Lichen Simplex Chronicus: Clinical Perspectives and Emerging Therapeutic Strategies.

Lichen simplex chronicus (LSC), also known as neurodermatitis, is a common chronic pruritic dermatosis defined by lichenified plaques resulting from persistent scratching. Though often secondary to underlying dermatologic, systemic, or psychological triggers, LSC represents a distinct clinical entity with significant morbidity. The hallmark itch-scratch cycle contributes not only to visible skin changes but also to substantial sleep disruption, emotional distress, and functional impairment. Psychological stress, anxiety, and depression are frequent comorbidities and can further perpetuate dise

Psychosis in Huntington's disease: a review and comparison with schizophrenia.

Psychosis is a relatively rare phenomenon in Huntington's disease (HD) yet it occurs more commonly amongst individuals with HD than in the general population. Its presence is associated with significant distress and caregiver burden. This review evaluates the epidemiology, aetiology, phenomenology, neurobiology and treatment of psychosis in HD, drawing comparisons with schizophrenia as an archetypal psychotic disorder. We conducted a detailed literature search and narrative synthesis and found that prevalence estimates of psychosis in HD varied widely (4.1-17.6 %). While generally more common

Neurodegenerative disorders, metabolic icebergs, and mitohormesis.

Neurodegenerative disorders are typically "split" based on their hallmark clinical, anatomical, and pathological features, but they can also be "lumped" by a shared feature of impaired mitochondrial biology. This leads us to present a scientific framework that conceptualizes Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS), and Huntington's disease (HD) as "metabolic icebergs" comprised of a tip, a bulk, and a base. The visible tip conveys the hallmark neurological symptoms, neurodegenerative regions, and neuronal protein aggregates for each disorder. The

Sports participation & childhood neurocognitive development.

Various psychosocial factors like collaboration inherent to team sports might provide a more dynamic environment for cognitive challenges that could foster enhanced neurocognitive development compared to individual sports. We investigated the impact of different organised sports on neurocognitive development in children (N = 11,878; aged 9-11) from the Adolescent Brain Cognitive Development (ABCD) study. Participants were classified into four categories based on their sports involvement at baseline and two years later: none, individual-based, team-based, or both. Cross-sectional and longitudin

Tuberous Sclerosis Complex as a Social Determinant of Health, Alongside Its Psychiatric Comorbidities: A Case Report.

Tuberous sclerosis complex (TSC) is a neurocutaneous disease that manifests across multiple body systems. While there are substantial guidelines and protocols for managing the physical presentation of the disease, managing the psychosocial factors and the adverse effects as a social determinant of health is complex and unclear. This study discusses a patient with TSC who was hospitalized for pneumonia and how both her psychiatric and somatic symptoms were managed. Here we present the case of a 38-year-old Caucasian female with shortness of breath and generalized weakness. She had a past medica

Parallel Hatching: A New Method of Hand Deformity Surgery in Epidermolysis Bullosa.

PURPOSE: In dystrophic epidermolysis bullosa (EB), progressive blistering, scarring, and fusing of digits lead to mitten-hand and pseudosyndactyly in early childhood, inflicting psychosocial stress and low self-esteem in EB patients. Treatment is a challenge in the hand surgery field because of the recurring nature of this inherited disease. We aim to evaluate the results of our surgery method for the hand deformities in EB patients. METHODS: We retrospectively reviewed the chart of the EB patients from 2013 to 2023 and included the patients that had the surgery by the same surgeon with the t

Obesity and its impact on female reproductive health: unraveling the connections.

In the modern era, the escalating global prevalence of obesity has profound implications on female reproductive health. Obesity, transcending mere lifestyle choices, has evolved into a complex disorder affecting physiological and metabolic functions. Concurrently, female infertility is rising as a significant global health issue. Obesity, with its extensive systemic effects, is pinpointed as a major disruptor. The convergence of these health challenges reveals a multifaceted scenario: on one hand, obesity directly impacts female reproductive health, particularly in the context of conditions li

Clinical implication of sarcopenia in patients with acute decompensated heart failure: Design and rationale.

BACKGROUND: Sarcopenia is widely recognized as an age-related syndrome that involves a progressive loss of skeletal muscle mass and muscle strength. Many studies have shown that sarcopenia is associated with disease severity and exercise intolerance, as well as an independent predictor of mortality in patients with chronic heart failure. However, there is little research on the impact of sarcopenia on patients hospitalized with acute decompensated heart failure (ADHF). METHODS: This is a prospective, multicentre, observational cohort study. Enrolment of at least 500 participants adults aged o

Conditioned pain modulation, kinesiophobia, and pain catastrophizing as prognostic factors for chronicity in a population with acute low back pain: An exploratory prospective study.

BACKGROUND: Psychosocial factors and alteration of the somatosensory functions have been associated with persistent low back pain (LBP). A decreased capacity of the central nervous system to modulate pain has been suggested as a potential contributor to the persistence of pain. OBJECTIVE: To investigate whether conditioned pain modulation (CPM), initial symptoms/disability, kinesiophobia, and pain catastrophizing is associated with the transition from acute to chronic LBP. DESIGN: Prospective cohort study. METHOD: Fifty participants presenting with acute LBP (<6 weeks) took part in three ev

Early Daily Activity: Development and description of an occupation-based intervention for surgically repaired distal radius Fractures.

OBJECTIVE: To describe the theoretical development and structure of an occupation-based intervention for people with a surgically repaired distal radius fracture. INTERVENTION DEVELOPMENT AND RATIONALE: The Early Daily Activity (EDA) intervention uses the performance of strategically selected daily activities as the primary rehabilitative strategy. Occupation-based interventions are recommended for hand injury rehabilitation but are often poorly described and lack explicit theoretical underpinnings. The EDA-intervention was developed from exploratory research that informed the theory and stru

Chronic non-specific low back pain disability and depressive symptoms in working men and women: a cross-sectional study.

Chronic non-specific low back pain (LBP) is a serious public health issue that impairs the functional abilities of working men and women. The burden and experience of chronic non-specific LBP are largely influenced by psychological and psychosocial aspects. The objective was to investigate the association between the severity of chronic non-specific LBP disability and depressive symptoms in a sample of Croatian working active men and women with an age distribution from 35 to 65 years. The Roland Morris Disability Questionnaire (RMDQ) assessed disability, whereas the Beck Depression Inventory-I

Spinal cord stimulation combined with exercise in patients diagnosed with persistent spinal pain syndrome. Study protocol for a randomized control trial.

INTRODUCTION: Administration of spinal cord stimulation to individuals with PSPS-T1/2 may induce supraspinal descending activation. Similarly, exercise is recognized as a fundamental aspect of spinal pain management. Studies have demonstrated its impact on neurophysiological factors, including the release of spinal and supraspinal beta-endorphins, which activate μ-opioid receptors. Therefore, the purpose of this study will be to examine the effect of SCS in combination with lumbo-pelvic stability core training on perceived low back pain, quality of life and disability in PSPS-T2 patients. A survey of practice on the use of condition-specific patient reported outcome measures with patients who have distal radius fractures.

INTRODUCTION: Distal radius fractures (DRF) are common injuries seen by therapists. Patient Reported Outcomes Measures (PROMs) are a way to measure recovery, yet previous research suggests PROM utilisation is low amongst therapists. This study looks at current use of condition specific PROMs amongst UK therapists treating patients with DRF and explores possible barriers and facilitators to their use. METHODS: An online anonymised cross-sectional survey hosted on Qualtrics software was distributed to UK physiotherapists, occupational therapists, and therapy technicians via special interest gro

Are psychedelic medicines the reset for chronic pain? Preliminary findings and research needs.

Chronic pain is a leading cause of disability, reduced productivity, healthcare seeking behavior, and a contributor to opioid overdose in the United States. For many people, pain can be satisfactorily managed by existing medicines and comprehensive psychosocial treatments. For others, available treatments are either ineffective or not acceptable, due to side effects and concerns about risks. Preliminary evidence suggests that some psychedelics may be effective for certain types of pain and/or improved quality of life with increased functionality and reduced disability and distress in people wh
