Endothelin receptor antagonist therapy in congenital heart disease with shunt-associated pulmonary arterial hypertension: a qualitative systematic review.

BACKGROUND: Congenital heart disease (CHD) with systemic-topulmonary shunting is associated with pulmonary arterial hypertension (PAH). There are similar clinical and pathophysiological features between CHD with shunt-associated PAH and idiopathic PAH. Endothelin-receptor antagonists (ERAs) are oral medications that improve pulmonary hemodynamics, symptoms and functional capacity in many PAH patients. However, the role of ERAs in CHD with shunt-associated PAH is unclear. METHODS: MEDLINE, EMBASE and the Cumulative Index of Nursing and Allied Health Literature (CINAHL) databases were searched

Formoterol. A review of its pharmacological properties and therapeutic potential in reversible obstructive airways disease.

Formoterol, a long-acting beta 2-selective adrenoceptor agonist, produces dose-proportional bronchodilation in patients with obstructive airways disease with a reversible component. A significant effect occurs within minutes of inhalation of a therapeutic formoterol dose and persists for approximately 12 hours. Oral formoterol has a slower onset of action than the inhaled formulations, but also produces prolonged bronchodilatory effects. Inhaled formoterol has shown a therapeutic efficacy equivalent to or better than comparable dosages of the conventional beta 2-agonists salbutamol, fenoterol

Semaglutide use in a patient with a long-term ileostomy: A case report highlighting clinical risks and evidence gaps.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs), such as semaglutide, dulaglutide, liraglutide, and exenatide, are increasingly used to treat type 2 diabetes and support weight loss. These agents function by enhancing glucose-dependent insulin secretion, suppressing inappropriate glucagon secretion, slowing gastric emptying, and regulating appetite through central nervous system pathways. Since semaglutide's approval in 2017 and U.S. market availability in 2018, its use has expanded significantly. However, there remains limited literature evaluating its safety and efficacy in

Proteomic Profiling of Human Sweat Reveals Molecular Adaptations to Heat Acclimatization Induced by Isothermic Conditioning.

INTRODUCTION: Prolonged physical exertion in hot and/or humid environments can induce physiological strain and increase the risk of heat exhaustion or exertional heat stroke (EHS). Heat acclimatization (HA) optimizes an individual's ability to tolerate heat and is therefore widely adopted to improve exercise performance in thermally challenging environments. Sweat is a key thermoregulatory indicator reflective of physiological adaptations during HA and hence is regarded as a valuable resource for monitoring HA. However, knowledge of molecular-level changes and adaptations in sweat associated w

Cognitive behavioral therapy for insomnia in neurodegenerative disorders-targeting sleep disturbances in Alzheimer's and Parkinson's disease: a scoping review.

INTRODUCTION: Insomnia is highly prevalent in neurodegenerative disorders, yet pharmacological options carry safety and tolerability concerns. This scoping review mapped contemporary evidence for cognitive behavioral therapy for insomnia (CBT-I) across Alzheimer's disease (AD), mild cognitive impairment (MCI), and Parkinson's disease (PD). METHODS: Following a preregistered protocol (OSF DOI: 10.17605/OSF.IO/8VP3F), we searched PubMed, Cochrane Library, Web of Science, and Scopus for studies published 2015-2025. We screened English-language studies in adults and applied dual independent revie

Performance and injury-related effects of low-load blood flow restriction training in amateur soccer players: a randomized controlled trial.

BACKGROUND: Blood flow restriction (BFR) has gained interest in its application in exercise and potential benefits. This study aimed to investigate the effects of low-load, low-frequency BFR training compared with traditional high-intensity training on soccer-related performance outcomes and injury indicators in amateur athletes. METHODS: Twenty licensed male soccer players competing in a university-level league were randomized to either a BFR group (n = 10, 22.3 ± 1.3 years) or a control group (n = 10, 22.1 ± 1.8 years) for 6 weeks of training. Pre- and post-intervention assessments included

Acute systemic and energy metabolism responses to velocity-based resistance training following an oral glucose load in individuals with excess body weight.

We investigated the acute metabolic effects of two velocity-based resistance training (RT) protocols, differing in intra-set velocity loss (VL) thresholds, on postprandial substrate oxidation and glycaemic responses following a 75 g oral glucose tolerance test in individuals with excess body weight. A single-group, randomized, cross-over design was used, in which each participant completed three experimental conditions in random order: (1) control (rest); (2) RT with 20% velocity loss (VL20); and (3) RT with 40% velocity loss (VL40). Twenty-four participants (50% female; median body mass index

A four-session mindfulness-based cognitive therapy enhances problem-focused coping strategies in mothers of neonates in NICUs.

BACKGROUND: The birth of an infant with an abnormality or a postpartum disease and subsequent admission to the neonatal intensive care unit (NICU) is very stressful for mothers. The adoption of effective coping strategies can enable the mother to tolerate the critical conditions of the newborn’s illness. This study aimed to evaluate the effectiveness of a short-term (four 2-hour sessions) mindfulness-based cognitive therapy (MBCT) program on improving coping strategies among mothers of infants admitted to the NICU. METHODS: This quasi-experimental study was conducted using a pre-test-post-test

The transmural activation interval: a new mapping tool to identify ventricular tachycardia substrates in right ventricular cardiomyopathy.

AIMS: In right ventricular cardiomyopathy (RVCM), intramural scar may prevent rapid transmural activation, which may facilitate subepicardial ventricular tachycardia (VT) circuits. A critical transmural activation delay determined during sinus rhythm (SR) may identify VT substrates in RVCM. METHODS AND RESULTS: Consecutive patients with RVCM who underwent detailed endocardial-epicardial mapping and ablation for scar-related VT were enrolled. The transmural activation interval (TAI, first endocardial to first epicardial activation) and maximal activation interval (MAI, first endocardial to las

Workup and Management of Patients With Paradoxical Low-Flow, Low-Gradient Aortic Stenosis.

About 60% of patients with paradoxical low-flow, low-gradient (PLF-LG) aortic stenosis (AS) have a severe disease that justifies aortic valve replacement (AVR). The first step in patients with symptomatic PLF AS should be to rule out measurement errors and treat hypertension. The second step is to distinguish pseudo-severe from true severe AS (TSAS). The third step is to select the optimal treatment modality at the right time. Regarding the second step, projected aortic valve area calculated using stress echocardiography is superior to traditional severity criteria (AVA < 1.0 cm2 and mean grad

Sildenafil for pulmonary hypertension.

BACKGROUND: Pulmonary Hypertension (PH) can be either of unknown aetiology (primary pulmonary hypertension (PPH)) or due to a known underlying cause (secondary pulmonary hypertension (SPH). Pulmonary arteriolar vasoconstriction is considered to be an important characteristic of PH. Therapies which aim to vasodilate are used to treat pulmonary hypertension. OBJECTIVES: To determine the clinical efficacy of sildenafil, a vasodilator which works through inhibition of the enzyme phosphodiesterase type V (PDE5I), administered via any route to people with pulmonary hypertension in primary or second

Salbutamol in the 1980s. A reappraisal of its clinical efficacy.

Salbutamol (albuterol) is a beta 2-selective adrenoceptor agonist which accounts for its pronounced bronchodilatory, cardiac, uterine and metabolic effects. During the intervening years since salbutamol was first reviewed in the Journal (1971), it has become extensively used in the treatment of reversible obstructive airways disease. Numerous studies in this disease (including severe acute, childhood and exercise-induced asthma) have confirmed the bronchodilatory efficacy of salbutamol, and it has been shown to be at least as effective as most of the currently available bronchodilators, if not
