Osteogenesis imperfecta (OI) is a heritable connective tissue disorder characterized by defective type I collagen synthesis, leading to reduced bone strength and increased susceptibility to fractures, often with minimal trauma. Fractures involving the tibial tuberosity are rare and typically occur in adolescents during periods of rapid growth, usually following high-demand activities. We report a case of a 14-year-old male with type I OI who sustained a displaced tibial tuberosity avulsion fracture with epiphyseal and intra-articular extension (Salter-Harris type III, Ogden type IIIA) followin

This retrospective study evaluated complication rates for radius and ulna fractures in small breed dogs in which 1.5 mm to 2.7 mm cuttable bone plates were used for internal fixation. The medical records of all cases from 2004 to 2011 that were presented to our clinic were reviewed. Inclusion criteria were: dogs with body weight < 9 kg, fracture of the radius and ulna with open reduction, and internal fixation utilizing a cuttable bone plate. Thirty-four fractures in 31 dogs met the inclusion criteria. Of 25 dogs that were available for follow-up, all achieved union, minor complications occurr

PURPOSE: This study determined whether elbow stability could be restored with open reduction and internal fixation (ORIF) of type II coronoid fractures and evaluated the role of collateral ligament repair.
METHODS: Passive varus and valgus and simulated active vertical motion were performed using an in vitro elbow motion simulator. Varus/valgus angle and internal/external rotation were measured with the coronoid intact, with 50% removed, and after ORIF. Testing was performed with the collateral ligaments detached and repaired.
RESULTS: Vertical: stability was normal when both the lateral col

PURPOSE: To assess the treatment outcome of temporary coracoclavicular screw fixation for displaced distal clavicle fractures.
METHODS: From January 1995 to December 2003, 30 consecutive patients with Neer type II displaced distal clavicle fractures were treated with open reduction and internal fixation using a coracoclavicular screw. The screw was removed under local anaesthesia after bony union. Patients were evaluated by clinical and radiological examination. Functional outcome was assessed by a simple shoulder test.
RESULTS: There was 100% union rate. All patients returned to their pre-i

Distal humerus fractures in adults are treated by open reduction and internal fixation, which produces good results in the majority of patients. However, in elderly patients or in cases with metaphyseal comminution, stable fracture fixation still remains problematic. Anatomical joint reconstruction and primary stable osteosynthesis are often particularly difficult to achieve in this group of patients since poor bone mineral quality is frequently encountered. Consequently, longer immobilization is necessary, which is known to negatively influence functional outcome. Over the past few years, dou

METHODS: Between 1980 and 1996 we treated 23 patients for dislocated fractures of the talus. The injury was caused by a car accident in 61% and a high fall in 22%. Five patients had open wounds (22%), two developed compartment syndrome of the foot (9%) at an early stage, and 11 patients had multiple injuries. We used the classifications of Hawkins and Marti/Weber. All fractures were surgically treated by fixation with screw osteosynthesis, percutaneous wire transfixation, and/or external fixation. Fifteen patients with dislocated fractures of the talus underwent clinical and radiological follo

Hip fractures have a high economic and social cost to society. Rising healthcare costs, coupled with managed healthcare systems, have forced a close inspection of healthcare expenditures. To evaluate the impact of geriatric intertrochanteric hip fractures upon a hospital system, an economic cost-analysis was undertaken. An analysis was made of financial and hospital data of elderly patients who sustained an intertrochanteric hip fracture and subsequently underwent open reduction and internal fixation. Increasing patient age did not correlate with length of stay, overall hospital cost, or net h

Fractures of the scaphoid are most common in young men; immobilisation in a cast usually means a long period away from work and athletic activities. Early rigid fixation has been shown to promote a rapid functional recovery, but open reduction and internal fixation is technically demanding with the dangers of damage to the radiocarpal ligaments, the scaphotrapezial joint, and the blood supply of the scaphoid. For minimally displaced or undisplaced B1 or B2 fractures, these problems can be overcome by percutaneous fixation. We report our technique and the results of a pilot study in 15 patients

Thirty polytraumatized patients underwent immediate osteosynthesis of two or more long bone fractures for a total of 82 operations (plus an additional 14 for pelvis, hand, and foot fractures). Since 1983, according to the modern and rational concepts of "all at one time treatment", open fractures as well as associated closed lesions are stabilized immediately if the patient's general condition permits. This type of procedure requires adequate facilities as well as availability of and collaboration between teams of specialists, but it is inarguably advantageous for the patient (relief of pain,

BACKGROUND: Although various implants exist for the fixation of isolated greater tuberosity fractures, few implants are specifically designed for such fractures. The purpose of this study was to investigate the clinical and radiologic outcomes of open reduction-internal fixation with a low-profile anatomic locking plate for comminuted greater tuberosity fractures of the proximal humerus.
METHODS: From November 2012 to February 2018, 24 patients with displaced and comminuted isolated greater tuberosity fractures were treated with the new low-profile anatomic locking plate. To determine clinica

INTRODUCTION: Lateral humeral condyle fractures typically require a longer period of internal fixation than other distal humeral fractures due to the increased risk of non-union. K-wires can be buried and left in situ until union or they can be left unburied and require removal after four weeks, with plaster immobilisation until union. There is no consensus as to whether wire burial is preferable or not. The aim of this study was to determine whether K-wire burial is associated with more complications than non-buried wires in treating lateral condyle fractures of the elbow.
METHODS: All patie

A retrospective study of 27 pediatric patients with femoral shaft fractures treated by external fixation was made to identify complications and evaluate outcomes. The average age at the time of injury was 8 years, 9 months (range 5 years, 6 months to 13 years, 2 months). Sixteen fractures were isolated, and nine were associated with polytrauma. There was only one open fracture. Data obtained from chart review (n = 27), radiographs (n = 27), physical exam (n = 16), and questionnaire (n = 21) identified eight major complications (30%) in six patients and 29 minor complications (107%) in 20 patie

INTRODUCTION: Clavicle fractures are common in children, representing 7-15% of pediatric fractures, typically healing with conservative management. Non-union is exceedingly rare, especially in children under 3 years of age.
CASE REPORT: We report a case of a 3-year-old male with traumatic midshaft clavicle fracture non-union, presenting 2 years post-injury with persistent pain, swelling, deformity, and restricted shoulder function. Radiographs revealed a hypertrophic non-union with significant displacement and 2 cm shortening. The patient was managed surgically with open reduction, internal f

Posterior wall components of acetabular fractures are challenging injuries to treat due to the frequency of comminution, marginal impaction and high contact forces of the femoral head on the posterior wall. A variety of fixation techniques for these components have been utilized including conventional buttress plating and free screws. The objective of this report is to describe the technical application of the DePuy Synthes 2.7mm nonlocking hindfoot and midfoot plate, a 5-hole implant (the "H-plate") (DePuy Synthes; Raynham, MA), in the surgical treatment of posterior wall fractures and to pre

Gaucher disease (GD) is an inherited lysosomal storage disorder caused by glucocerebrosidase (GCase) deficiency. A 35-year-old male patient was admitted to our hospital due to left thigh pain and restricted mobility for 10 h. Following comprehensive evaluations, the patient was diagnosed with GD complicated by a pathological fracture of the left femur. He has a known L444P mutation, a suspected pathogenic A170H mutation, and an A271 V mutation of uncertain significance not in GD databases. However, a potential association with the disorder cannot be excluded. We speculate that the patient's ma

Most proximal humeral fractures in the elderly population are related to osteoporosis. Several methods have been proposed to treat surgical neck fractures of the proximal humerus in elderly people. This study investigates a new method of intramedullary pinning with tension-band wiring. From June 1998 to March 2001, 10 female patients with a mean age of 73.0 years and displaced two- or three-part surgical neck fractures of the proximal humerus were studied. Two intramedullary pins were used with tension-band wiring via a deltopectoral approach with minimum dissection. The mean follow-up was 20.
