Hepatitis C virus and HIV co-infection in people with severe mental illness and substance use disorders.

OBJECTIVES: The 5-7% of adults in the United States with severe mental illness (SMI), especially the 50% who are 'dually diagnosed' with co- … ection was associated with psychiatric illness severity, ongoing drug abuse, poverty, homelessness, incarceration, urban residence and minority status. HCV-mono-infected clients continued to engage in high levels of risk behavior for HIV. CONCLUSION: In addition to efforts to identify and treat SMI patients with HIV/HCV co-infection, HCV-mono-infected clients …

Healthcare in a system of punishment: how clinicians experience care work within carceral settings.

BACKGROUND: The harms experienced by individuals incarcerated in prisons and jails have been well-documented; however, little is known about the experience of healthcare clinicians in their work in carceral institutions. Using qualitative methods, we interviewed 20 clinicians who deliver care in California’s prisons and jails to better understand their experiences. RESULTS: Participants described a carceral culture which impeded the delivery of care. These barriers to providing healthcare included limiting the clinicians’ ability to develop a trusting patient-clinician relationship and structu

Assessing childhood trauma in New Zealand prisons: Mixed method validation of the CTQ-SF.

BACKGROUND: Childhood trauma has crucial implications for correctional rehabilitation, with higher rates of abuse and neglect being associated with lower rates of treatment completion. This necessitates a valid screening tool for prison-based rehabilitation programmes. OBJECTIVE: To examine the psychometric properties of the Childhood Trauma Questionnaire-Short Form (CTQ-SF) in a New Zealand (NZ) correctional context and explore participants' experiences completing the questionnaire. PARTICIPANTS: The quantitative study involved 174 men from prison-based rehabilitation programmes (n = 75) an

Alcohol's harm to others among adults in Australia: Findings from the 2022-2023 national drug and health household survey.

BACKGROUND: Alcohol's harm to others is a significant public health and social concern, yet data on specific harm types across population subgroups in Australia are limited. METHODS: Data were drawn from the 2022 to 2023 National Drug Strategy Household Survey (N = 21,097 adults). Weighted prevalence estimates were calculated for three types of harm (verbal abuse, physical abuse, being put in fear) across sociodemographic subgroups. Adjusted logistic regression models identified independent predictors of each harm. RESULTS: In adjusted models, First Nations status and being not partnered wer

Reverse total shoulder arthroplasty is a surgical option for patients over 60 years of age with locked shoulder dislocation.

BACKGROUND: in locked dislocation of the shoulder, instead of reducing back to the glenoid, the humeral head remains incarcerated on the glenoid in a locked fashion. This clinical situation is fairly uncommon. It is essential to conduct an individual evaluation of each patient to determine the appropriate treatment. OBJECTIVE: the aim of this study was to evaluate the functional outcomes of reverse total shoulder arthroplasty (rTSA) in the treatment of locked shoulder dislocation. METHODS: patients with locked shoulder dislocation who underwent reverse shoulder prosthesis surgery and were ad

Simultaneous Management of May-Thurner Syndrome and Spigelian Hernia: A Case Report.

BACKGROUND A Spigelian hernia is a ventral hernia that occurs through the Spigelian fascia, formed by the aponeuroses of the transversus abdominis and internal oblique muscles; it may contain peritoneum or abdominal contents. This report describes an older woman who exhibited left leg swelling and intermittent right lower abdominal pain due to compression and thrombosis of the left common iliac vein by an overriding right common iliac artery (May-Thurner syndrome), in conjunction with a right-sided Spigelian hernia. CASE REPORT A 63-year-old woman presented with left leg swelling and intermitt

Integrated Diagnosis of Hepatitis B, C, and D Viruses and HIV in Populations Evaluated for Sexually Transmitted Infections: A Narrative Review and Operational Framework.

Hepatitis B virus (HBV), hepatitis C virus (HCV), and hepatitis D virus (HDV), together with HIV and bacterial sexually transmitted infections (STIs), account for a rising toll of more than one million deaths annually and overlap within shared behavioral and social networks, yet are still diagnosed through separate, pathogen-specific pathways. This narrative review synthesizes contemporary evidence on the convergence between viral hepatitis, HIV, and high-risk STI groups-men who have sex with men, pre-exposure prophylaxis (PrEP) users, people practicing chemsex, people who inject drugs, incarc

Challenging Treatment Issues: Exploring Presence and Absence in a Gay Men's Health Crisis Discursive Text.

This paper examines Gay Men's Health Crisis (GMHC) and its archival newsletter Treatment Issues (1993-2000) to explore one medium by which community-based health organizations construct collective identity and disseminate public health discourse. As the first HIV/AIDS service organization, GMHC played a pivotal role in translating complex medical information into accessible content for affected communities. Through a critical archival analysis, this study interrogates how Treatment Issues signals the presence or absence of various identity categories, revealing how public health discourse fore

Voices from the Margins: Barriers and Facilitators to HPV Self-Sampling Among Structurally Marginalized People with a Cervix in the Greater Toronto Area and Ontario.

Sex workers and formerly incarcerated people with a cervix face significant structural, interpersonal, and emotional barriers to cervical cancer screening, despite being at elevated risk for HPV infection. HPV self-sampling (HPV-SS) is a validated, user-directed method that has the potential to address these barriers, yet it remains excluded from Ontario's organized screening program. This qualitative study explored the lived experiences of structurally marginalized individuals with a cervix who were offered HPV-SS as part of a mixed-methods pilot in the Greater Toronto Area. Five virtual focu

Mental health services in jail: Identifying and quantifying barriers to implementation.

Although jails are the largest provider of mental health care nationally, access to treatment in U.S. jails is limited. Limited empirical information on factors impacting the implementation of mental health services in jail may be contributing to low treatment rates. We documented potential barriers impacting the provision of a brief cognitive-behavioral group-based intervention for people with serious mental illness in jail, including the rates of participants' recruitment, as well as starting and completing the intervention; time required for each group from recruitment to completion; and th

Exploring Polypharmacy in the BC Correctional Health Setting.

BACKGROUND: Greater burdens of comorbidity and exposure to unique environmental factors predispose incarcerated individuals to polypharmacy and inappropriate prescribing. There is limited literature investigating polypharmacy within the correctional health setting. OBJECTIVES: To determine the median number of medications prescribed per client at facilities in British Columbia, to determine the proportion of clients meeting the definition for polypharmacy, to identify the top medication categories prescribed, and to screen for potential drug therapy problems among those with polypharmacy. The use of solitary confinement and in-custody mortality in North Carolina State Prisons, 2021-2023.

PURPOSE: Solitary confinement is associated with increased mortality post-release. Little is known about its use following recent reforms and COVID-19 challenges, and its association with in-custody deaths. We investigated patterns of and associations between solitary confinement and mortality in North Carolina (NC) state prisons. METHODS: Using weekly housing data from Disability Rights NC, we created a retrospective cohort of those newly incarcerated in NC prisons between 2021 and 2023. We calculated the weekly proportion of individuals experiencing solitary confinement for 2+ weeks by type

Overdose Data to Action: A qualitative analysis of funded activities tailored for women.

BACKGROUND: According to the Centers for Disease Control and Prevention (CDC), there was a 480 % increase in overdose deaths among all women from 1999 to 2021 (Jones et al., 2024). Women are uniquely impacted by substance use and overdose due to biological and cultural differences compared to men. CDC launched the Overdose Data to Action (OD2A) cooperative agreement in 2019 with the goal of helping to mitigate the opioid overdose crisis across the United States by supporting state and local health departments. METHODS: This study focused on a review of programmatic data submitted by OD2A juri

In Deep Water? Psychopathic Traits and Related Externalizing Problems as Detrimental Adaptations to Child Adversity.

Child maltreatment is a risk factor for developing psychopathic traits and other externalizing problems. However, little is known about the relationship between specific sorts of adverse childhood experiences (ACEs), psychopathic traits, and related externalizing problems, especially in non-English-speaking countries. Therefore, this study investigates the role of ACEs in a wide range of externalizing problems, including psychopathic traits, in a sample of Spanish emerging adults. A total of 490 Spanish participants (62.4% female), aged 18-20 (M = 18.90), participated. Participants retrospecti

Recanalization of a Petersen Defect After Prophylactic Closure: A Case of Internal Hernia Following Laparoscopic Distal Gastrectomy.

Petersen hernia (PH) is a type of internal hernia in which a portion of the small intestine protrudes through a defect located between the small bowel limbs, transverse mesocolon, and retroperitoneum after any type of gastrojejunostomy. The laparoscopic approach facilitates the occurrence of this type of hernia owing to the lack of postoperative adhesions, which otherwise help prevent bowel motility and herniation. Closure of this anatomical space, formed between the jejunal mesentery, transverse mesocolon, and retroperitoneum, has been shown to significantly lower the incidence of hernia and

Preoccupied and Fearful-Avoidant Attachment Styles May Mediate the Relationship Between Poor Parental Relationship Quality and Sexual Interests in Violence.

Research suggests that adverse rearing conditions such as poor-quality relations with one's parents are associated with a range of potentially problematic sexual behaviors and interests. Less is known, however, about the mechanisms linking adverse rearing conditions and paraphilic interest, especially in non-incarcerated individuals. In a representative sample of 1600 Czech adults that oversampled people with paraphilic interests, the current study examined the association between lower quality parental relations and violent paraphilic interest (sexual interest in immobilization, biastophilia,

Epidemiology of hepatitis B and C among risk groups in Czechia.

Hepatitis B (VHB) and C (VHC) are significant global public health issues, particularly for certain risk groups. In the Czech Republic, individuals who inject drugs (IDUs), incarcerated people, and those with high-risk sexual behaviours are especially susceptible. This research investigates the epidemiology, risk factors, and effects of both diseases on these groups. It analyzes the incidence of VHB and VHC in Czechia over the last twenty years in the post-vaccine era, compares their occurrence, and identifies factors influencing their rates. A total of 28,160 VHB and VHC cases reported in the

Characterization of vaginal prokaryotes and eukaryotes microbiota and its link to sexually transmitted infections.

The vaginal microbiota plays a critical role in maintaining reproductive health. Although Lactobacillus species often dominate the vaginal environment in healthy individuals, recent studies have shown that healthy microbiota compositions can also include Lactobacillus-depleted states, as described by the Community State Types (CSTs) framework. However, disruptions in the microbiota composition, such as those caused by sexually transmitted infections (STIs), can lead to increased diversity and instability, compromising host protection. This study investigates the association between vaginal mic

Metabolic acidosis due to d-lactate in a patient with intestinal resection: Diagnostic challenges and nutritional strategies.

INTRODUCTION: Metabolic acidosis, marked by decreased plasma bicarbonate and arterial pH, is a common complication following extensive abdominal surgeries. D-lactate acidosis presents additional diagnostic challenges due to nonspecific symptoms. PRESENTATION OF CASE: A 65-year-old woman with hypertension and morbid obesity was admitted to the ICU for intestinal obstruction and peritonitis due to an incarcerated hernia. Extensive bowel resection required ileostomy and prolonged antibiotic therapy. She developed refractory metabolic acidosis, suspected to be D-lactate acidosis. Management inclu
