Delayed Diagnosis of an Ischial Tuberosity Avulsion Fracture in a Young Gaelic Football Player.

PRESENTATION: Ischial tuberosity avulsion fractures account for a minority of pelvic apophyseal injuries in adolescents but are frequently misdiagnosed as hamstring strains. We report one case of delayed diagnosis following a Gaelic Football Injury. DIAGNOSIS: A male teenaged footballer player developed persistent right buttock pain after kicking the ball at full sprint. Initial management was conservative for presumed hamstring strain. After two years of persistent pain and inability to sprint, he was referred for an Orthopaedic opinion. He had persistent focal bone tenderness over the ischi

[Metabolic bone disorders: what the internist must not overlook].

BACKGROUND: Metabolic bone disorders are common, albeit underestimated, causes of fractures and chronic bone pain in internal medicine. They can remain asymptomatic for long periods and are frequently only diagnosed when insufficiency fractures occur. AIM OF THE STUDY: The aim of this review article is to discuss common and rare metabolic bone disorders and to outline a practice-oriented diagnostic approach. MATERIALS AND METHODS: Narrative review of current literature with particular emphasis on clinically relevant differential diagnoses and diagnostic algorithms. RESULTS: In addition to s

Risk for osteoporosis and fracture with glucocorticoids.

Glucocorticoid use is ubiquitous and is associated with multiple adverse reactions. Among them, osteoporosis and bone fractures are of our concern. In this review, we present current evidence on the effect of glucocorticoids on bone mineral density and the risk of fractures, the mechanisms underlying those effects, and the recommendations for monitoring and treating patients who take them. The bone mineral density of the lumbar spine and total hip is lower, and the risk of fractures is higher in glucocorticoid users than non-users. These effects have a rapid onset, are dose-dependent, and impr

Mutiple Spontaneous Rib Fractures in Patient with Cushing's Syndrome.

Glucocorticoid (GC) excess, including Cushing's syndrome, is a common cause of secondary osteoporosis. Thirty to fifty percent of Cushing's syndrome patients experience non-traumatic fractures, which is often the presenting manifestation of Cushing's syndrome. However, there have been rare cases of Cushing's syndrome diagnosed only based upon bone manifestations. We describe a case of Cushing's syndrome that was diagnosed in a 44-year-old woman who initially visited our hospital due to multiple non-traumatic rib fractures. She did not exhibit any other manifestations of Cushing's syndrome such

The effect of leg fracture on pedestrian kinematic response and injury severity in car-pedestrian collisions.

This study aims to clarify the relationship between pedestrian leg bone fracture, the head, chest, abdomen and pelvis kinematic responses, and knee joint injury severity in car-pedestrian collisions, based on a coupled FE model with two scenarios: leg bone fracture and non-fracture. It finds leg fractures cause abdominal and pelvic upward Z-axis shifts, reduce knee ligament elongation rates and reduce femur/tibia peak bending moment, but have no significant effects on head and chest kinematics and injuries. This study demonstrates that kinematic responses of abdomen and pelvis and knee injury

The Aging Skeleton.

Skeletal aging begins after peak bone mass is reached; progressive bone loss then occurs. Peak bone mass may occur at different ages in different skeletal sites and varies between sexes. Accelerated loss of bone occurs in the perimenopausal period in women, whereas more gradual but progressive loss of bone occurs in aging men. Changes in bone quality as well as bone quantity occur during growth and subsequent aging. These include changes in bone microarchitecture which may differ between cortical and trabecular compartments and in different sites, and may impact on bone size and geometry. Chan

Scaphoid Fracture Patterns-Part One: Three-Dimensional Computed Tomography Analysis.

Objective  Using three-dimensional (3D) computed tomography models of acute scaphoid fractures, we looked for differences between volumetric size of the fracture fragments, recognizable groups, or a shared common fracture area. Methods  We studied 51 patients with an adequate computed tomography scan of an acute scaphoid fracture using 3D modeling. Fracture surfaces were identified and fragment volumetric size of the fracture fragments was measured. A principal component analysis was used to find groups. Density mapping was used to image probable common fracture areas in the scaphoid. Results

Diagnosis of occult scaphoid fractures and other wrist injuries. Are repeated clinical examinations and plain radiographs still state of the art?

To examine the efficacy of repeated clinical examinations and follow-up radiographs, 121 patients were prospectively and consecutively randomised and clinically followed until a final diagnosis was achieved. All of these patients additionally underwent magnetic resonance imaging (MRI) scans within an average of 3 days after trauma to control the results of this study. MRI detected 112 injuries in 82 patients (67%). Twenty-eight (25%) of these injuries were scaphoid fractures. There were 15 fractures of other carpal bones, 14 avulsion fractures of extrinsic ligaments (AFL), 26 other bone injuri

Endoprosthetic Reconstruction for a Displaced Atypical Femoral Fracture in a Cancer Patient with Poor Prognosis.

Zoledronate or denosumab treatment is beneficial for cancer patients with bone metastasis. However, each agent may trigger atypical femoral fractures. Incomplete atypical femoral fractures can be successfully treated with prophylactic intramedullary nailing. On the other hand, intramedullary nailing for displaced atypical femoral fractures occasionally causes problems with regard to bone healing, resulting in long-term treatment. In cancer patients with poor prognosis who experience atypical femoral fractures, improvement in activities of daily living should be the priority. Thus, we performed

Operative treatment of distal femoral fractures proximal to total knee replacements.

Twenty fractures of the distal part of the femur proximal to a total knee replacement were treated operatively by members of the New England Trauma Study Group. Notching of the anterior aspect of the femoral cortex was associated with only two of these fractures, and none of the knee prostheses was loose at the time of the fracture. All twenty fractures were treated with open reduction and stable internal fixation, and the operation on fifteen fractures was supplemented with bone grafts. Every fracture healed, and eighteen healed after a mean of sixteen weeks (range, six to forty weeks). Union

[Primary controlled endolymphatic antibiotic therapy in traumatology and orthopedics].

The work analyses 55 cases of traumatic osteomyelitis of the leg bones, open fractures of the leg bones, and medial fractures of the femoral neck. Patients with medial fractures of the neck of the femur were treated by endoprosthesis after Mur-Kaplan. Endolymphatic antibiotic therapy was included in the complex of therapeutic measures in each group of patients. This method allowed suppuration of the postoperative wounds to be avoided and promoted in this manner rehabilitation of the patients.

The Ideal Location of the Lateral Hinge in Medial Closing Wedge Osteotomy of the Distal Femur: Analysis of Soft Tissue Coverage and Bone Density.

BACKGROUND: Although an appropriate hinge position to prevent unstable lateral hinge fractures is well established in medial opening wedge high tibial osteotomy, the position during medial closing wedge distal femoral osteotomy has not been elucidated. PURPOSE/HYPOTHESIS: The purpose was to evaluate the ideal hinge position that would prevent an unstable lateral hinge fracture during biplanar medial closing wedge distal femoral osteotomy based on soft tissue coverage and bone density around the hinge area. The hypothesis was that the ideal hinge position could be clarified by analyzing soft t

[Calcium supplementation in the menopause--unnecessary, advisable, or necessary?].

With the menopause, the multiple positive effects of estrogens on bone come to an end, which leads to an increased bone turnover an accelerated bone loss and an increased fracture risk. Although the antifracture effect of calcium alone is questionable, several observations justify the recommendation to avoid calcium deficiency in this age: Very low Calcium intake is related to low BMD and higher hip fracture incidence in several studies; regular Calcium supplementation lowers fracture incidence, when observed over years. Calcium from dairy products, mineral waters and supplements is almost equ

Biochemical and clinical manifestations in adults with hypophosphatasia: a national cross-sectional study.

UNLABELLED: Hypophosphatasia (HPP) is a rare disease affecting bone mineralization. Adults with HPP have an increased occurrence of low-energy fractures, which cannot be explained by reduced bone mass assessed by dual energy X-ray absorptiometry. The bone phenotype in adults with HPP requires further studies investigating bone strength and bone structural parameters. INTRODUCTION: Hypophosphatasia (HPP) is a rare inherited disorder of bone and mineral metabolism, characterized by broad-ranging clinical manifestations and severity. However, studies investigating the clinical spectrum in adults

Maternal diabetes and fracture risk in offspring: a population-based analysis.

Factors affecting intrauterine environment exerts influence on skeletal health and fracture risk in later life. Diabetes during pregnancy is known to influence birth weight and is associated with fetal overgrowth. However, the effects of maternal diabetes on fracture risk in offspring is unknown. This study was aimed to evaluate the association between maternal diabetes and fracture risk in offspring. Using population-based administrative health data for Manitoba, Canada, we identified deliveries complicated by gestational diabetes and type 2 diabetes between April 1, 1980, and March 31, 2020.

In vitro/ in vivo evaluation of double crosslinked bone glue with different degrees.

Successful bone fragment fixation is a crucial factor in bone fracture healing, the fixation of crushed bone fragments could hinder bone fracture healing. Thus, ideal bone glues to effectively adhere and splice comminuted bone fragments are needed in clinical. Herein, an osteoinductive and biodegradable double cross-linked bone glue (GelMA-oDex-AMBGN) was constructed through Schiff's base reaction between commercial GelMA (with different substitution degrees of amino groups) and Odex mixed with amine-modified mesoporous bioactive glass nanoparticles (AMBGN), followed by crosslink with blue lig

Tomosynthesis: A new radiologic technique for rapid diagnosis of scaphoid fractures.

Scaphoid fractures constitute 71% of all carpal bone fractures.1 Early diagnosis and treatment has significant bearing on fracture union rates and better clinical outcomes. While displaced fractures can be readily seen on plain radiograph, undisplaced fractures can require advanced imaging modalities to confirm that diagnosis. Advanced imaging such as Magnetic Resonance Imaging (MRI), Computerised tomography (CT) and bone scintigraphy are routinely used for the diagnosis of scaphoid fractures but require significant radiation exposure, increased cost and can be difficult to access.2 Tomosynthe

A locked hip screw-intramedullary nail (cephalomedullary nail) for the treatment of fractures of the proximal part of the femur combined with fractures of the femoral shaft.

Twenty-seven patients with complex femoral fractures (combined shaft and proximal femoral fractures) were treated with a modified Grosse-Kempf slotted locking nail (cephalomedullary nail), wherein two screws were inserted in the hip. Four types of complex, multifocal femoral fractures were represented in the series. Eleven of the femoral shaft fractures were secondary to a previous, internally fixed, not yet united hip fracture (type I). Ten comminuted peritrochanteric fractures occurred in normal bone (type II). Three similar fractures were pathologic because of metastasis. Two patients had a
