Localised gastrocnemius myositis in Crohn's disease.

A19-year-old woman presented with pain and tenderness in both calves associated with pyrexia and neutrophil leukocytosis. Gastrocnemius muscle biopsy showed a non-specific lymphocytic myositis and she was found to have positive c-ANCA, in the absence of other evidence of systemic vasculitis. Subsequent investigation of her gastrointestinal tract revealed extensive Crohn's disease. The myositis responded promptly to treatment with prednisolone 0.5 mg/kg. A review of the literature showed that localised calf pain in the setting of Crohn's disease can be caused by non-specific myositis, granuloma…

Cerebral aneurysms associated with Behçet's disease: a case report.

Cerebral aneurysms in Behçet's disease are very rare. The role of vasculitis in the aetiology of these aneurysms has not been clarified. A 57 year old man with Behçet's disease is described, who had a subarachnoid hemorrhage due to rupture of a peripheral middle cerebral artery aneurysm. He underwent a successful aneurysmal clipping. Three years later he had seizures and was found to have a new aneurysm on the contralateral peripheral middle cerebral artery as well as some radiological features of vasculitis. After 3 months of steroid therapy, the aneurysm disappeared. Although surgical treatm…

Disease of the month. The Churg Strauss Syndrome.

The Churg Strauss Syndrome is an eosinophil-associated small vessel vasculitis. Although its pathogenesis may be distinctive and the association with severe late-onset asthma typical, the clinical features during the vasculitic phase widely overlap with those of the other forms of necrotizing vasculitis, and no single clinical or histologic feature is pathognomic of the condition. Renal involvement is common, although usually mild, and even when severe it tends to respond well to treatment. The prognosis for both patient and renal survival with adequate treatment is in general good. The optima…

[Cryoglobulinemia related to the hepatitis B and C viruses].

Most cases of "essential" mixed cryoglobulinemia are due to chronic infection with the hepatitis C virus (HCV). Although cryoglobulinemia is a common finding in HCV-infected patients, it is rarely symptomatic. However, some patients develop diffuse vasculitis predominantly affecting small-caliber blood vessels, particularly those of the skin. Treatment with steroids, immunosuppressives, and more recently interferon alpha is effective in the short and medium term but fails to provide a permanent cure for the vasculitis. Cryoglobulinemia associated with hepatitis B virus (HBV) infection is less …

Challenges with Takayasu's arteritis: a case study.

Takayasu's arteritis (TA) is a systemic vasculitis that involves an autoimmune-mediated transmural degeneration of the aorta and its major branches, leading to severe arterial stenoses or occlusions with subsequent cerebral, cardiopulmonary, mesenteric, renal, or limb ischemia. Patient history and physical examination, arteriography, magnetic resonance imaging, and specific laboratory tests facilitate the diagnosis of TA. Major treatments for the condition include the use of corticosteroids, cytotoxic agents, antiplatelet drugs, percutaneous transluminal angioplasty (PTA), and surgical revascu…

[Anti-cytoplasmic antibodies of polynucle or "ANCA". A new class of autoantibodies].

The specific antigens to antineutrophil cytoplasmic antibodies (ANCA) and their relations with renal diseases begin to be well-known. In patients with systemic vasculitis, two major antigens have been recognized: proteinase 3 and myoloperoxydase. These enzymes are located in the azurophilic granules of neutrophils and migrate to the surface of these cells when they are activated. Other antigens, such as elastase, lactoferrin, CAP57 and cathepsin G, have been identified, but they are less commonly encountered. The presence of ANCA is particularly frequent in Wegener's granulomatosis, microscopi…

Autoimmune ocular disease in MRL/Mp-lpr/lpr mice is suppressed by anti-CD4 antibody.

MRL/Mp-lpr/lpr(MRL/lpr) mice spontaneously have a systemic autoimmune disease, characterized by vasculitis, lymphadenopathy, glomerulonephritis, and autoantibody formation. Among the many autoimmune lesions present are focal ocular inflammatory infiltrates, involving the choroid and sclera. These lesions appear to be related to the vasculitis seen in MRL/lpr mice and are mediated by L3T4-positive helper T-cells (CD4-positive T-cells). Systemic treatment of MRL/lpr mice with a monoclonal anti-L3T4 antibody (anti-CD4) resulted in a dramatic reduction of both the frequency and severity of the ocu…

[Prostaglandins in the pathogenesis of vascular lesions in patients with rheumatic diseases].

Using the radioimmunological method the authors studied the level of prostaglandins (PG) E and F2 alpha in 76 patients with rheumatic diseases (RD) including 34 with various forms of systemic vasculitis (SV), 30 with systemic lupus erythematosus (SLE) and 12 with systemic sclerodermia (SSD). The condition of the microcirculatory bed was assessed by the method of bulbar angioscopy. A relationship between PG and first of all the pressor series, and the nature of vascular lesions in patients with RD was established. Hyperproduction of PGE was revealed in patients with lupus nephritis and SV in wh…

Subcortical cerebral hemorrhages associated with cocaine abuse: CT and MR findings.

Four young individuals with histories of heavy cocaine abuse occurring several hours to days before the development of acute symptoms of severe headaches, disorientation, and subsequent stupor were shown to harbor subcortical cerebral hemorrhages. Thorough workup of these patients revealed no underlying pathology (i.e., arteriovenous malformations) or other possible causes such as hemorrhage into a tumor. It is well known that heroin, ephedrine, and methamphetamine use may result in cerebral vasculitis, but only one case study in the literature has reported on cerebral vasculitis with ischemic…

Coronary artery aneurysm.

The case is presented of a 28-year-old woman with systemic vasculitis, aortic insufficiency, and an aneurysm of the proximal right coronary artery. More than 100 patients with coronary artery aneurysm have been reported, and this represents the twenty-first case treated surgically. Coronary artery aneurysms occur more frequently than previously realized and are seen in approximately 1.5% of patients with coronary artery disease. In addition to arteriosclerotic aneurysm, other common types of coronary aneurysm include those occurring congenitally or from mycosis, dissection, trauma, vasculitis,…

From Nerves to Vessels: Unmasking Churg-Strauss Behind Polyneuropathy.

Eosinophilic granulomatosis with polyangiitis (EGPA), formerly Churg-Strauss vasculitis, is a rare systemic autoimmune disease. Neurological involvement is a significant complication. This report details a case of EGPA with severe, progressive polyneuropathy and atypical serological findings, highlighting diagnostic challenges and the importance of early intervention. A 55-year-old male with asthma and uncontrolled diabetes presented with progressive neuropathic pain and paraplegia. He had a history of significant eosinophilia. Clinical examination revealed a rash, neurological deficits, and s…

Isolated pulmonary capillaritis and diffuse alveolar hemorrhage in rheumatoid arthritis and mixed connective tissue disease.

STUDY OBJECTIVES: To demonstrate that pulmonary capillaritis and diffuse alveolar hemorrhage (DAH) occur and are isolated to the lung and therefore not part of systemic vasculitis at the time of the DAH episode in rheumatoid arthritis (RA) and mixed connective tissue disease (MCTD). DESIGN: Lung biopsy specimens from patients with DAH were reviewed and those with the histologic features of pulmonary capillaritis were identified. SETTING: The patients were selected from seven Denver-area general hospitals. PATIENTS: Fifty-eight patients with biopsy specimen proved pulmonary capillaritis (199…

Uptake and degradation of soluble aggregates of IgG by monocytes of patients with rheumatoid arthritis: relation to disease activity.

Monocytes from patients with rheumatoid arthritis (RA) and rheumatoid vasculitis have a diminished ability to degrade soluble complexes of aggregated IgG in the absence (mediated by Fc receptors) as well as in the presence of complement (C) (mediated by (Fc + C) receptors). To investigate whether a relation exists between the receptor mediated degradation of aggregated IgG by adherent monocytes and disease activity a longitudinal study was performed in 79 patients with RA and rheumatoid vasculitis over a period of 16 months. Adherent monocytes were incubated in vitro with 125I labelled IgG agg…

High titer of serum antiphospholipid antibody levels in adult Henoch-Schönlein purpura and cutaneous leukocytoclastic angiitis.

OBJECTIVE: To investigate a possible role of antiphospholipid (aPL) antibodies in adult Henoch-Schönlein purpura (HSP) and cutaneous leukocy … s markers for adult HSP and should be monitored as an indicator of adult HSP activity. Small-vessel vasculitis could be dependently associated with the presence of IgM anti-PS/PT antibodies. These findings suggest that aPL antibodies are closely related to the pathogenic factors that trigger the development of vasculitis.
