnemius muscle by electrical stimulation of the sciatic nerve. Intravenous injection of baclofen decreased EEG and arterial blood pressure and light reflex, however, it had no significant influence on EMG or ABR at doses of 10 and 20 mg/kg/h. Pancuronium bromide immediately inhibited respiration, decreased EEG and EMG, however, it had no significa

Central nervous system monitoring during open heart surgery: an update.

Electroencephalography (EEG) is used to monitor the function of the central nervous system in a variety of surgical procedures. During cardiac surgery with cardiopulmonary bypass, ischemic changes on the EEG as a result of impaired central nervous system blood supply can be noted. The EEG also may be used to monitor the functional state of the brain during and after profound hypothermic circulatory arrest. If a desired goal is to reduce cerebral metabolism using a barbiturate or other such agent, the EEG can be monitored to provide a measurable pharmacodynamic end point (burst suppression). Te

A topographical ERP study of healthy premature 5-year-old children in the auditory and visual modalities.

The aim of this research is to study the impact of extreme prematurity on the cognitive development of the child as assessed at age 5 years 9 months. Our samples include 15 healthy prematures born between 25 and 28 weeks of gestational age carefully matched with 15 full-term controls. In the first experiment, two different auditory stimuli were presented to the subjects who listened passively without instruction. The second experiment consisted of a standard visual oddball task in which the subjects were instructed to 'catch' two different animals, by pushing a left or right button for a moose

Comparison between multiple sclerosis in India and the United States: a case-control study.

The prevalence of MS in India is low, and it is unclear whether the manifestations of the disease in India are similar to the United States. We carried out a case-control study to compare the disease in the two populations and used clinical, evoked potential, and MRI criteria to assess similarities and differences. Our results indicate that the rate of disease progression and frequency of involvement of the cerebral hemispheres, cerebellum, spinal cord, and brainstem were similar in the two populations. The visual system was more frequently involved in Indian patients. No Indian patient had a

Alpha-pattern coma in primary brainstem hemorrhage; clinical, electrophysiologic, and cerebral blood flow studies.

A patient with a brainstem hemorrhage and alpha-pattern coma is reported. Brainstem auditory and somatosensory evoked responses were severely abnormal, suggesting bilateral lesions of the pons, which was confirmed by magnetic resonance imaging. Regional cerebral blood flow, studied using IMP-SPECT, showed no remarkable regional radioactive accumulation or deficit in either the cerebral cortex or the subcortical regions. Based on these findings, we speculate that the alpha-pattern coma may be observed when the functions of brain structures rostral to the pontomesencephalic junction are preserve

Behavioral, electrophysiologic, and otoacoustic measures from a child with auditory processing dysfunction: case report.

This case was selected to highlight the importance of the test battery approach in the assessment of a child with auditory processing deficits. The utility of behavioral, electrophysiologic, acoustic immittance, and evoked otoacoustic emission procedures, as well as the problems associated with interpreting these multiple measures with differing results, is discussed. This case was confounded by the possibility that both peripheral and central auditory problems existed. The outcome stresses the importance of examining the results of multiple auditory measures in the determination of the site o

Euphemisms modulate the neural processing of norm violations.

Euphemisms are commonly used in media to soften or obscure the severity of unethical or controversial actions. The present study explored the neural correlates underlying the processing of euphemized vignettes portraying norm violations. We recorded event-related potentials (ERPs) and collected behavioral severity ratings while participants (n = 38) read vignettes describing the same norm-violating events framed with euphemized, non-euphemized or control critical words. Behaviorally, participants rated norm violations as significantly less severe when described with euphemisms. At the neurophy

Resilience of otolithic reflex to gravitational changes.

Otoliths within the vestibular system detect the magnitude and orientation of gravity and provide a reference frame for spatial orientation. Although vestibular processing has been studied in altered gravity, little is known about the initial minutes of exposure, when space motion sickness emerges. This study investigated whether otolithic detection is modified in different gravities or by sensory conflicts. Otolithic function was assessed using vestibular-evoked myogenic potentials (VEMPs) recorded from three muscles, reflecting utricular and saccular activity. VEMPs were measured during para

A learning-evoked slow-oscillatory architecture paces population activity for offline reactivation across the human medial temporal lobe.

Memory processing requires coordinated engagement of neuronal populations across brain networks and over time. How such coordination is organized in the human medial temporal lobe (MTL) remains unclear. Here, we show that MTL population activity is dynamically structured by a transient slow-oscillatory architecture that emerges during learning to promote offline consolidation and later recall. Using intracranial recordings that combine single-neuron spiking activity and local field potentials in human participants, we find that mnemonic engagement elicits on-demand slow-oscillatory bursts in t

Temporally Dissociated Neurophysiologic Findings in Pediatric Bickerstaff Brainstem Encephalitis: A Case Report.

Bickerstaff brainstem encephalitis (BBE) is rare in children, and neurophysiologic abnormalities may be detected and resolve at different times. A 12-year-old boy developed ophthalmoplegia, ataxia, somnolence, hyporeflexia, and weakness after a respiratory illness. On illness day 3, cerebrospinal fluid protein and magnetic resonance imaging were normal. On day 4, distal limb studies and needle electromyography were unrevealing, but tibial F-wave persistence was reduced, bilateral R1 and R2 blink responses were absent despite preserved facial compound muscle action potentials, and electroenceph

Probable linezolid-associated bilateral toxic optic neuropathy after prolonged therapy: A case report.

A woman in her 30s was administered linezolid treatment for pneumonia. Without continuous medical prescriptions, the patient self-administered linezolid for a total of 9 months, which subsequently led to symptoms of bilateral visual impairment and central scotomas. Fundus examination revealed blurred disc margins, accompanied with mild edema, flame-shaped hemorrhages around the optic disc, and mild tortuosity and dilation of retinal veins in both eyes. Visual field testing indicated the presence of central scotomas in both eyes. Visual evoked potentials testing showed reduced amplitudes in bot…

Fluctuations in arousal reflect latent state transitions that facilitate behavioural optimization.

People are often faced with surprising events that defy expectations. Such events elicit transient activity in the locus coeruleus/norepinephrine system and elevation of peripheral arousal markers including pupil dilation and a late positive peak in stimulus evoked EEG response, the P300, but the function of these signals remains unclear. We propose that they reflect latent state transitions that dynamically control the mental context governing learning and perception. We tested and confirmed five preregistered predictions of this theory using EEG and pupil measurements collected from people p

Remyelination and neuroprotection translational trials: Lessons from optic neuritis.

Multiple sclerosis disease-modifying treatments have reduced inflammatory activity but have limited impact on progressive neurodegeneration. In this context, remyelination and neuroprotection appear as key therapeutic goals. The visual pathway offers an attractive translational model, as optic neuritis recapitulates the demyelinating and neurodegenerative features of multiple sclerosis while providing accessible functional and structural readouts through visual evoked potentials and optical coherence tomography. Over the past two decades, several clinical trials have evaluated remyelinating an…
