An Evidence-Based Quality Improvement Programme for Glycaemic Management in Critically Ill Patients Receiving Enteral Nutrition.

BACKGROUND: Hyperglycaemia is highly prevalent, affecting approximately one-third of critically ill patients receiving enteral nutrition (EN). Persistent hyperglycaemia is associated with an elevated risk of infection, increased mortality and prolonged length of stay, negatively impacting patient prognosis. Therefore, implementing an evidence-based hyperglycaemia management strategy is crucial to improve patient outcomes. AIM: This study aimed to reduce the incidence of hyperglycaemia during EN in critically ill patients through evidence-based practice. STUDY DESIGN: A non-concurrent control

Cross-economy stability of predictor rankings for adolescent school belonging and mathematics anxiety: an explainable machine-learning analysis of PISA 2022.

INTRODUCTION: School belonging and mathematics anxiety reflect distinct relational and motivational processes, but cross-economy comparisons can blur distinctions among psychological relationships, model-based importance, and questionnaire availability. METHODS: We analyzed Program for International Student Assessment (PISA) 2022 data from 613,744 students; outcome-valid samples comprised 561,339 students in 78 economies for school belonging and 475,792 students in 76 economies for mathematics anxiety. Per-economy LightGBM models and TreeSHAP rankings estimated relative predictive salience. S

Impact of postoperative BMI change on patient-reported outcomes following total knee arthroplasty: A retrospective cohort study.

BACKGROUND: The aim of this study was to investigate whether postoperative weight loss after total knee arthroplasty (TKA) is associated with improved clinical outcomes. We hypothesized that patients who experienced postoperative weight loss would demonstrate greater improvements in patient-reported outcomes than those who maintained or gained weight. METHODS: A total of 312 patients (383 knees) who underwent primary TKA for osteoarthritis between July 2017 and July 2024 were retrospectively reviewed. Percent change in body mass index from preoperative to final follow-up was calculated, and p

Intravenous Paricalcitol Versus Calcitriol for Secondary Hyperparathyroidism in Maintenance Hemodialysis: A Six-Month Single-Center Retrospective Cohort Study.

Background/Objectives: Secondary hyperparathyroidism is a frequent component of chronic kidney disease-mineral and bone disorder in maintenance hemodialysis. Calcitriol and paricalcitol are used to suppress intact parathyroid hormone (iPTH), but the comparative advantage of paricalcitol in routine practice remains uncertain. This study compared 6-month biochemical PTH control in patients treated with intravenous calcitriol or intravenous paricalcitol. Methods: In this single-center retrospective cohort study, 312 adult hemodialysis patient files from 2023 to 2025 were screened. Patients who di

Comparative child mental health and academic outcomes after primary school teachers deliver task-shifted mental health care in India.

Teachers are promising lay counselors but have limited time for traditional therapy. To viably leverage teachers to deliver care, we developed Teachers Leading the Frontlines (Tealeaf), where teachers deliver a novel therapy, "education as mental health therapy" (Ed-MH); measures are an extension of classroom management, not an added task. We investigated whether Tealeaf teachers improved child mental health symptoms and academic achievement versus teachers delivering an enhanced usual care (EUC) comparator, Responding to Students' Emotions through Education (RE-SEED), a truncated version of T

[Personalised treatment of patients with immune thrombotic thrombocytopenic purpura].

BACKGROUND: Treatment of immune thrombotic thrombocytopenic purpura (iTTP) includes plasma exchange (PEX) and immunosuppression (glucocorticoids and rituximab). The addition of caplacizumab to therapy has improved treatment outcomes in iTTP. However, the available therapies focus on the duration of drug administration and clinical response rather than ADAMTS13 activity. AIM: To evaluate the efficacy of therapy for iTTP targeting ADAMTS13 activity. MATERIALS AND METHODS: Treatment of patients with iTTP was started with PEX, prednisolone (1 mg/kg) and caplacizumab (10 mg/day). PEX was disconti
