Impact of Coronary Collaterals on Infarct Size and Left Ventricular Ejection Fraction After Acute Myocardial Infarction: A Systematic Review and Meta-Analysis.

Many factors influence outcomes in patients with coronary artery disease (CAD) and acute myocardial infarction (AMI), including patient demographics, comorbidities, cardiac function, clinical presentation, and revascularization therapies. Percutaneous coronary intervention (PCI) is associated with reduced infarct size (IS) and improved left ventricular ejection fraction (LVEF), which have both been shown to reduce all-cause mortality and hospitalization for heart failure following AMI. Coronary collateral circulation may also have a protective effect in CAD and, like PCI, may play a role in li…

Selective pharmacological blockade of GPR39 markedly reduces no-reflow and infarct volumes in rat acute myocardial infarction when administered before reperfusion.

Our aim was to determine whether selective pharmacological blockade of G protein-coupled receptor 39 (GPR39) by the novel drug, VC108, reduces no-reflow (NRV) and infarct (INV) volumes during acute myocardial infarction (AMI). Immunocytochemistry and quantitative polymerase chain reaction (qPCR) of isolated rat cardiac cells as well as immunohistochemistry and Western blot of rat myocardium were performed for presence of GPR39. Rats underwent 1 h of coronary occlusion and 1 h of reperfusion. Groups 1 and 2 animals received drug/vehicle before or during coronary occlusion. Groups 3 and 4 receiv…

Efficacy of IntraCoronary Erythropoietin Delivery BEfore Reperfusion-Gauging Infarct Size in Patients with Acute ST-segment Elevation Myocardial Infarction (ICEBERG).

Previous clinical studies have shown inconsistent results regarding the effect of erythropoietin in ST-segment elevation myocardial infarction (STEMI). This study investigated whether directed intracoronary infusion of darbepoetin-α into ischemic myocardium before reperfusion would reduce infarct size or post-infarct remodeling in STEMI patients.Eighty STEMI patients received one of the following treatments simultaneously with the first balloon inflation: intracoronary darbepoetin-α 300 μg (n = 40) or saline (n = 40), administered via the over-the-wire balloon system. The primary endpoint was …

Monocyte Subsets Are Differently Associated with Infarct Size, Left Ventricular Function, and the Formation of a Potentially Arrhythmogenic Scar in Patients with Acute Myocardial Infarction.

To investigate the role of classical (CLM, CD14++CD16-), intermediate (INTM, CD14++CD16+), and non-classical (Non-CLM, CD14+CD16++) monocytes in scar formation after ST-elevation myocardial infarction (STEMI), evaluated with cardiac magnetic resonance (CMR). One hundred two patients with a first STEMI had serial blood analyses after 1, 3, and 7 days. A CMR was performed at 7 days and 6 months, depicting scar core (CO), border zone (BZ), and the presence of BZ channels. CLM and INTM levels progressively decreased, correlated with the scar mass, CO, and BZ at 7 days and 6 months (p < 0.05), and …

A 20-minute, free-running, whole-heart cardiac MRI to image myocardial injury following reperfused myocardial infarction: demonstration in a large animal model.

BACKGROUND: Staging irreversible tissue injury in myocardial infarction (MI) enables risk assessment for post-MI major cardiovascular events. While cardiac MRI is the preferred modality for staging the severity of tissue injury, conventional scan protocols require long acquisition times with multiple breath-held and ECG-gated acquisitions, limiting its utilization. PURPOSE: To develop a free-breathing, whole-heart, non-ECG gated cardiac MRI for staging irreversible tissue injury in MI that can be completed in <20 minutes. MATERIALS AND METHODS: A fast cardiac MRI (Biograph, Siemens Healthcar…

Design, Synthesis, and Preclinical Evaluation of a High-Affinity 18F-Labeled Radioligand for Myocardial Growth Hormone Secretagogue Receptor Before and After Myocardial Infarction.

The peptide hormone ghrelin is produced in cardiomyocytes and acts through the myocardial growth hormone secretagogue receptor (GHSR) to promote cardiomyocyte survival. Administration of ghrelin may have therapeutic effects on post-myocardial infarction (MI) outcomes. Therefore, there is a need to develop molecular imaging probes that can track the dynamics of GHSR in health and disease to better predict the effectiveness of ghrelin-based therapeutics. We designed a high-affinity GHSR ligand labeled with 18F for imaging by PET and characterized its in vivo properties in a canine model of MI. M…

Ergothioneine improves myocardial remodeling and heart function after acute myocardial infarction via S-glutathionylation through the NF-ĸB dependent Wnt5a-sFlt-1 pathway.

Myocardial infarction (MI) remains the leading cause of cardiovascular death worldwide. Studies have shown that soluble fms-like tyrosine kinase-1 (sFlt-1) has a harmful effect on the heart after MI. However, ergothioneine (ERG) has been shown to have protective effects in rats with preeclampsia by reducing circulating levels of sFlt-1. In this study, we aimed to investigate the mechanism by which ERG protects the heart after MI in rats. Our results indicate that treatment with 10 mg/kg ERG for 7 days can improve cardiac function as determined by echocardiography. Additionally, ERG can reduce …

Coronary microcirculation dysfunction evaluated by myocardial contrast echocardiography predicts poor prognosis in patients with ST-segment elevation myocardial infarction after percutaneous coronary intervention.

BACKGROUND: The mortality rate of acute ST-segment elevation myocardial infarction (STEMI) remains substantial, despite advances in treatment strategies. Coronary microcirculation dysfunction (CMD) persists after percutaneous coronary intervention (PCI) in a substantial proportion of STEMI patients. The association between CMD assessed using myocardial contrast echocardiography (MCE) and prognosis requires further elucidation. This study aimed to evaluate the impact of CMD after successful PCI on the prognosis of patients with STEMI. METHODS: We enrolled 167 patients with STEMI after PCI who …

Cardioprotective Mechanisms of Exenatide in Isoprenaline-induced Myocardial Infarction: Novel Effects on Myocardial α-Estrogen Receptor Expression and IGF-1/IGF-2 System.

Myocardial infarction (MI) is one of the main causes of morbidity and mortality in diabetic patients. The antidiabetic glucagon-like polypeptide-1 receptor (GLP-1R) agonists, such as exenatide, proved to confer cardioprotection; however, their exact mechanisms are not fully elucidated. Although the cardioprotective effect of α-estrogen receptor (ERα) activation is well established, its involvement in exenatide-induced cardioprotection has never been investigated. Moreover, modulation of insulin-like growth factor-1/2 (IGF-1/IGF-2) system by exenatide, and the consequent effect on cardiomyocyte…
