Management of acquired adult dropfoot.

Dropfoot is a catchall term for ankle equinus, equinovarus, and equinovalgus. The deformity can be flexible or rigid and may be associated with other pathology. In the adult, dropfoot may be congenital or acquired. Acquired dropfoot results from weakness of the ankle dorsiflexors, overpull of the plantarflexors, contracture of the soft tissues, bony deformity, or any combination of these factors. Appropriate treatment includes observation, orthotic devices, bracing, tendon transfers, arthrodesis, and neurolysis. The purpose of this paper is to review the pathophysiology and treatment of acquir

Radiographic diagnosis of acute peroneal tendon dislocation.

Acute dislocation of the peroneal tendons at the ankle is accompanied, in a significant proportion of cases, by a characteristic small shelllike avulsion fracture of the lateral malleolus, rendering the lesion diagnosable radiographically. This small fracture is usually best seen on internal rotation oblique or occasionally on anteroposterior films of the ankle. It is seen as a bone flake only a few millimeters thick but often 1 cm or more in vertical diameter lying adjacent to the posterolateral surface of the lateral malleolus. It is an infrequent injury, now usually the result of a ski acci

Chronic Ischial Avulsion Fracture Excision With Primary Proximal Hamstring Repair: A Technique.

Ischial avulsion fractures classically occur in the pediatric population and are relatively uncommon. These injuries are treated conservatively; however, in cases where there is greater than 2 cm of displacement, surgical intervention is recommended. In some cases, displaced fractures are either misdiagnosed or proper treatment is neglected, and patients who transition into adulthood are left with chronic nonunions that can become a source of pain and disability. Here we present a surgical technique for a chronic ischial avulsion fracture nonunion that is excised, and the hamstring tendons are

Anatomy of the palmar cutaneous branch of the median nerve: A review.

The palmar cutaneous branch of the median nerve (PCBm) supplies afferent innervation to the volar aspect of the hand. It consistently originates from the radial side of the median nerve, travels in relation to the tendons of the palmaris longus and flexor carpi radialis muscles, and courses superficially through fascial planes to reach the surface of the palm. Because it is at risk of injury in numerous operations, this review serves to provide a summary of anatomical findings regarding the PCBm across various studies to aid orthopedists and other clinicians in anticipating the location of the

Using Tongue Depressors as a Technique to Aid Cord Localization During Collagenase Injection For Dupuytren Disease.

Collagenase injections are used as a minimally invasive treatment for Dupuytren contracture of the palm and fingers, with good published success. Most complications are related to injection-site swelling, pain, and ecchymosis. However, the risk of flexor tendon and digital nerve injury can occur when injecting cords that cause proximal interphalangeal joint contractures. In addition, poor localization and incorrect injection of the solution into cords may result in an incomplete correction. We describe a technique to improve visualization and isolation of Dupuytren cords using a tongue depress

Abductor Reconstruction with Gluteus Maximus Transfer in Primary Abductor Deficiency during Total Hip Arthroplasty.

Abductor deficiency in native hip joint may cause severe limping and pain. It is more serious situation in case of arthroplasty due to instability and recurrent dislocation. Well-known causes of abductor deficiency are repeated surgery, chronic trochanteric bursitis, superior gluteal nerve injury, failure of repair of abductor tendon insertion to the greater trochanter. Author had experienced primary abductor deficiency during total hip replacement and treated successfully with the transfer of gluteus maximus. We'd like to introduce the operation technique with the review of literature.

Lumbar plexopathy after radical nephrectomy -A case report-.

Lumbar plexopathy is characterized by an abrupt onset of sensory disturbances, weakness, and loss of deep tendon reflexes of lower extremities. The various causes of lumbar plexopathy include trauma, infections, space-occupying lesion, vascular diseases, metabolic diseases, and the use of drugs such as heroin. Postoperative rhabdomyolysis occurs secondary to prolonged muscle compression due to surgical positioning. Herein, we report a case of lumbar plexopathy, complicating an injury to the paraspinal muscle and iliopsoas muscle that occurred in the flexed lateral decubitus position following

The acute calcific prevertebral tendinitis: report of two cases.

Acute calcific prevertebral tendinitis, which is also known as retropharyngeal calcific tendinitis and longus colli tendinitis, is an under-recognized cause of acute cervical pain produced by an inflammation of the longus colli muscle. The typical characteristics of this entity are calcifications at the superior insertion of the longus colli tendons at the C1-C2 level and fluid collection in the retropharyngeal space. The differential diagnosis includes a retropharyngeal abscess, infectious spondylitis or traumatic injury. Knowledge of the clinical and imaging findings can prevent a misdiagnos

New technique for knee flexion contracture with patella infera using patellar tendon reconstruction combined with anterior capsular shift: a case report.

A 24-year-old male presented with an open fracture of the left patella (Gustilo-Anderson grade III-A) and fracture of the left femoral lateral condyle, and subsequent patella alta suggesting rupture of the left patellar tendon. This initial injury was treated 6 months earlier in another hospital. He was referred because of limited knee motion and radiographs showed severe patella infera. By means of a combination of a contralateral BTB graft and soft tissue repair named anterior capsular shift, the knee contracture with patella infera was resolved successfully.

Deep venous thrombosis and inferior vena cava agenesis causing double crush sciatic neuropathy in Behçet's disease.

We report here the case of a 18-year-old young man with Behçet's disease who had suffered deep venous thrombosis of the right femoral and popliteal veins. Consequently, right sciatic nerve injury, drop foot and tightness of the achilles tendon also ensued. The clinical scenario was further challenged by demonstration of the agenetic inferior vena cava and epidural vein dilatations compressing the lumbar nerve roots. To the best notice of the authors, this is the first patient encompassing all these complications in the literature concerning Behçet's disease.

Lateral epicondylitis: review and current concepts.

Lateral epicondylitis is a painful and functionally limiting entity affecting the upper extremity and is frequently treated by hand surgeons. The anatomic basis of the injury to the extensor carpi radialis brevis origin appears to be multifaceted, involving hypovascular zones, eccentric tendon stresses, and a microscopic degenerative response. Although many treatments have been advocated, there is little clear consensus on which modality works best, for both conservative and operative options. In this article, we present an overview of this difficult problem and an evidence-based review of tre

[Surgical rehabilitation of the upper limbs after traumatic tetraplegia].

Patients with a cervical spinal cord injury experience a complete paralysis of their lower limbs and a partial paralysis of their upper limbs associated with a dramatic decrease of upper limbs function. Twenty years ago, surgical rehabilitation of their upper limbs started to develop, based mainly on tendon transfer procedures. We report a consecutive series of 69 such patients operated in the past 12 years, amounting to 188 rehabilitation procedures on their upper limbs. Surgery was beneficial in all cases, there was no case of functional loss, and the patients were mostly satisfied with the

Shoulder pain: the role of diagnostic injections.

Many different shoulder disorders cause similar symptoms and pain patterns. An accurate diagnosis can generally be made by obtaining a detailed history, performing a comprehensive, directed physical examination and obtaining selected radiographs. Occasionally, shoulder injections can be of great assistance in establishing a clear diagnosis and providing relief of symptoms. Subacromial space injection, acromioclavicular joint injection, intra-articular injection and injection of the biceps tendon are helpful in identifying such disorders as subacromial bursitis, acromioclavicular arthritis, inj

Iatrogenic inferior rectus palsy.

We examined two patients who developed unilateral inferior rectus muscle palsy following inferior oblique myectomy. Surgical exploration showed a normal inferior rectus muscle and tendon in both patients. Surgical correction consisted of inferior rectus resection in one patient and inferior rectus resection combined with an ipsilateral superior rectus recession in the other. We propose inadvertent excision of the lateral portion of the inferior rectus muscle during inferior oblique myectomy as the probable mechanism of inferior rectus injury.

The third metacarpal boss.

A swelling and ache on the dorsum of the wrist in relation to the base of the 3rd metacarpal bone may indicate a bony lesion or an enlarged styloid process. It may also be due to an old fracture of the process or an accessory ossicle (os styloideum). Approximately half the 14 patients (15 wrists) had a history of an injury, usually a hit with the fist. Recurrent occupational strain may also produce symptoms at this site. Excision of the metacarpal base with the adjacent capitate or trapezoid, and repair of the periosteum and extensor tendon, will remove the lump and usually relieve symptoms.

Rupture through the short head of the biceps muscle belly. A case report.

Isolated involvement of the short head of the biceps in a bicipital rupture is a rare injury. The most common location of an isolated short head rupture is through the tendon. The only reported cases of rupture through the short head muscle belly were described in a series by Gilcreest, in which one partial and one complete tear were noted. A rarely observed traumatic rupture of the short head of the biceps muscle belly occurred in a 19-year-old male water-skier. The rupture was successfully treated by surgical repair.

[Restoration after burns of the back of the hand].

In burn injuries due to their exposed situation the hands are frequently injured and in the moment of the accident they are often used as a shield in front of the face. Mainly the back of the hands are involved. In reconstruction after burns of the back of the hands the scared skin is excised under tourniquet, the displaced interdigital webs are corrected by the method of Tanzer and the skin defect is covered by a thick split-skingraft. From case to case additional operations are: replacement of lost extensor tendons, capsulotomies of the MP-joints, arthrodeses of destroid fingerjoints and amp
