[Pharmaceutical formulations and adherence to pharmacological treatment in psychiatry: the example of oral disintegrating tablet of olanzapine].

Adherence to treatment in psychiatric as in other chronic or recurrent conditions, is often suboptimal. A high proportion of relapses is due to non-adherence to prescribed treatment. Adherence to treatment is a potent predictor of effectiveness, both in clinical trials and cohort studies, therefore is a very relevant area where any improving tool is looked forward. Orally Disintegrating Tablets (ODT) were developed with the aim to improve patient's compliance due to their fast oral absorption. They are particularly useful in psychiatric patients who often simulate drug assumption or experience

Successful treatment of widespread eczema and sleep disruption in a 7-month-old infant with methylprednisolone aceponate 0.1%.

A 7-month-old infant, with no known food allergy or intolerance, presenting with eczema lesions affecting many parts of his body, was diagnosed with infantile eczema. A skin barrier defect, due to filaggrin deficiency, was suspected. Prior treatment with emollients was not effective and produced discomfort. After twice-daily application of methylprednisolone aceponate (MPA) 0.1% (Advantan(®)), in addition to temporary withdrawal of emollients, for 10 days. At the follow-up examination, improvements in all eczema lesions, scratching behavior and sleep quality were observed, and eczema was consi

Methylprednisolone aceponate 0.1% in the treatment of pruritic lichenified eczema in a 3-year-old child with chronic atopic dermatitis.

A 3-year-old girl with a > 2-year history of atopic dermatitis (AD) and known egg allergy, presented with an extensive AD flare including exudative eczematous lesions and lichenification on her flexures. The patient reported intense pruritus and showed signs of scratching on her trunk, arms, buttocks and thighs. After 2 weeks of twice-daily application of methylprednisolone aceponate (MPA) 0.1% (Advantan(®)), temporary withdrawal of emollients, and mild bathing/cleanser use, the eczematic condition was considered clear, and marked improvement of all treated lesions (including disappearance of

Interventions for primary (intrinsic) tracheomalacia in children.

BACKGROUND: Tracheomalacia, a disorder of the large airways where the trachea is deformed or malformed during respiration, is commonly seen in tertiary paediatric practice. It is associated with a wide spectrum of respiratory symptoms from life-threatening recurrent apnoea to common respiratory symptoms such as chronic cough and wheeze. Current practice following diagnosis of tracheomalacia includes medical approaches aimed at reducing associated symptoms of tracheomalacia, ventilation modalities of continuous positive airway pressure (CPAP) and bi-level positive airway pressure (BiPAP), and s

[Developments in research of local bisphosphonate delivery system of implant denture].

Dental implant is an advanced prosthodontic treatment widely accepted by patients with missing tooth. However, peri-implant bone loss is still an important reason which limits wider application of the implants to a certain extent. Bisphosphonates is an osteoclastic bone resorption inhibitor that is widely used in clinical practice with the function of inhibiting bone resorption and increasing bone density. As the defect of systemic BPs treatment, local application of BPs in implant has become a research hotspot recently. Calcium phosphate ceramics, polylactic acid, fibrinogen film and collagen

Zoon's plasma cell balanitis: a report of two cases treated with pimecrolimus.

UNLABELLED: Initially called (in French) "Balanoposthite chronique circonscrite benigne a plasmocytes", Zoon's plasma cell balanitis is a chronic inflammatory dermatosis affecting the glans and foreskin of uncircumcised men. The different treatment options for this condition often present partial results. Reports have shown therapeutic success using topical tacrolimus. We report the use of pimecrolimus, a homologue of tacrolimus, with good response. Two male patients diagnosed with Zoon's plasma cell balanitis, confirmed by biopsy, were subjected to daily treatment with topical pimecrolimus1%.

Oral steroids for nasal polyps.

BACKGROUND: This is an update of a Cochrane Review first published in The Cochrane Library in Issue 1, 2007.Benign nasal polyps are lesions that arise from the mucosa of the nasal cavity or one or more of the nasal sinuses. The presenting symptoms are nasal obstruction, watery anterior rhinorrhoea (excessive nasal secretions) or mucopurulent postnasal drip (or both), hyposmia and anosmia (reduced or absent sense of smell) with a concomitant alteration in taste and infrequently pain over the dorsum of the nose, forehead and cheeks. The main aim of treatment is to relieve these symptoms. The aet

Today's treatments options for onychomycosis.

Onychomycosis can be cured even if fungi are sometimes difficult to eradicate; the treatment is often lengthy and requires patience. The choice of agents should be based on numerous factors including patient's age and health, causative organism, clinical type of onychomycosis, number of affected nails and severity of nail involvement. We review current and future treatments for onychomycosis. We will also consider treatment options in patients with poor prognostic factors suggesting possible treatment failure.

Cushing's syndrome and chronic venous ulceration--a clinical challenge.

Cushing's syndrome is a condition caused by high levels of glucocorticoids, or most commonly as a result of prolonged exposure to exogenous steroids. Clinical features include diabetes, hypertension, obesity, skin atrophy, immune suppression and delayed wound healing. We report a patient with iatrogenic Cushing's syndrome, in whom long-term topical steroid therapy was used to treat varicose eczema, which contributed to the development of type 2 diabetes, morbid obesity, sleep apnoea and chronic wound sepsis. In this case, repeated hospital admissions and systemic antibiotics were associated wi

Keratomycosis in a dog treated with topical 1% voriconazole solution.

A 2-year-old Jack Russell Terrier was presented for treatment of a worsening corneal ulcer and keratomalacia following removal of a vegetative foreign body. The keratomalacia responded to topical gentamicin therapy; however, the eye became suddenly painful and at examination two areas of increased opacity had developed. Fungal keratitis was demonstrated by corneal scrape showing branching, septate fungal hyphae. Treatment with 1% voriconazole was effective in resolving the keratomycosis.

Effect of daily aspirin on long-term risk of death due to cancer: analysis of individual patient data from randomised trials.

BACKGROUND: Treatment with daily aspirin for 5 years or longer reduces subsequent risk of colorectal cancer. Several lines of evidence suggest that aspirin might also reduce risk of other cancers, particularly of the gastrointestinal tract, but proof in man is lacking. We studied deaths due to cancer during and after randomised trials of daily aspirin versus control done originally for prevention of vascular events. METHODS: We used individual patient data from all randomised trials of daily aspirin versus no aspirin with mean duration of scheduled trial treatment of 4 years or longer to dete

Cutaneous sporotrichosis: unusual clinical presentations.

Three unusual clinical forms of sporotrichosis described in this paper will be a primer for the clinicians for an early diagnosis and treatment, especially in its unusual presentations. Case 1, a 52-year-old man, developed sporotrichosis over pre-existing facial nodulo-ulcerative basal cell carcinoma of seven-year duration, due to its contamination perhaps from topical herbal pastes and lymphocutaneous sporotrichosis over right hand/forearm from facial lesion/herbal paste. Case 2, a 25-year-old woman, presented with disseminated systemic-cutaneous, osteoarticular and possibly pleural (effusion

The use of topical subgingival gels of non-steroidal anti-inflammatory drugs (NSAIDs) as an adjunct to non-surgical management of chronic periodontitis.

The treatment of chronic periodontitis consists of conventional mechanical debridement and removal of plaque and calculus'. Topical NSAIDs could be used to complement the therapy as an adjunct to resolve the inflammatory process and clinical signs of the disease more rapidly. A randomized clinical trial was performed on 33 systemically healthy patients diagnosed with chronic periodontitis, aged 21 to 40 years. All patients underwent scaling and root planing on one quadrant per week, and sub-gingival gel was applied 48 hours after each session. Patients were grouped into 4 treatment protocols w

Crusted (Norwegian) scabies following systemic and topical corticosteroid therapy.

It is a case study of a 62-yr-old female with crusted (Norwegian) scabies, which appeared during her treatment with systemic and topical corticosteroid therapy, under the diagnosis of erythroderma. In the same time, the patient had been suffered from hypothyoidism, and her skin changes were misdiagnosed, because it was thought that they are associated with her endocrine disorder. Suddenly, beside the erythema, her skin became hyperkeratotic, with widespread scaling over the trunk and limbs, and crusted lesions appeared on her scalp and ears. The microscopic examination of the skin scales with

Pyogenic granuloma in ten children treated with topical imiquimod.

Traditional therapy for pyogenic granuloma is procedural. In young children it can require a general anesthetic and may be complicated by scarring, dyspigmentation, and recurrence. We report a series of 10 children, highlighting the safety and efficacy of topical 5% Imiquimod as an alternative noninvasive treatment of pyogenic granuloma. Ten children with a mean age of 2.5 years and 10.8 week duration of facial pyogenic granuloma lesion were recruited. Treatment regime with topical Imiquimod 5% cream varied in frequency of application and duration according to clinical response. Clinical outco
