The effect of theophylline on sleep-disordered breathing in patients with stable chronic congestive heart failure.

OBJECTIVE: To determine the prevalence of sleep-disordered breathing in patients with stable, optimally treated chronic congestive heart failure and the effect of short-term oral theophylline therapy on periodic breathing in these patients. METHODS: Patients with stable, optimally treated chronic congestive heart failure were monitored by polysomnography during nocturnal sleep. The effects of theophylline therapy on periodic breathing associated with stable heart failure were observed before and after treatment. RESULTS: Patients were divided into two groups. Group I (n = 21) consisted of in

Estimating sleep disordered breathing based on heart rate analysis.

Heart rate variability and the analysis of the ECG with ECG derived respiration has been used to diagnose sleep disordered breathing. Recently it was possible to distinguish obstructive sleep apnea and central sleep apnea. This can be achieved by analyzing both, heart rate variability and the more mechanically induced ECG derived respiration in parallel. In addition the analysis of cardiopulmonary coupling facilitates to predict the personal risk factor for cardiovascular disorders. The analysis of heart rate, ECG and respiration goes beyond this analysis. Some studies indicate that it is poss

The role of polysomnography in Parkinson's disease: objective sleep assessment and clinical indications.

Sleep disorders (SDs) are highly prevalent in Parkinson's disease (PD), affecting up to 80% of patients during the disease and significantly contributing to impaired quality of life, cognitive decline, motor worsening, fall risk, and increased caregiver burden. Polysomnography (PSG) remains the gold standard for objective sleep assessment in PD and plays a central role in differential diagnosis and therapeutic planning. This review examines the spectrum of SDs identifiable by PSG, including rapid eye movement (REM) sleep behavior disorder (RBD), obstructive sleep apnea (OSA), periodic limb mov

Sleep-disordered breathing in acute ischemic stroke and transient ischemic attack: effects on short- and long-term outcome and efficacy of treatment with continuous positive airways pressure--rationale and design of the SAS CARE study.

OBJECTIVES: Sleep-disordered breathing represents a risk factor for cardiovascular morbidity and mortality and negatively affects short-term and long-term outcome after an ischemic stroke or transient ischemic attack. The effect of continuous positive airways pressure in patients with sleep-disordered breathing and acute cerebrovascular event is poorly known. The SAS CARE 1 study assesses the effects of sleep-disordered breathing on clinical evolution, vascular functions, and markers within the first three-months after an acute cerebrovascular event. The SAS CARE 2 assesses the effect of conti

Hypersomnia-sleep apnea due to micrognathia. Reversal by tracheoplasty.

A 67-year-old woman with acquired micrognathia developed severe daytime hypersomnia, loud snoring, nocturnal enuresis, encopresis, and hypertension. A polysomnogram demonstrated 564 sleep apneas, primarily obstructive, recurrent hypoxia, a bradytachycardia, and absent stages III, IV, and REM sleep. Endoscopy during sleep revealed recurrent active closure of the upper pharynx associated with loud snoring. A tracheoplasty was done because of severity of symptoms and failure of conservative therapy. Dramatic improvement in sleepiness and hypertension occurred within 48 hours. On postoperative nig

Sleep Disorders Affecting Prognosis of Dental Treatment.

This study provided an overview of the knowledge on the main sleep-related disorders and conditions affecting the prognosis of dental treatment: sleep bruxism (SB), obstructive sleep apnea (OSA), and gastroesophageal reflux disease (GERD). Current scientific evidence seems to suggest that these phenomena (ie, SB, OSA, GERD) belong to a circle of mutually relating sleep disorders and conditions where dental practitioners can play a key role in diagnosis and treatment.

Sleep disordered breathing in patients with chronic obstructive pulmonary disease.

Sleep has effects on breathing, including changes in respiratory control, airways resistance and muscular contractility. These sleep-related modifications in the respiratory system do not induce adverse effects in healthy subjects, but may cause problems in patients with chronic obstructive pulmonary disease (COPD). Hypo-ventilation causes the most important gas-exchange alteration during sleep in COPD patients, leading to hypercapnia and hypoxemia, especially during rapid-eye-movement (REM) sleep. Blood gases alterations lead to increased arousals, sleep disruption, pulmonary hypertension and

Experience With Adaptive Servo-Ventilation Among Veterans in the Post-SERVE-HF Era.

BACKGROUND: The sleep medicine community has struggled to identify the ideal role for adaptive servo-ventilation (ASV) therapy following a study that found increased mortality in patients with central sleep apnea (CSA) and heart failure with reduced ejection fraction who used ASV therapy. We aimed to identify characteristics of patients who benefit from ASV therapy. METHODS: We performed a record review of all patients treated with ASV therapy at the Hampton Veterans Affairs Medical Center in Virginia from January 1, 2015, to April 30, 2020. Clinical and polysomnographic characteristics of pa

Polygraphic evaluation of night-to-night variability in sleep characteristics and apneas in infants.

The study was designed to evaluate whether results of a single overnight recording session are sufficient for the study of sleep profiles and detection of apneas in infants, or whether it would be beneficial to extend the recording period. Nineteen infants were recorded during successive nights. Eight of the 19 infants were studied after an idiopathic apparent life-threatening event, whereas the other 11 were healthy. There were 13 boys and six girls, with a median age of 11 weeks (range 5-36 weeks). All infants were recorded polygraphically during 2 nights, and 11 were recorded during 3 succe

Multiple system atrophy manifesting as complex sleep-disordered breathing.

A 56-year-old female presented with multiple system atrophy (MSA) and pure obstructive sleep-disordered breathing (SDB) with an apnea hypopnea index of 29.6/h. However, after the elimination of obstructive events, central apneas emerged with a central apnea index (CAI) of 45.3/h as determined by continuous positive airway pressure titration. Adaptive servoventilation titration decreased the CAI; however, it remained at 14.2/h. There are two different underlying causes of SDB in MSA patients, one is upper airway obstruction at the glottic level, and the other is central components owing to the

Repeated inspiratory occlusions in anesthetized rats acutely increase blood coagulability as assessed by thromboelastography.

Many of the components contributing to coagulability are enhanced by repeated episodes of hypoxia, as occurs in obstructive sleep apnea, but no one has yet measured the global hemostatic properties of blood in an animal model of this disease. Using thromboelastography, a hemostatic assay, we measured hemostasis in six pentobarbital-anesthetized rats before and after 3h of repeated inspiratory occlusions lasting 30s applied every 2 min and compared the results to those in six identically prepared rats before and after 3h of resting breathing. Rats subjected to occlusions displayed faster onset

Advancements, challenges, and prospects of explainable AI in sleep disordered breathing.

Rapid advancements in artificial intelligence (AI), in combination with increased availability of rich large-scale clinical data, has paved the way for promising implementations in both diagnosing/subtyping as well as managing of sleep disordered breathing (SDB). A central strength of AI in this regard is how it facilitates analysis of complex multidimensional/modal data, for example pertaining to comorbidities and so-called treatable traits. However, the utility of such applications remains somewhat limited, as most AI models are so-called “black boxes”, for which it is not intuitive to asses

Impaired sexual maturation associated with sleep apnea syndrome during puberty: a case study.

A 20-year-old hypogonadal man was discovered to have had obstructive sleep apnea syndrome--secondary to hypertrophied tonsils, adenoids, and uvula--spanning the years of puberty. All-night polysomnographic recordings and 24 hr measurements of plasma luteinizing hormone (LH) concentrations (sampling at 20 min intervals) were performed before and after combined tonsillectomy, adenoidectomy, and uvulectomy. Two weeks preoperatively, nocturnal sleep was markedly disturbed by 407 apneic episodes, and the patient was found to be hypogonadotropic. Daytime LH concentrations were in the low-normal rang

Lateral sleeping position reduces severity of central sleep apnea / Cheyne-Stokes respiration.

INTRODUCTION: The influence of sleeping position on obstructive sleep apnea severity is well established. However, in central sleep apnea with Cheyne Stokes respiration (CSA-CSR) in which respiratory-control instability plays a major pathophysiologic role, the effect of position is less clear. STUDY OBJECTIVES: To examine the influence of position on CSA-CSR severity as well as central and mixed apnea frequency. METHODS: Polysomnograms with digitized video surveillance of 20 consecutive patients with heart failure and CSA-CSR were analyzed for total apnea-hypopnea index, mean event duration,

Classification and definition of respiratory disorders during sleep.

Growing interest in sleep disorders has led to increased research in this direction. Increasingly sophisticated instrumental tests have disclosed new breathing patterns and complex syndromes. The initial identification of obstructive apneas was followed by studies characterizing snoring, hypopneas, respiratory effort-related arousal and flow limitation events. Since Pickwickian syndrome, an historical term currently deemed obsolete and confusing, sleep investigations have differentiated secondary hypoventilation, central hypoventilation and syndromes resulting from narrowing of the upper airwa
