Effects of experimentally induced low back pain on the sit-to-stand movement and electroencephalographic contingent negative variation.

It is becoming increasingly evident that people with chronic, recurrent low back pain (LBP) exhibit changes in cerebrocortical activity that associate with altered postural coordination, suggesting a need for a better understanding of how the experience of LBP alters postural coordination and cerebrocortical activity. To characterize changes in postural coordination and pre-movement cerebrocortical activity related to the experience of acutely induced LBP, 14 healthy participants with no history of LBP performed sit-to-stand movements in 3 sequential conditions: (1) without experimentally indu

Functional rehabilitation interventions for chronic ankle instability: a systematic review.

CONTEXT: Functional rehabilitation is often employed for ankle instability, but there is little evidence to support its efficacy, especially in those with chronic ankle instability (CAI). OBJECTIVE: To review studies using both functional rehabilitation interventions and functional measurements to establish the effectiveness of functional rehabilitation for both postural control and self-reported outcomes in those with CAI. EVIDENCE ACQUISITION: The databases of Medline, SPORTDiscus, and PubMed were searched between the years 1988 and 2008. Inclusion criteria required articles to have used a

The 2009 Carl Ludwig Lecture: Pathophysiology of the human sympathetic nervous system in cardiovascular diseases: the transition from mechanisms to medical management.

Sympathetic nervous system responses typically are regionally differentiated, with activation in one outflow sometimes accompanying no change or sympathetic inhibition in another. Regional sympathetic activity is best studied in humans by recording from postganglionic sympathetic efferents (multiunit or single fiber recording) and by isotope dilution-derived measurement of organ-specific norepinephrine release to plasma (regional "norepinephrine spillover"). Evidence assembled in this review indicates that sympathetic nervous system abnormalities are crucial in the development of cardiovascula

Walkaround: mobile balance support for therapy of walking.

We describe here the Walkaround, a new rehabilitation device that allows walking without hand support of individuals with limited ability to control posture. The design of this device was motivated by the recent development of functional electrical therapy of walking in individuals with hemiplegia, who in many cases need postural and body weight assistance while walking. The Walkaround assists humans with a compromised posture, inability to fully support the body weight on one leg, and prevents their falls. These three features are accomplished by an ergonomic lumbar belt being attached to a r…

The moulded baby syndrome: incidence and risk factors regarding 1,001 neonates.

Postural deformities are frequent in neonates. The moulded baby syndrome (MBS) comprises one or more of the following disorders: plagiocephaly, torticollis, congenital scoliosis, pelvic obliquity, adduction contracture of a hip and/or malpositions of the knees or feet. We analysed the incidence of MBS in healthy neonates and identified the risk factors of its composing elements. One thousand and one healthy neonates were examined on the second or third day of life by the same paediatrician. Familial, obstetrical, perinatal history and putative risk factors for postural deformities were collect

Posturographic analysis of balance control in patients with haemophilic arthropathy.

Musculoskeletal disorders in haemophiliacs represent the highest percentage of lesions, giving rise to haemophilic arthropathy (HA) which predominantly affects lower limbs, influencing postural control, standing and walking. Leading a sedentary lifestyle seems to influence strength and muscular resistance in haemophiliacs which, in turn, are related to articular stability and the prevention of articular degenerative processes. The objective of this work was to study alterations in balance to subsequently evaluate the appropriate therapeutics and how this influences the development of arthropat

Artificial neural network: a new diagnostic posturographic tool for disorders of stance.

OBJECTIVE: To determine the accuracy of diagnoses made with artificial neural network techniques (ANNW) that identify postural sway patterns typical for balance disorders. METHODS: Body sway was measured by means of posturography during 10 test conditions of increasing difficulty. From a database of 676 subjects 60 training cases (TCs) and 60 validation cases (VCs) were selected in which the following diagnoses had been established clinically: normal subject (NS), postural phobic vertigo (PPV), anterior lobe cerebellar atrophy (CA), primary orthostatic tremor (OT), and acute unilateral vestib

HS1-BP3 gene variant is common in familial essential tremor.

Essential tremor (ET) is a movement disorder characterized by a postural or kinetic tremor of the hands, head, or voice. It is typically a familial condition and affects 1% to 4% of the general population. The trait is genetically linked to chromosome 2p in some families. A variant (828C-->G) in exon 7 of the hematopoietic-specific protein 1 binding protein 3 gene (HS1-BP3) on chromosome 2p recently has been found to segregate with ET in 2 families. To determine the frequency of this variant in a larger series, we studied patients with ET, Parkinson disease (PD), and controls without tremor. A

Early motor disturbances in Rett syndrome and its pathophysiological importance.

Assessment of the development of motor function of Rett syndrome (RTT) revealed hypotonia with failure of crawling and disturbance in skillful hand manipulation are shown as early motor signs. Clinical evaluation has revealed the former as postural hypotonia with failure in locomotion and neurophysiological examinations have showed this to be due to hypofunction of the aminergic neurons of the brainstem. The latter signs are considered to indicate dysfunction of the corticospinal tract at higher levels. As the signs appear along with deceleration of head growth, dysfunction of the noradrenergi

A review of standing balance recovery from stroke.

Recently, interest in the mechanisms underlying balance recovery following stroke has grown, because insight into these mechanisms is necessary to develop effective rehabilitation strategies for different types of stroke. Studies dealing with the recovery of standing balance from stroke are, however, limited to rehabilitation inpatients with a unilateral supratentorial brain infarction or haemorrhage. In most of these patients, stance stability improves in both planes as well as the ability to compensate for external and internal body perturbations and to control posture voluntarily. Although …

Computer-assisted detection of systolic murmurs associated with hypertrophic cardiomyopathy: a pilot study.

A pilot study was conducted to ascertain the level of agreement between auscultatory findings derived from heart sound recordings by a cardiologist and the results of a computer-based heart sound analysis algorithm. Heart sound recordings were obtained from volunteer subjects previously diagnosed with hypertrophic cardiomyopathy. Twenty-second recordings were obtained at each of 4 standard auscultatory locations on the precordium in 2 postures: standing and reclining. Detailed auscultatory findings were derived by a cardiologist, who listened to the heart sound recordings and was blinded to th…

Autonomic function in cyclic vomiting syndrome and classic migraine.

Cyclic vomiting syndrome is an idiopathic disorder characterized by attacks of severe vomiting, interspersed with normal periods, and found in patients with a family history of migraine headaches. In this report, we investigated the characterization of the autonomic abnormalities in cyclic vomiting syndrome, contrasting them with values in pediatric population, as well as adults with migraine headache. We studied five groups: 41 normal pediatric controls (NPC), 12 patients with pediatric chronic vomiting (PCV), 15 patients with cyclic vomiting syndrome (CVS), 21 adults patients with migraine h

Blood pressure and heart rate response of panic disorder patients receiving imipramine in a dose-response treatment paradigm.

This article reports the effects of imipramine on heart rate and blood pressure in panic disorder patients who participated in an 8-week double-blind dosage response treatment protocol. At the end of a placebo baseline, patients were randomly assigned to placebo or one of three weight-adjusted imipramine dosages: low (0.5 mg/kg per day), medium (1.5 mg/kg per day), or high (3.0 mg/kg per day). It was demonstrated that imipramine had no significant effect on sitting or standing diastolic or systolic blood pressure. Although there was a trend toward a systolic blood pressure drop with positional
