Dental Arch Relationships and Reverse Headgear Effects in Southern Chinese Patients with Unilateral Cleft Lip and Palate: A Retrospective Study.

OBJECTIVE: To evaluate the severity of the dental arch relationships and the treatment outcomes of reverse headgear (RHG) in southern Chinese patients with unilateral cleft lip and palate (UCLP). DESIGN: A retrospective study. SETTING: Faculty of Dentistry, The University of Hong Kong. PATIENTS: Thirty-eight UCLP patients with complete records. Among them, 14 were later treated with RHG (RHG group) and 24 were under review (non-RHG group) before definitive orthodontic or in conjunction with orthognathic surgery. INTERVENTIONS: Study models at T1 (aged 9.4 ± 0.4 years old), prebone grafting

Correction of a severe Class II malocclusion in a patient with Noonan syndrome.

Noonan syndrome is a developmental disorder characterized by a dysmorphic facial structure, short stature, and mild mental retardation, with associated cardiac defects and skeletal malformations. It may be sporadic or inherited as an autosomal dominant or recessive trait. The incidence of occurrence is 1 in 1000 to 2500 live births. The responsible gene is located on the long arm of chromosome 12. Diagnosis of the syndrome is made by both clinical inspection and karyotype. This is the case report of a 10-year-old Mexican boy who was referred for correction of orofacial and occlusal defects.

A single-person reduction and splinting technique for ankle injuries.

Ankle injuries are one of the most common orthopaedic conditions treated in the emergency department. Initial reduction and splinting techniques of these injuries are variable and can place undue stress on the physician and cause patient discomfort. Novice and experienced practitioners have had to repeat splint application because of poor preparation, variable assistant experience, loss of fracture reduction, and improper application. We present a modified Quigley technique for ankle reduction and splinting that simplifies these issues and reduces stress on the patient and physician. The techn

Upper airway obstruction and resultant growth factors influencing malocclusions.

Upper airway obstruction is associated with many dental and skeletal malocclusions. Recognizing and removing the problems at an early age is encouraged. However, the malocclusions caused by upper airway obstruction can be addressed and corrected at different stages of dentitions using various types of appliances and orthodontic techniques. This article reports several cases, which were treated with the help of medical experts and otolaryngologists.

Soft tissue profile changes following maxillary protraction in Class III subjects.

The dentoskeletal effects of maxillary protraction (MP) therapy have been extensively investigated, while those relating to soft tissue profile changes are limited. Thus, the aim of this study was to determine the degree of soft tissue profile changes following MP therapy. The material consisted of the cephalometric films of 24 female subjects (12.69 +/- 1.08 years) with a Class III malocclusion who underwent MP therapy; these were compared with a control group of 15 females (12.13 +/- 0.63 years) with a Class I occlusion and matched for chronological age and observation period. Cephalometric …

Treatment of an adolescent with total ankyloglossia.

This is a report about the orthodontic treatment of a 13-year, 10-month-old boy with total ankyloglossia combined with a Class III occlusion. The patient's tongue was fixed to the floor of his mouth and could not be elevated at all. He had a maxillary deficiency and a mandibular protrusion with a negative overjet. However, he was able to retrude his mandible to an edge-to-edge position. Before orthodontic treatment, the ankyloglossia was surgically rectified. Orthodontic treatment was initiated to improve the patient's occlusion and facial appearance by correcting his retruded maxilla by means

The importance of the facial profile in orthodontic diagnosis and treatment planning: a patient report.

Orthodontic treatment to improve not only tooth alignment but also facial esthetics is a given in modern society. This paper illustrates this assumption with a report of a patient with a Class I, maxillary retrusion, concave profile, and retrusion of the upper and lower lips. Maxillary expansion was followed by face mask therapy and orthodontic treatment, which significantly improved the soft tissue profile by projecting the upper lip.

Nonsurgical treatment to regain hip abduction motion in Perthes disease: a retrospective review.

The purpose of this study was to document the ability of a nonsurgical program to improve restricted passive hip abduction in children with Perthes disease. Containment as a form of treatment was recommended if passive hip abduction of 30 degrees or more could be achieved. Medical records and radiographs were retrospectively reviewed for 74 children. Age at admission and onset, side, length of stay, and measurement of passive hip abduction at admission/discharge were recorded. The average increase in abduction with the hip extended was 13 degrees. Forty-two children achieved 30 degrees or more

Spinal deformity after selective dorsal rhizotomy in patients with cerebral palsy.

Selective dorsal rhizotomy is used widely as a means of treating spasticity associated with cerebral palsy. Little is known regarding the effect of the procedure on the development or progression of spinal deformity. The authors reviewed six patients with progressive deformity after rhizotomy. Prerhizotomy and postrhizotomy records of physical examinations and radiographs were reviewed retrospectively in an attempt to identify risk factors for development of and/or rapid progression of, spinal deformity. Detailed preoperative and postoperative evaluation of spinal alignment should be undertake

[Traumatic atlanto-axial dislocation in children: 7 cases].

Seven cases of atlantoaxial rotatory dislocation in children are reported. The CT scan permitted differentiation of the lesions in all the cases. Analysis of the results has led us to introduce two modifications for Fielding's classification: a distance between the atlas and odontoid of less than 5 mm is normal. sagittal dislocation must be included in the same classification. We propose the following therapeutic protocol: rotatory luxation without tears of the transverse ligament should receive orthopedic treatment, rotatory luxation with tears of the transverse ligament should have surgical

Two Class II, division 1 patients with congenitally missing lower central incisors.

Although orthodontic treatment objectives and procedures for apparent protrusion of the maxillary teeth vary among orthodontists and specific cases, the differences are even greater where there is disharmony of jaw relationship between the maxilla and the mandible. The two cases presented in this article resemble each other in appearance, but the growth patterns and reactions to appliance, as well as treatment progress, are quite different. The initial excessive overjet seemed equally severe. Treatment was started at the same age, both patients were eldest daughters, both had a convex type fac

[Fracture of the femoral diaphysis in children. Ascending or descending centro-medullary nailing? A choice of principle or of necessity?].

Over a period of 4 years, 40 children between the ages of 6 and 14 years (mean: 10 years) were treated for a fracture of the femoral shaft (39 closed fractures and 1 Cauchoix type I open fracture) by flexible medullary nailing, either ascending (5 cases) or descending (35 cases). Although it is classical to use the ascending route described by the Nancy team, the sub-trochanteric descending route was preferred for its simplicity; non-involvement of the knee by the insertion of the nail allows earlier autonomy in children. The results have been found to be satisfactory; no complications related

Anatomical and functional results after treatment of dislocated supracondylar fractures of the humerus in children.

From 1972 to 1983 105 patients were admitted with a dislocated supracondylar fracture of the humerus. The average follow-up was five years. In general the functional results of treatment were good. The anatomical results were less satisfactory, and most times concerned varus deformities of the elbow, which did not influence the elbow function. Lesions of arteries and nerves only occurred in seriously dislocated fractures. The isolated nerve injuries were reversible. Two patients developed a Volkmann's ischemic contracture; their case history is discussed. In 24 cases a second reduction was per

Sequential radionuclide bone imaging in avascular pediatric hip conditions.

Radionuclide bone imaging was performed on six patients with various hip conditions. Initial bone images revealed diminished uptake of isotope 99mTc-MDP in the capital femoral epiphysis. Following therapeutic intervention, repeat bone scans revealed normal uptake of 99mTc-MDP in the capital femoral epiphysis. Subsequent radiographs revealed that avascular necrosis had not occurred. There are two types of avascularity: the potentially reversible, and the irreversible. Attempts should be made toward early recognition of the potentially reversible avascular insult. With early recognition, surgica
