An alternative treatment for osteoporotic Su Type III periprosthetic supracondylar femur fractures: Double locking plate fixation.

INTRODUCTION: Patients with Su Type III fractures based on total knee arthroplasty (TKA) constitute a patient group with problematic treatment and management. Although it has difficulties, open reduction and internal fixation is one of the treatment options. METHOD: A retrospective evaluation was made of 22 patients surgically treated in our clinic with double locking, low contact titanium plate and screw for a Su Type III periprosthetic fracture based on TKA. The patients were evaluated with bone mineral densitometry, postoperative Knee Society Score (KSS), WOMAC and radiological evaluations

Revision Total Wrist Arthroplasty Due to Polyethylene Wear, Metallosis-Induced Carpal Tunnel Syndrome, Distal Ulnar Impingement, and Fourth Carpometacarpal Joint Pain: Case Report and Pitfalls to Avoid.

Background: Total wrist arthroplasty (TWA) is indicated to alleviate pain secondary to arthritis while preserving wrist motion. Despite vigilant measures, TWAs are susceptible to complications. The current case illustrates an array of preventable complications which are addressed through operative technique in a revision setting. Case presentation: The patient was a 72-year-old man who presented with chronic pain at the fourth carpometacarpal joint (CMCJ) and distal radioulnar joint with paresthesias in the median nerve distribution. Surgery was undertaken to address the following complication

"Within ring"-based sacroiliac rod fixation may overcome the weakness of spinopelvic fixation for unstable pelvic ring injuries: technical notes and clinical outcomes.

PURPOSE: Spinopelvic fixation and triangular osteosynthesis give firm internal fixation for unstable pelvic ring injuries (UPRI), but with sacrifice of mobility of the lumbar spine. Here, we describe the procedure and outcomes of a new approach, which we refer to as "within ring"-based sacroiliac rod fixation (SIRF). METHODS: The patient was placed in a prone position and longitudinal skin incisions were made at the medial margins of the bilateral posterior superior iliac spines (PSIS). After reduction of fracture, a pedicle screw was inserted into the first sacral vertebra on the injured sid

Insufficiency Fractures at Unusual Sites: A Case Series.

INTRODUCTION: Insufficiency fractures (IFs) are a subtype of stress fractures commonly associated with osteoporosis and Vitamin D deficiency. These nontraumatic fractures often occur in the pelvis and spine, but the involvement of other unusual sites is also not very uncommon. Plain radiographs and magnetic resonance imaging scan are the most commonly used imaging modalities for the diagnosis of IF. CASE REPORT: We are presenting a series of five cases of IFs at unusual sites, along with a detailed literature review on this individual. A 50-year-old male patient with chronic kidney disease pr

Treatment of Proximal Humeral Fractures: A Critical Analysis Review.

There is substantial variability in the treatment of proximal humeral fractures because of classification systems with poor interobserver reliability, rapid advances in technology (e.g., anatomically designed locking plates and reverse shoulder arthroplasty), and limited Level-I and II evidence for best treatments based on fracture patterns and physiological age. Almost three-quarters of proximal humeral fractures can be treated nonoperatively with good functional results as nearly 50% are nondisplaced or Neer one-part fractures. Another quarter occur in infirm patients or are characterized by

Prosthetic rehabilitation of an edentulous maxilla with microstomia, limited interarch space, and malaligned implants: a clinical report.

BACKGROUND: Although dental implant treatment is a very successful option today, a meticulous treatment planning and close collaboration between the oral surgeon and restorative doctor is crucial to eliminate undesired outcomes. PURPOSE: To present a challenging case restored with a maxillary screw-retained fixed prosthesis using malpositioned/malaligned implants. MATERIALS AND METHODS: A 47-year-old female had a Lefort I fracture and lost all maxillary teeth due to traumatic injury in a traffic accident. Seven maxillary implants were placed using a 1-stage surgical approach, 6 months after

Redefining orthodontic space closure: sequential repetitive loading of the periodontal ligament--a clinical study.

AIMS: To assess the rate of tooth movement, anchorage loss, root resorption, and alkaline phosphatase (ALP) activity in the gingival crevicular fluid (GCF) as a marker for bone remodeling during orthodontic space closure using two different mechanisms. METHODS: Space closure was completed in 20 patients with extraction of all 4 premolars. Lateral cephalograms and radio-visiographs taken before (T1) and after (T2) space closure were assessed for anchorage loss and root resorption. Alkaline phosphatase levels were measured in 10 patients, which were divided into two groups of five each. Spaces

Treatment of metastatic bone lesions in the upper extremity: indications for surgery.

Pathological fractures caused by metastatic malignant disease have been the subject of increasing interest in recent years. This article describes our experience with the treatment of metastatic bone disease of the upper extremity and our attempt to clarify the indications for different surgical procedures. Of 53 patients with metastatic lesions to the upper extremity, 20 who had been surgically treated were analyzed retrospectively. These comprised 13 men and 7 women with a mean patient age of 62 years. The most common primary tumors to metastasize were lung and liver, with the humerus involv

Management of craniocervical junction dislocation.

The discovery of a craniocervical junction malformation requires management in three steps: (1) The patterns must be recognized using tomographic measurements (Chamberlain's line, Wackenheim's line). Dynamic flexion-extension studies are necessary to assess stability or instability. Stable patterns range from platybasia to basilar invagination, with gradual deformation, and are frequently associated with Chiari malformation. Unstable patterns characterized by odontoid instability are the equivalent of an odontoid fracture. The origin is malformative (hypoplasia, aplasia of the dens, os odontoi

The application of the limited contact dynamic compression plate in the upper extremity: an analysis of 114 consecutive cases.

We sought to assess the clinical effectiveness of a new plate design which offers improved biological and biomechanical features, the limited contact dynamic compression (LCDC) plate. We analysed 114 LCDC plates applied consecutively for upper extremity fractures or reconstruction in 94 patients. Three patients were lost to follow-up, leaving 111 plates in 91 patients followed to definitive fracture/osteotomy outcome. Thirty-seven plates were applied for reconstruction in 35 patients, including 11 where standard implants had failed. Fifty-six patients had 74 plates applied for acute fractures

[Treatment of unstable fractures of thoracolumbar spine with neurologic injury using a reduction fixation spinal pedicle screws system].

Twenty-six patients with unstable burst fractures, chance fractures and fractures dislocations of the lower thoracic and lumbar spine were treated with a spinal pedical screw reduction fixation system (RF system). This system is a new device designed by Chinese scientists. In biomechanical testing, it provided three-dimensional reduction forces. The special design of angle pedicle screw provided accurate angle to restore the normal thoracic lumbar lordosis and to maintain it. The three-column spine in a lordotic position maximized the reduction and indirectly achieved a neurologic decompressio

[kinematic consideration of the shoulder girdle and its consequences on common surgical methods].

Motility of the upper extremities is the sum of movements of an open kinematic chain made up of the hands and the lower and upper arms in the humero-scapular joints which are integrated in the closed kinematic chains of the bilateral symmetrical shoulder girdles. Each shoulder girdle assembles three kinematic links (scapula, clavicle, hemithorax), with three joints (sternoclavicular, acromioclavicular, thoraco-scapular), creating a three-bar linkage. This linkage may be characterized as an effective transformer of motion with 11 degrees of freedom ensuring wide ranges of motions. This motion-t

Treatment of unicameral bone cyst: a comparative study of selected techniques.

BACKGROUND: There is a variety of treatment modalities for unicameral bone cysts, with variable outcomes reported in the literature. Although good initial outcomes have been reported, the success rate has often changed with longer-term follow-up. We introduce a novel, minimally invasive treatment method and compare its clinical outcomes with those of other methods of treatment of this lesion. METHODS: From February 1994 to April 2008, forty patients with a unicameral bone cyst were treated with one of four techniques: serial percutaneous steroid and autogenous bone-marrow injection (Group 1,

Comparison of intramedullary nailing versus plate-screw fixation of extra-articular metacarpal fractures.

PURPOSE: To compare the clinical and radiographic outcomes of intramedullary nail (IMN) fixation of metacarpal fractures with those of plate-screw (PS) fixation. METHODS: Between 2004 and 2006, we treated 52 consecutive closed, displaced, extra-articular metacarpal fractures operatively using 1 of 2 fixation methods: IMN or PS. Patient characteristics, mechanism of injury, pattern of fracture, and preoperative radiographic parameters were similar in both groups. The outcome measures were collected and analyzed:total active motion of the digit and Disabilities of the Arm, Shoulder, and Hand sc

Open reduction and internal fixation of acute displaced scaphoid waist fractures.

Fourteen consecutive patients with acute displaced scaphoid waist fractures were treated with open reduction and internal fixation. The operative technique consisted of anatomic reduction of the displaced scaphoid waist fracture, correction of carpal instability, radial bone grafting for comminution, and internal fixation with K-wires or Herbert screw. The patients were evaluated an average of 26 months (range, 4-48 months) after surgery. Thirteen of the 14 (93%) fractures united. The average time to union was 11.5 weeks (range, 8-20 weeks). Fracture union was confirmed with trispiral tomograp

Management of frontal sinus fractures.

Frontal sinus fractures have been reported in 2 to 12 percent of all cranial fractures and, according to one study, in 8 percent of all facial fractures. Although immediate frontal bone reconstruction is not universally accepted, it is rapidly becoming the standard of care in treating cranial facial fractures, even in the face of gross contamination. This article presents a historical overview of fracture management, a review of the relevant anatomy, and a discussion of appropriate management principles. Three case studies are presented to demonstrate appropriate techniques for the management

Internal fixation of fractures in children and adolescents. A comparative analysis.

A retrospective analysis of 4,411 consecutive pediatric fracture patients managed between 1979 and 1983 demonstrated that only 3.6% (170 patients) required internal fixation. Two patient groups with sufficient follow-up study were compared: Group 1-90 skeletally immature children and young adolescents, and Group 2-66 skeletally mature adolescents. Upper-extremity fractures, especially of the distal humerus, and displaced epiphyseal fractures were the major indication for internal fixation in Group 1, while lower-extremity diaphyseal and intra-articular fractures predominated in Group 2.
