Antiplatelet therapy in the elderly. Aspirin, ticlopidine-clopidogrel, and GPIIb/GPIIIa antagonists.

Antiplatelet agents including aspirin, dipyridamole, the thienopyridines, and the GPIIb/IIIa antagonists have collectively demonstrated their ability to have a significant impact on the incidence of recurrent MIs, strokes, and other vascular ischemic events in the geriatric population. Low-dose aspirin also seems to be effective and safe for the primary prevention of ischemic heart disease in men considered at high risk. There is no evidence that the recommendations from these studies had increased relevance to younger adults, and the studies considering age as a variable found antiplatelet ag

Mycotic infections. Effective management of conditions involving the skin, hair, and nails.

Superficial mycotic infections of the skin, hair, or nails are recurring presentations in the geriatric primary care setting. The most common infections are those caused by dermatophytes. The genus Trichophyton gives rise to most of the tinea dermatophytoses, including tinea capitis, tinea pedis, and tinea unguium (onychomycosis). Part of the diagnostic challenge lies in distinguishing the mycotic lesions from those caused by cutaneous diseases such as psoriasis, eczema, dyshidrosis, and contact dermatitis. Because environmental conditions play a major role in fungal infection onset, clinical

Cardiac Disease in the Elderly: Radiographic Features.

The plain film appearances of cardiac disease in the geriatric age group are often different in younger patients because of the increased incidence of age related heart diseases, the physiological alterations that affect the range of normal values in the elderly, and the complicating effects of age related noncardiac diseases. Although the clinical significance of conditions, such as coronary artery calcification, calcification of the mitral annulus, and senile calcification of the aortic valve, is not completely understood at the present, they are indicative of significant disease in an appre

Abnormal behavior of elderly people residing in institutions of medical and social welfare systems.

We investigated factors correlated with abnormal behavior in the elderly residing in a special nursing home (group A) and a psychiatric hospital (group B) using the dementia behavior disturbance scale, the mini mental state examination, the Japanese version of the Philadelphia Geriatric Center morale scale, and the ADL assessment scale. The cognitive function of group B was decreased compared with that of group A, but most activities of daily living (ADL) in the latter group were disrupted compared with those in the former. Only a few categories of ADL correlated with abnormal behavior in grou

Cognitive decline in patients with Alzheimer's disease, vascular dementia and senile dementia of Lewy body type.

One hundred and twenty-four patients with DSM-III-R dementia were assessed with a standardized battery which included the Geriatric Mental State Schedule, the History and Aetiology Schedule, the Secondary Dementia Schedule and the CAMCOG. Patients with Alzheimer's disease, vascular dementia and senile dementia of Lewy body type (SDLT) all had a similar degree of cognitive impairment at the time of the baseline interview. Patients with Alzheimer's disease and vascular dementia each experienced a mean decline of 27 points in patients with SDLT. Patients with SDLT had a significantly greater decl

Special considerations in the use of NSAIDs in the elderly.

As the number of elderly patients with rheumatic conditions continues to increase, rheumatologists are faced with the challenge of providing more effective but safer therapy to their geriatric patients. In 1991, nonsteroidal anti-inflammatory drugs (NSAIDs) accounted for more than 29 million prescriptions worldwide. The elderly are at increased risk for NSAID-associated toxicity, particularly gastrointestinal side effects. Predisposing factors include age-related physiologic changes, underlying conditions, and polypharmacy. Clinical experience shows etodolac, 300 mg twice daily, given to elder

The delivery of interim discharge summaries to general practitioners by the elderly.

Two hundred and four patients discharged from an acute geriatric unit were given an interim discharge summary for delivery to their General Practitioner (GP). All but three summaries were received by the practices. Information regarding mode and timing of receipt was available in 191 patients. The summaries were received in a median of 1.0 day in both those living alone and those living with family; 75% of summaries had been received within two days of discharge and 93% within four days. General practitioners collected the summaries from patients' homes in a high proportion of both groups. Int

[Dysphagia in the elderly].

The prevalence of dysphagia in the elderly ("presbyphagia") is probably still underestimated, though this disorder represents a major geriatric problem; special attention is necessary to prevent malnutrition, dehydration and aspiration pneumonia. Primary presbyphagia due to physiological, age-related changes of the swallowing mechanism must be differentiated from secondary presbyphagia attributable to diseases which are more frequent in the elderly. Transnasal pharyngo-laryngo-fiberendoscopy, videofluoroscopy and the "modified barium swallow" are of particular value in the diagnostic approach

Longitudinal study of thyroid function in acutely ill elderly patients using a sensitive TSH assay-defer testing until recovery.

A prospective longitudinal study of thyroid function was conducted in 180 consecutive admissions to an acute geriatric unit. The rest strategy included assay of TSH and TRH testing when appropriate. On admission TSH was suppressed (less than 0.1 mU/l) in 17 patients (9.4%) and elevated (greater than 4.0 mU/l) in 8 (4.4%). Follow-up of these initial abnormalities showed resolution in almost all cases. Our findings suggest that acute illness may interfere with the hypothalamic-pituitary-thyroid axis, causing either temporary suppression or stimulation of TSH release. Although no new cases of thy

Diarrhea and fecal impaction in elderly long-stay patients.

A retrospective screening of one year's duration was carried out on 245 permanently hospitalized geriatric patients to study the frequency and cases of periods of at least three days' diarrhea. Such periods were suffered by 65/245 patients and 32% had recurrencies. Fecal impaction was the most common cause of diarrhea (in 55%). Diarrhea was induced by laxatives in 20%. Gastrointestinal infections caused 5% of the diarrhea cases. Immobility and fecal incontinence were strongly associated with fecal impaction in cases of diarrhea. Urinary tract infections treated with antibiotics in the patients

Diagnostic evaluation of the mentally impaired elderly living at home.

Of 334 elderly persons aged 75 years and over, drawn from a random sample living at home, 38 were diagnosed as mentally impaired by a GP during an examination in the respondent's home. The GP also made diagnoses as to the cause of the mental impairment. These diagnoses agreed in 79% of cases with the final diagnoses after psychogeriatric assessment, including cerebral CAT-scan, EEG, and ECG investigation. The GP's diagnosis was changed in 8 cases; in 5 primarily due to the findings on the cerebral CAT-scan, and in 3 to the psychogeriatric assessment. The study showed that a GP with some experi

Extracorporeal shock wave lithotripsy for the treatment of urinary calculi in the elderly.

Renal calculus in the elderly patient is associated with the same symptoms and risks as in other patient populations. With the recent development of extracorporeal shock wave lithotripsy (ESWL), many older patients can now be treated for renal calculi. The Department of Urology at the University of Iowa Hospitals and Clinics has performed 107 ESWL treatments on a total of 96 patients over 70 years of age. This represents over 12% of all patients treated at our institution. No deaths related to ESWL treatment have occurred in this group, nor has increased morbidity been observed in comparison t

A model of risk of falling for psychogeriatric patients.

One hundred community-dwelling psychiatric outpatients, 60 years and older, were evaluated for factors associated with symptoms of dizziness, falling, and orthostatic hypotension. Thirty-nine percent complained of dizziness or falling, and 34% had systolic orthostatic hypotension. Together, systolic and diastolic blood pressure drop, type of somatic illness, type and number of drugs, and psychiatric diagnosis accounted for 50% of the variance in dizziness and falling. Type of illness, drug category, and psychiatric diagnosis accounted for only 19% of the variance in orthostatic hypotension. St

Robotic-assisted transverse colectomy for transverse colon cancer in a centenarian: a case report.

A 100-year-old woman with transverse colon cancer underwent robotic-assisted colectomy with intracorporeal anastomosis and Pfannenstiel specimen extraction. She was independent in activities of daily living and classified as non-frail based on a comprehensive geriatric assessment. Preoperative imaging showed localized disease without metastasis. The operation was completed without complications, and postoperative recovery was uneventful, with preservation of functional status. Histopathology revealed poorly differentiated adenocarcinoma with direct invasion into the jejunum and microsatellite

Unilateral Vocal Cord Palsy as Presenting Feature of Amyotrophic Lateral Sclerosis.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative pathology marked by the degeneration of upper and lower motor neurons, resulting in muscle weakness and atrophy, impairing motor function. Bulbar-onset ALS is a distinct clinical subtype, with initial involvement of bulbar motor neurons, often causing severe speech and swallowing difficulties. Despite its impact, bulbar-onset ALS, especially with rare symptoms like unilateral vocal cord palsy (UVCP), lacks extensive research. Here, we detail the case of a 79-year-old nonambulatory diabetic male with a 1-year history of hoar

Quality of Life Over Quantity: Conscious Refusal of Treatment and Stable Course of Gastric Signet-Ring Cell Carcinoma in a 94-Year-Old Woman.

We report the case of a 94-year-old woman with histologically confirmed signet-ring cell carcinoma (SRCC) of the stomach (ICD-O-3.2 code 8490/3). The patient consciously declined oncological treatment and received only supportive and symptomatic care. Over a follow-up period of at least 12 months, she has remained alive, active, cognitively intact, and socially engaged. Despite the typically aggressive histological subtype, the disease followed an indolent clinical course. This case highlights the biological heterogeneity of SRCC and the importance of personalised oncogeriatric decision-making

Bone Cements Used in Vertebral Augmentation: A State-of-the-art Narrative Review.

Vertebral compression fractures (VCFs) are common in osteoporotic patients, with a frequency projected to increase alongside a growing geriatric population. VCFs often result in debilitating back pain and decreased mobility. Cement augmentation, a minimally invasive surgical technique, is widely used to stabilize fractures and restore vertebral height. Acrylic-based cements and calcium phosphate cements are currently the two primary fill materials utilized for these procedures. Despite their effectiveness, acrylic bone cements and calcium phosphate cements have been associated with various int

Acute myeloid leukemia in the medically unfit elderly patients.

Acute Myeloid Leukemia is a clonal proliferative disorder whose incidence increases with age. While studies have shown that the elderly medically unfit patients have poorer outcomes with intensive chemotherapy, a vast majority of them are deemed ineligible for intensive chemotherapy. Multiple studies have also shown that poor performance status prior to treatment initiation affects the prognosis. An accurate comprehensive assessment is hence vital to the selection of such patients. The chemotherapy agents that have been used for AML in the medically unfit patients have also changed significant

Metabolites in aging and aging-relevant diseases: Frailty, sarcopenia and cognitive decline.

Aging shows biologically complex features with high individual variability, which reflects the exposure to several stimuli and the adaptation to them. Among them, metabolic changes are well observed as consequences or possible causes of aging. Calorie restriction extends organismal life span in experimental models. Several metabolites; for example, resveratrol or nicotinamide mononucleotide, are reported to mimic calorie restriction effects in vivo. Metabolomic research would be useful to evaluate metabolites as biomarkers in aging-relevant events and to identify metabolic regulation of aging.
