Binocular function in pseudophakic children.

PURPOSE: The author reports the results of visual and binocular tests performed on patients who had primary insertion of intraocular lenses following the removal of cataracts in their childhood. METHODS: Monocular and binocular tests were performed on 21 patients using the same equipment for sensory testing for binocularity on all patients before the history was abstracted from the clinical records. These patients were selected from a consecutive series and observed for a minimum of 5 years by their surgeons. RESULTS: The mean age at surgery performed on the first eye was 6.3 years. The mean

Linear nevus sebaceous syndrome in a patient with atypical associated abnormalities.

A 3-year-old girl was diagnosed as having linear nevus sebaceous syndrome. The nevus sebaceous was located on the left side of the face and neck. An unusually large lacrimal gland was completely ectopic below the entire left conjunctiva, with secondary ectropion of the left upper eyelid. The patient had the unusual finding of dentigerous cysts in both maxillary sinuses. Computed tomography scan showed atrophy of the left cerebral hemisphere, accompanied by a history of seizures; furthermore, the patient was mentally retarded. This phenotypic pattern of linear nevus sebaceous syndrome is typica

Retinal detachment despite aggressive management of aggressive posterior retinopathy of prematurity.

Posterior retinopathy of prematurity (ROP) is unusual in its atypical features and its aggressive, rapidly progressive course. It is more difficult to recognize and to treat, with many of these eyes progressing to retinal detachment despite multiple treatments with laser or cryotherapy. The authors present a case of aggressive posterior ROP refractory to multiple laser treatment. This patient was successfully treated with intravitreal bevacizumab, but required repeat treatment 4 months later. The second injection with bevacizumab was followed by progression to retinal detachment requiring surg

High-resolution spectral domain optical coherence tomography images of Alström syndrome.

Alström syndrome is a multisystemic disorder characterized by cone-rod dystrophy, hearing loss, obesity, insulin resistance and hyperinsulinemia, type 2 diabetes mellitus, dilated cardiomyopathy, and progressive hepatic and renal dysfunction. The cone-rod retinal dystrophy usually develops within a few weeks after birth. The authors examined a young boy with Alström syndrome by means of microperimetry and spectral domain optical coherence tomography (OCT). Instead of the typical alterations observed in cone-rod dystrophies, the characteristics of the central foveal tissue suggest signs of reti

Congenital cystic eye: a clinicopathologic study.

Congenital cystic eye is an extremely rare congenital ocular anomaly where a cystic mass or neuroglial tissue replaces normal eyeball secondary to failure of invagination of the optic vesicle. The authors describe a case of a 13-year-old girl, including clinical, imaging, intraoperative, and histopathological findings of this uncommon malformation. In addition, it was associated with ectopic glial tissue in the brain, which is also a rare association of this anomaly.

Case report: Duane Retraction Syndrome associated with hand anomaly.

Duane Retraction Syndrome is a congenital eye movement disorder characterized by a failure of cranial nerve VI to develop normally, resulting in restriction or absence of abduction, restricted adduction and narrowing of the palpebral fissure and retraction of the globe on attempted adduction. Patients with Duane Retraction Syndrome appear to have a significant increase in the number of associated congenital malformations. In the present paper, the authors report a case of Duane Retraction Syndrome with a unique hand abnormality not reported previously.

Acute bilateral cataract with phacomorphic glaucoma in a girl with newly diagnosed type 1 diabetes mellitus.

The authors present a rare case of acute bilateral cataract with phacomorphic glaucoma in a girl with newly diagnosed type 1 diabetes mellitus without a known history of ocular problems. Within 3 months after the diagnosis of diabetes mellitus, she presented with high intraocular pressure. Her visual acuity was limited to hand motions. The patient required immediate surgical intervention. Postoperatively, the intraocular pressure normalized and bilateral visual acuity was 6/6.

Pigmented epithelioid melanocytoid tumor of the ocular adnexa.

An 8-year-old girl with a history of microphthalmia in the right eye presented with a left medial upper eyelid mass with a dark blue-green nodule that could be seen through the skin but did not appear to involve the overlying epidermis. A biopsy demonstrated a pigmented epithelioid melanocytoid tumor with rare mitoses arising in association with a congenital nevus and positive margins. Due to the pathological findings, the patient underwent excision of the tumor with 5-mm margins and a sentinel lymph node biopsy. The re-excision of the upper eyelid margins demonstrated residual benign congenit

Two cases of congenital retrobulbar cyst or peripapillary staphyloma with an alcoholic mother.

The authors report two cases of congenital retrobulbar cyst or large peripapillary staphyloma with alcoholic mothers. In the first case, a 53-day-old newborn was referred for evaluation of microphthalmia in her left eye. Ophthalmic examination showed microcornea, microphthalmia, and an abnormally enlarged optic disc with an excavation in her left eye. Imaging revealed the presence of a cystic lesion in her left orbit that seemed to be communicated with the vitreous cavity. She did not have chromosomal abnormality. In the second case, a large peripapillary staphyloma was detected in the right e

Traumatic rupture of the superior oblique muscle tendon resulting in acquired Brown's syndrome.

Traumatic rupture of the superior oblique tendon is a rare cause of acquired Brown's syndrome. This report describes a case of traumatic rupture of the superior oblique tendon, distal to the trochlea, after injury with a plant hook. The ruptured tendon was reapproximated. Postoperatively, asymptomatic Brown's syndrome developed, although residual function of the superior oblique muscle was retained. Full-thickness upper eyelid lacerations, especially those caused by a hook, must be explored carefully to exclude superior oblique tendon rupture. Careful reapproximation of the tendon can result i

Congenital absence of salivary and lacrimal glands accompanied by growth and development retardation.

Congenital absence or hypoplasia of some or all major salivary or lacrimal glands, nasolacrimal ducts, and puncta is a rare autosomal dominant disorder. Phenotypical expression may show great variability, and the diagnosis may be difficult. Only a few cases have been reported in the literature. This report describes a patient who has all of the essential features suggestive of aplasia of the lacrimal and salivary glands, as well as retardation of growth and development. Although the characteristic features of aplasia of the lacrimal and salivary glands have been described, an association with

Macular hole in a newborn associated with forceps delivery.

A newborn who had familial high myopia presented with bilateral dense vitreous hemorrhage after forceps delivery. Vitrectomy in the right eye revealed a macular hole surrounded by pigmented cells. After subsequent surgery, the macular hole healed and remained closed during 10 years of follow-up, but profound amblyopia persisted. Macular hole is a possible complication of forceps delivery. Vitrectomy associated with inner limiting membrane peeling and gas tamponade was effective in closing macular hole in a newborn but was unable to prevent amblyopia.

Retinal involvement in patients with juvenile dermatomyositis.

The authors describe the clinical presentations and ophthalmic findings of two patients with juvenile dermatomyositis. The results of their dilated eye examinations proved to influence the treatment of the disease process because retinal pathology was used as a factor to escalate the degree of anti-inflammatory therapy. Therefore, an initial ophthalmic examination may be considered in patients with new-onset juvenile dermatomyositis.
