Xerostomia results from salivary gland irradiation during treatment of head and neck malignancies. In addition to having difficulty with speech and swallowing, these patients experience loss of taste, dental caries, and chronic fungal infections. The paired submandibular glands provide 70 percent of the normal salivary flow and are difficult to shield during radiation therapy. Another sicca condition, xerophthalmia, may result from facial nerve injury or other medical disorders and results in pain, corneal ulceration, and possible vision loss. Treatment options for xerostomia are limited, and

Prior work has established that hypoglossal motoneurons (HMs) change postnatally in their response to serotonin (5-HT), in part as a result of a decline in expression of 5-HT(1A) receptors. In the current study, two issues were addressed. First, using in situ hybridization we found that transient expression of 5-HT(1A) receptors occurs in other populations of brainstem (facial and trigeminal) and spinal (cervical and lumbar) motoneurons. Second, the participation of motoneuronal afferent (serotonergic) and efferent (neuromuscular) innervation in inducing and maintaining this decline in express

Neuromuscular/neurodegenerative disorders, such as the death of spinal cord motor neurons in amyotrophic lateral sclerosis (ALS) or the degeneration of spinal cord motor neuron axons in certain peripheral neuropathies, present a unique opportunity for therapeutic intervention with neurotrophic proteins. We have found that in mixed rat embryonic spinal cord cultures or in purified motor neuron preparations, recombinant human insulin-like growth factor 1 (rhIGF-1) enhances the survival of motor neurons at EC50 concentrations of 2 nM, consistent with an interaction at the tyrosine kinase-coupled

Of 200 patients with acoustic neurinoma undergoing an operation via the retrosigmoid transmeatal approach in the semisitting position, 18 patients had a high jugular bulb on the tumor side. The frequency was 9%. From a neurosurgical point of view, a jugular fossa above the low border of the internal auditory canal (IAC) is classified as a high one. All 200 patients were evaluated by computed tomography with bone window reconstruction of high-resolution thin axial slices (1.5 mm). High jugular bulbs were classified into three grades as follows: Grade I, jugular bulb situated less than 1.5 mm ab

Twenty-three patients with facial port wine stains were studied to determine whether chilling lesional skin at the time of treatment could improve the outcome of argon laser therapy and whether this effect could be attributed to increased hemoglobin content of chilled sites, as hypothesized on clinical grounds. Each patient was biopsied in two representative and clinically identical sites, once at room temperature and once immediately after application of ice to the skin surface for 2 to 3 minutes. Two additional identical sites were treated with an argon laser in the same manner. Histologic s

This study aims to assess the clinical value of computed tomographic angiography (CTA) and three-dimensional reconstruction techniques in the repair of facial skin defects. A total of 40 patients with facial trauma or scars underwent free flap transplantation. CTA and 3D reconstruction were used preoperatively in 23 patients to measure blood vessel diameter, location, direction, and branching, as well as the relationship between the vessels, skin flap, and surrounding tissues. Superficial temporoparietal, facial, thoracic, dorsal, and scapular arteriovenous vessels were analyzed. The flap harv

BACKGROUND: Skin texture and color are important considerations during the reconstruction of facial defects, and anatomical borders should be preserved. Therefore, a local flap is a better option. In these cases, the authors repaired facial defects using a bilateral interdigitated VY flap.
OBJECTIVE: We aim to present a modified bilateral Pacman flap technique for the reconstruction of round and oval facial defects.
MATERIALS AND METHODS: We performed a retrospective chart review of 25 patients (26 cases) who underwent bilateral interdigitated Pacman flap repair of round and oval facial defe

The complexity of the craniofacial patient mandates the cooperation of a multidisciplinary team that can systematically evaluate each individual and ensure that a protocol-driven pathway is undertaken for the best patient care. Oral and maxillofacial surgeons contribute to surgical care in this setting with specific knowledge of growth and development of the face. This enables optimum timing for early skeletal correction where appropriate, and definitive surgery following the cessation of growth to maximize function and aesthetics. This chapter will describe the major principles in managing pa

Background The clinical application of end-to-side (ETS) neurorrhaphy is under debate partly due to a lack of consensus on the source of axonal sprouting. Methods In this study, 24 rats were divided into three groups: sham operation, facial-hypoglossal ETS neurorrhaphy, and end-to-end (ETE) neurorrhaphy. Electrophysiological tests were employed to detect the evoked compound muscle action potentials (CMAPs) in different situations, and the latencies and maximal amplitudes of the CMAPs recorded were compared. Fluorescence retrograde tracing studies, hematoxylin and eosin (HE) staining, and immun

BACKGROUND: Management of full-thickness facial burns remains one of the greatest challenges. Controversy exists among surgeons regarding the use of early excision for facial burns. Unfortunately, delayed excision of deeper burns often results in more scarring and subsequent reconstruction becomes more difficult. A collagen-elastin matrix is used to improve the quality of the reconstructed skin, to reduce scarring and to prevent wound contraction. It serves as a foundation for split thickness skin graft and enhances short and long-term results.
AIM: We report the usage of a collagen-elastin m

OBJECT: The optimal treatment for sporadic vestibular schwannoma (VS) is highly controversial. To date, the majority of studies comparing treatment modalities have focused on a narrow scope of technical outcomes including facial function, hearing status, and tumor control. Very few publications have investigated health-related quality of life (HRQOL) differences between individual treatment groups, and none have used a disease-specific HRQOL instrument.
METHODS: All patients with sporadic small- to medium-sized VSs who underwent primary microsurgery, stereotactic radiosurgery (SRS), or observ
