Early revision of Monteggia-variant fractures.

PURPOSE: Monteggia-variant fractures involve a fracture of the proximal ulna combined with a radiocapitellar dislocation and/or additional injuries to the radial head. These injuries are associated with severe outcomes, high complication rates, and frequent need for revision surgery. This study aimed to investigate strategies for early revision following failed operative treatment of these injuries. METHODS: We retrospectively included all patients over 18 years of age who underwent revision surgery within the first 3 months after failed initial treatment of a Monteggia-variant fracture due t

Treating pathological metastatic fractures of the humerus by compound osteosynthesis: a retrospective cohort study.

BACKGROUND: The number of malignant tumors is increasing as are bone metastases, such as those in the humerus. Arm function is important for an independent everyday life. In this study, compound osteosynthesis of metastatic fractures of the humerus is examined for its suitability in light of the competing risk of death. METHODS: This retrospective monocentric study includes a cohort of tumor patients who underwent primary compound osteosynthesis for pathological humeral fractures. The main endpoint was the continued existence of compound osteosynthesis using competing risk analysis to contras

Onlay Uncemented Lateralized Reverse Shoulder Arthroplasty for Fracture Sequelae Type 1 with Valgus/Varus Malunion: Deltoid Lengthening and Outcomes.

The successful treatment of proximal humeral fractures remains challenging for shoulder surgeons, and failure rates are high, regardless of initial treatment. This study aimed to analyze the clinical and radiographic midterm results of onlay lateralized cementless stem reverse shoulder arthroplasty (RSA) in patients with valgus/varus malunion proximal humerus fracture sequelae without metaphyseal osteotomy. We retrospectively studied 35 cases with the diagnosis of fracture sequelae of the proximal part of the humerus with valgus/varus malunion. The mean duration of follow-up was 4.6 years (ran

Early ventral surgical treatment without traction of acute traumatic subaxial cervical spine injuries.

BACKGROUND: Spinal cord decompression after cervical spinal cord injury (SCI) is the standard of care. However, there is a lack of consensus regarding the optimal management of these injuries, including the role of traction and timing of surgery. Here, we report the safety/efficacy of ventral surgery without preoperative traction for intraoperative fracture reduction following acute cervical SCI. METHODS: We prospectively collected a series of patients who sustained acute traumatic subaxial cervical (C3-7) spine fractures between 2004 and 2016. Patients underwent anterior cervical decompressi

Medium-sized posterior fragments in AO Weber-B fractures, does open reduction and fixation improve outcome? the POSTFIX-trial protocol, a multicenter randomized clinical trial.

BACKGROUND: Guidelines for treatment of the posterior fracture fragment in trimalleolar fractures are scarce and show varying advices. Did the increasing size of the posterior fragment seem to relate to worse outcome in the past, nowadays this has changed to the amount of dislocation of the posterior fragment post-operatively. Despite many retrospective cohort studies and some prospective cohort studies, no consistent guideline could be derived from the current literature. METHODS: The POSTFIX-study is designed as a multicenter randomized clinical trial to analyse the effects of anatomical re

Analysis of pelvic fracture pattern and overall orthopaedic injury burden in children sustaining pelvic fractures based on skeletal maturity.

PURPOSE: The purpose of this study was to review pelvic fractures and concomitant orthopaedic injuries in children who have a patent triradiate cartilage (TRO) compared with children whose triradiate cartilage has closed (TRC). We hypothesise that these injuries will differ, leading to correlated alterations in management. PATIENTS AND METHODS: Using a database, we retrospectively reviewed patients aged below 18 years with pelvic fractures presenting to our Level 1 trauma center. Radiographs and CT scans were reviewed to identify orthopaedic injuries and categorise pelvic injuries using the m

[Orbital compartment syndrome. The most frequent cause of blindness following facial trauma].

INTRODUCTION: Although orbital compartment syndrome is a rare condition, it is still the most common cause of blindness following simple or complicated facial fractures. Its pathomechanism is similar to the compartment syndrome in the limb. Little extra fluid (blood, oedema, brain, foreign body) in a non-space yielding space results with increasingly higher pressures within a short period of time. Unless urgent surgical intervention is performed the blocked circulation of the central retinal artery will result irreversible ophthalmic nerve damage and blindness. Aim, material and method: A retr

Plastic deformation in nanodiamonds.

Diamond, composed of sp³ covalent carbon bonds, is renowned for its exceptional hardness, thermal conductivity, and wide bandgap, yet its intrinsic brittleness severely limits deformation and processing. Here, we report an amorphization-mediated ultralarge plasticity in nanodiamonds using a custom-designed in-situ transmission electron microscopy mechanical holder. Unlike conventional mechanisms such as dislocation motion or crystalline phase transformation, the deformation is governed by the formation of an interconnected amorphous carbon network that accommodates stress and enables cooperati

Revision total hip arthroplasty using ETO for failed bipolar hemiarthroplasty with protrusio acetabuli.

A woman in her 40s presented with persistent left hip pain and restricted motion for 5 years. X-ray findings revealed an uncemented bipolar stem, tension band wiring of greater trochanter with K wires, a broken Shenton line and protrusion acetabuli. The patient previously underwent unsuccessful cancellous screws fixation for an intracapsular neck of femur fracture, leading to non-union and subsequent bipolar hemiarthroplasty outside our facility. The planned extended trochanteric osteotomy (ETO) aimed to remove the stem, assess the acetabular defect and fill it with allograft. The use of a dis

Fatal exsanguination following complete transection of femoral vessels due to angle grinder injury in an industrial accident.

Angle grinders are one of the most dangerous and frequently used tools in industrial settings. Angle grinder injuries range from superficial cuts to deep penetrating injuries with underlying fracture-dislocation and vascular trauma. The injuries caused by angle grinders mostly involve the head, face, or upper limbs, while the lower limb is an unusual site. The high-speed rotating disc of the angle grinders does not respect anatomical boundaries or structures; therefore, the injuries caused may be disfiguring, permanently incapacitating, or even fatal. We report a fatal case of an angle grinder

Axillary artery and brachial plexus injury secondary to blunt trauma.

Rupture of the axillary artery in the absence of a fracture of dislocation is a rare traumatic event. An associated injury to the brachial plexus may accompany an axillary artery injury but has rarely been reported in the literature. We present the case of an elderly female, who fell onto an outstretched arm and sustained an axillary artery rupture, combined with a brachial plexus injury. The patient in this case did well post-operatively. The challenge in these cases is early recognition and diagnosis of a vascular injury. A significant mechanism of injury needs to alert the clinician to the

Traumatic lateral spondyloptosis of L2 with complete neurological deficit: A case report.

UNLABELLED: Traumatic spondyloptosis of the lumbar spine is an uncommon and severe clinical entity, which is defined as complete fracture dislocation and subluxation (>100%) of one vertebral body in the coronal or sagittal plane from its adjacent vertebra. In coronal spondyloptosis the subluxated vertebral bodies lie beside each other, and the condition is lateraloptosis. CLINICAL CASE: A male patient aged 56 years had multiple injures with complete neurological deficit. Computed tomography(CT) revealed as spondyloptosis, which L2 detached from the rest of the spine, spinal canal stenosis, sa

Combined Soft Tissue Reconstruction of the Medial Patellofemoral Ligament and Medial Quadriceps Tendon-Femoral Ligament.

Patellar dislocation is a major orthopaedic concern in pediatric and adult populations, with both conservative and surgical options available. Several surgical techniques of the combined or isolated reconstruction of the medial patellofemoral ligament are described in the literature using different grafts, locations, and types of fixations, providing different advantages and disadvantages. New recent developments in cadaveric dissection studies have unveiled the importance of structures that were neglected until recently, such as the medial quadriceps tendon-femoral ligament, which provides a

Glenohumeral Osteoarthritis in the Young Patient.

Glenohumeral osteoarthritis in the young patient (aged <60 years) is a difficult condition, given both age and functional demands. Primary osteoarthritis is the most common etiology in this patient demographic, but secondary causes include osteonecrosis, previous trauma (eg, fracture-dislocation), previous infection (eg, septic arthritis), previous capsulorrhaphy, previous arthroscopic surgery (eg, chondrolysis), and glenoid dysplasia. Nonsurgical modalities, including activity modification, pharmacotherapy, physical therapy, and intra-articular injections, are the mainstay of management; howe
