High profile transvalvular microaxial flow pump as a bridge to heart transplantation for amyloid cardiomyopathy: A case series.

BACKGROUND: The definitive treatment for end-stage heart failure (ESHF) due to amyloid cardiomyopathy (ACM) is an orthotopic heart transplant (OHT). However, associated pulmonary hypertension (PH) can present as a contraindication to OHT and be challenging to manage with conventional therapies. We herein reported the successful use of Impella 5.5 in a series of patients with ACM to improve PH and successfully bridge to OHT. METHODS: Five patients with ACM associated ESHF were analyzed. All patients had moderate to severe PH on admission. As a bridge to transplant, Impella 5.5 was inserted thr…

Prevalence and incidence of amyloid transthyretin amyloidosis in the USA: insights from claims databases and electronic health records.

BACKGROUND: Amyloid transthyretin (ATTR) amyloidosis is a rare, life-threatening disease frequently manifesting with cardiomyopathy (ATTR-CM), polyneuropathy (ATTR-PN) or both (ATTR-mixed). We retrospectively analysed US electronic health records and claims data to provide up-to-date estimates of ATTR amyloidosis epidemiology (overall and by phenotype). METHODS: Data were extracted from the Clarivate Real-World Data repository (2016-2023). Given the lack of established coding for ATTR amyloidosis, we used different combinations of diagnostic codes to obtain narrow and broad estimates of incid…

Loss of PILRA promotes microglial immunometabolism to reduce amyloid pathology in cell and mouse models of Alzheimer's disease.

The Alzheimer's disease (AD) genetic landscape identified microglia as a key disease-modifying cell type. Paired immunoglobulin-like type 2 … ependent microglial functions. AD mice transplanted with human PILRA KO microglia exhibited reduced amyloid pathology and rescued synaptic markers. A high-affinity ligand blocking PILRA antibody phenocopied PILRA KO iMG. These findings suggest that PILRA is a pharmacologically tractable therapeutic target for AD.

Evaluation of serum amyloid-A protein in the diagnosis of sepsis among children at PICU of Al Zahraa University Hospital.

The early and efficient diagnosis of sepsis in critically ill children remains a difficult task as the clinical signs are nonspecific. Complete blood count parameters and C‑reactive protein have low sensitivity., Also, the difficulty of its diagnosis may be due to decreased positive values of blood culture and the need for longtime to detect blood culture results. The serum Amyloid A (SAA) protein level in the blood increases earlier and up to 1000‑fold in response to inflammation. This study aimed to assess the role of SAA as diagnostic and prognostic marker in pediatric sepsis in the first 2…

A ratio electrochemiluminescence sensor for monitoring amyloid-beta based on g-C3N4-heme with luminol as the internal standard.

Monitoring of the amyloid-beta (Aβ) peptide is crucial for the diagnosis and treatment of Alzheimer's disease. Here, a label-free, signal-on ratiometric electrochemiluminescence (ECL) aptasensor was fabricated for the detection of Aβ peptide. Specifically, graphite-like carbon nitride (g-C3N4) nanosheets and luminol served as the cathodic and anodic ECL emitters, respectively. K2S2O8 and H2O2 were used as the coreactants for the two emitters. Luminol was first coated on the indium tin oxide (ITO) electrode and used as the internal standard in the ratiometric biosensor. Polyaniline was then ele…

The Functional Epididymal Amyloid Cystatin-Related Epididymal Spermatogenic (CRES) is a Component of the Mammalian Brain Extracellular Matrix.

CRES is the defining member of a reproductive subgroup of family 2 cystatins of cysteine protease inhibitors. We previously showed that CRES and other subgroup members are part of a highly plastic amyloid-containing extracellular matrix (ECM) with host defense functions in the mouse epididymal lumen. Based on parallels between the epididymis and the brain, we hypothesized that CRES and CRES amyloids might also function within the brain including the ECM. Here we show that CRES is produced by hippocampal neurons and astrocytes in the male and female mouse and human brain. Further, approximately…

Early Amyloid Diagnosis and Treatment: Experience of Single Hand Surgeon With a Series of 182 Carpal Tunnel Biopsies.

BACKGROUND: Amyloidosis is a systemic disease characterized by buildup of abnormal amyloid deposits that may lead to multiple organ failure and is associated with early onset musculoskeletal manifestations. Orthopedic surgeons are positioned to aid with early diagnosis in the form of a biopsy which allows for workup for other areas of disease, specifically cardiac manifestations, and intervention prior to irreversible cardiac damage. In this review, we describe the biopsy results of a series of 182 patients and workup and treatment of the 46 amyloid positive patients biopsied by a single surge…

Nanoamplifier Agents Transiently Rise the Metabolism of β‑Amyloid Peptide in Urine for the Early Diagnosis of Alzheimer's Disease.

Alzheimer's disease (AD) is the most common form of dementia without effective treatment. Therefore, early diagnosis for timely treatment and delayof the onset of AD are critical. At present, detecting β-amyloid (Aβ) in cerebrospinal fluid is still the most important clinical method. However, the invasive detection method is harmful and difficult to promote. Recent research has shown that Aβ was found not only in blood and cerebrospinal fluid, but also in urine, which could be used for noninvasive testing. Compared with blood/cerebrospinal fluid, the background proteins in urine are very low, …

Screening for transthyretin amyloid cardiomyopathy in patients with musculoskeletal symptoms: Red flags in the rheumatology/orthopedics practice setting.

Musculoskeletal manifestations of transthyretin amyloidosis (ATTR) are common, early in the disease course (usually years before cardiac involvement), and are potentially predictive. They include carpal tunnel syndrome (CTS), trigger finger, atraumatic tears of the brachial biceps tendon or rotator cuff, spinal stenosis, and large joint osteoarthritis. These extra-cardiac 'red flags' for ATTR amyloidosis may present individually or in clusters, particularly in older males, several years in advance of signs of ATTR cardiomyopathy (ATTR-CM), such as arrhythmia or heart failure. Deposition of ATT…

A reproducible approach for the use of aptamer libraries for the identification of Aptamarkers for brain amyloid deposition based on plasma analysis.

An approach for the agnostic identification and validation of aptamers for the prediction of a medical state from plasma analysis is presented in application to a key risk factor for Alzheimer's disease. brain amyloid deposition. This method involved the use of a newly designed aptamer library with sixteen random nucleotides interspersed with fixed sequences called a Neomer library. The Neomer library approach enables the direct application of the same starting library on multiple plasma samples, without the requirement for pre-enrichment associated with the traditional approach. Eight aptamer…

Dementia diagnostic and treatment services in the Western Pacific: challenges, preparedness and opportunities in the face of amyloid-targeting therapies.

Here we first review the limited available literature addressing the current landscape of specialist assessment services for dementia and cognitive decline and the preparedness for new amyloid-targeting therapies for Alzheimer's disease across the Western Pacific region. Considering the scarcity of literature, as national representatives of Western Pacific nations we were then guided by the World Health Organization's Global Action Plan on Dementia to provide country-specific reviews. As a whole, we highlight that the existing diverse socioeconomic and cultural landscape across the region pose…

The Dual Role of Amyloid Beta-Peptide in Oxidative Stress and Inflammation: Unveiling Their Connections in Alzheimer's Disease Etiopathology.

Alzheimer's disease (AD) is a progressive neurodegenerative disease, and it is currently the seventh leading cause of death worldwide. It is characterized by the extracellular aggregation of the amyloid β-peptide (Aβ) into oligomers and fibrils that cause synaptotoxicity and neuronal death. Aβ exhibits a dual role in promoting oxidative stress and inflammation. This review aims to unravel the intricate connection between these processes and their contribution to AD progression. The review delves into oxidative stress in AD, focusing on the involvement of metals, mitochondrial dysfunction, and …

Decoding the molecular and structural determinants of the neurokinin A and Aβ1-42 peptide cross-interaction in the amyloid cascade pathway.

Tachykinins are short neuropeptides, such as substance P and neurokinin B, that have been shown to interact with Alzheimer's β-amyloid (Aβ) peptide. Neurokinin A (NKA) is a secreted tachykinin neuropeptide that binds to neurokinin receptors and with an emerging role in the brain-gut axis. NKA shares the brain niche with Aβ; thus, we investigate whether and how NKA and Aβ peptide interact. We have used a combination of computational and experimental biophysics to assess the interaction of both peptides in vitro. Using Phe-to-Trp substitution, we have shown that Phe in the FXGLM signature in NKA…

Metformin effectively alleviates the symptoms of Alzheimer in rats by lowering amyloid β deposition and enhancing the insulin signal.

Alzheimer's disease (AD) exhibits distinct biochemical and histopathological attributes, encompassing cellular, neuronal, and oxidative impairment. There is also an abnormal buildup, misfolding and clumping of amyloid β (Aβ). The present study aimed to explore the influence of the antihyperglycemic agent metformin on rats with AD-like symptoms, while also elucidating the intricate relationship between insulin resistance and AD. The rats were categorized into five groups: a control group, a saline-administered group, a metformin-treated group, AD-model rats, and AD-rats treated with a 200 mg/kg…

Opposite Roles of Cholesterol and Lanosterol in Lipid Membrane on Amyloid-Beta 42 Peptide Nucleation and Fibril Formation.

Molecular self-assembly of amyloid-beta peptides to form fibrillar aggregates is a known cause of Alzheimer's disease. Although homogeneous nucleation of amyloid-beta is unfavorable, heterogeneous nucleation of amyloid-beta in cell membranes plays a key role in fibril formation. We observed these opposite roles in the effects of cholesterol and lanosterol, the precursor of cholesterol in the brain, on nucleation. As previously reported, cholesterol accelerated nucleation, whereas lanosterol decelerated it when mixed with dioleoyl-phosphatidylcholine at 20%. The observed opposite effects of cho…

Transmembrane protein 106B amyloid is a potential off-target molecule of tau PET tracers in the choroid plexus.

PURPOSE: Tau positron emission tomography (PET) has become an essential tool for the clinical diagnosis of neurodegenerative diseases and the study of tau pathology in the brain. However, some tau tracers exhibit off-target binding in the basal ganglia, choroid plexus, and meninges. Recently, transmembrane protein 106B (TMEM106B) was identified to form novel amyloid filaments in the brain during aging. In this study, we explored the possibility that TMEM106B aggregates might be responsible for off-target binding of tau PET tracers in the choroid plexus. METHODS: The binding properties of 18F-…
