Subjective visual vertical during eccentric rotation in patients with vestibular neuritis.

The subjective visual vertical (SVV) is a useful tool to evaluate clinical manifestations of vestibular loss, but there have been normal variations of the SVV within 1 degrees -3 degrees , and over time, the absolute deviated degrees of SVV tilts decreases. We investigated SVV values in patients with vestibular neuritis (VN) during eccentric rotation, the method used to assess utricular function during stimulation of one labyrinth. And we performed SVV in the resting state and during eccentric rotation to the lesion side and the healthy side of 15 patients with VN and 20 normal subjects. No di

Stem cells and molecular strategies to restore hearing.

Hearing loss is a costly and growing problem for the elderly population worldwide with millions of people being affected. There are currently two prosthetic devices available to minimize problems associated with the two forms of hearing loss: hearing aids that amplify sound to overcome middle ear based conductive hearing loss and cochlear implants that restore some hearing after neurosensory hearing loss. The current presentation provides information on the treatment of neurosensory hearing loss. Although the cochlear implant solution for neurosensory hearing loss is technologically advanced;

Association between variations in CAT and noise-induced hearing loss in two independent noise-exposed populations.

Noise-induced hearing loss (NIHL) is an important occupational hazard that results from an interaction between genetic and environmental factors. Although the environmental risk factors have been studied quite extensively, little is known about the genetic factors. On the basis of multiple studies, it was proposed that oxidative stress plays an important role in the development of NIHL. Here, we investigated whether variations (single nucleotide polymorphisms; SNPs) in the catalase gene (CAT), one of the genes involved in oxidative stress, influence noise susceptibility. Audiometric data from

Studies of otic capsule morphology and gene expression in the Mov13 mouse--an animal model of type I osteogenesis imperfecta.

Type I osteogenesis imperfecta (OI) is a disorder of skeletal bones characterized by bone fragility and blue sclera, which can result from mutations in genes encoding for type I collagen--the COL1A1 and COL1A2 genes. Fifty percent of patients with type I OI develop hearing loss and associated histopathological changes in the otic capsule that are indistinguishable from otosclerosis, a major cause of acquired hearing loss. In an attempt to elucidate molecular and cellular mechanisms of hearing loss in type I OI, we have studied the Mov13 mouse, which has served as an animal model of type I OI b

Conservative (labyrinth-preserving) transpetrosal approach to the clivus and petroclival region--indications, complications, results and lessons learned.

OBJECTIVE: Tumours or vascular lesions of the clivus and juxtaclival region present a unique challenge to neurosurgeons and a variety of techniques, with a wide spectrum of complexity, have been advocated. This report presents the use of a conservative transpetrosal approach which combines partial removal of the postero-lateral petrous bone with preservation of the labyrinth, with particular focus on efficacy and the reduction of complications. PATIENTS AND METHODS: Over 9 years, 52 patients underwent a conservative combined supra-infratentorial, labyrinth-preserving transpetrosal approach. T

Translabyrinthine approach for the management of large and giant vestibular schwannomas.

BACKGROUND: The removal of large vestibular schwannomas through the translabyrinthine approach is still controversial. MATERIALS AND METHODS: A retrospective review was performed of 81 patients (58 men and 23 women) with vestibular schwannomas 3 cm or greater, who underwent tumor removal via the translabyrinthine approach between 1985 and 2000. The mean tumor size was 3.7 +/- 0.81 cm, and the mean age of the patients was 47 +/- 16.1 years. The largest tumor was 6 cm. All surgical procedures were performed in collaboration with a neurosurgery team. RESULTS: Total tumor removal was accomplishe

[Trans-petrous surgery in acoustic neuroma. Value of preoperative audiovestibular and facial investigation in the risk evaluation of facial nerve function].

Between 1987 and february 1994, 162 consecutive patients with acoustic neuroma were operated on by an otoneurosurgery team, using transpetrous approaches (89% translabyrinthine, 8% middle fossa and 3% retrosigmoid). The relationship between the clinical, audiometric and vestibulographic characteristics and the post-operative facial nerve function were evaluated. In acoustic neuromas with cerebello-pontine component inferior to 3 cm without central neurologic signs (ic: central controlateral auditory and/or ipsilateral vestibular pathway alteration), good post-operative facial nerve function wa

[Follow-up of caloric test response after acute peripheral vestibular dysfunction].

This study examined retrospectively the spontaneous recovery of patients with an acute peripheral vestibular deficit in order to determine whether the caloric test response and with it vestibular function improves over time. The caloric bithermal was tested three times on 79 patients who were hospitalised with an acute deficit. The first test was recorded on emergency admission by observing nystagmus beats under the Frenzel glasses. Two to five days later a complete electronystagmus (ENG) examination was performed. A second ENG was performed, on average, 4 months later. 46% of the patients rec

Value of Gd-DTPA-enhanced MR imaging of the labyrinth in patients with sudden hearing loss.

Recent reports describe labyrinthine enhancement on MRI as a highly specific sign of labyrinthine disease. This paper reports 44 patients with unilateral sensorineural hearing loss (SNHL) and laboratory evidence of cochlear damage investigated with Gd-enhanced MR imaging. Enhancement of the cochlea was observed in only one patient with a lesion at the fundus of the internal auditory canal (IAC) that extended into the cochlea after Gd-DTPA administration. In one more patient, MR imaging demonstrated large vestibular aqueducts as underlying cause for his hearing loss, but no enhancement of the l

Clinical features of idiopathic bilateral sensorineural hearing loss.

The Research Committee of the Ministry of Health and Welfare in Japan has defined hearing loss of unknown etiology with fairly high-speed progression as idiopathic bilateral sensorineural hearing loss (IBSH). Its diagnostic criteria consist of the following: that it is (1) progressive, (2) with bilateral involvement, and (3) of unknown etiology. In this study, we evaluated the clinical course in 20 cases of IBSH. Many cases of IBSH finally showed symmetrical profound sensorineural hearing loss. The clinical course of IBSH consisted of various combinations of the following four states: (1) fixe

Contrast enhancement of the labyrinth on MR scans in patients with sudden hearing loss and vertigo: evidence of labyrinthine disease.

The sudden onset of hearing loss and vertigo presents a difficult diagnostic problem. We describe the finding of labyrinthine enhancement on MR images in five patients with sudden unilateral hearing loss or vertigo or both and correlate the MR findings with audiologic and electronystagmographic studies. All patients were studied with T2-weighted axial images through the whole brain, contrast-enhanced 3-mm axial T1-weighted images through the temporal bone, and enhanced T1-weighted sagittal images through the whole brain. Cochlear enhancement, on the side of hearing loss only, was found in all

[Removal of acoustic neuromas of the cerebellopontile angle with transtemporal approach by the middle cranial fossa].

The enlarged transtemporal middle fossa approach to the cerebello-pontine angle is adequate for complete removal of acoustic neuromas up to 4.0 cm in diameter (sizes B and C according to Fisch). A description of the surgical technique and the results of 63 operations are presented (13, 23 and 27 tumours of sizes A, B and C respectively). 51 tumours were completely removed, 6 had a voluntary palliative incomplete resection for medical reasons or on the only remaining hearing ear. Only 6 patients (one in the last 40 consecutive cases) had an incomplete resection due to the limited exposure of th

Normal and pathologic radiographic anatomy of the motor innervation of the face.

Facial motor disorders, including facial paralysis, myokymia, dyskinesia, and hemifacial spasm, are common clinical problems in which radiographic evaluation plays a crucial role. Since every segment of the motor innervation of the face from the brain to the parotid gland can now be seen radiographically, radiologists must understand the normal anatomy, the common pathologic lesions at each level, and the clinical findings that help localize the abnormality so that the most sensitive and accurate radiographic approach can be planned. Though computed tomography alone allows for visualization of

Audiometric results following endolymphatic sac surgery.

Decompression and drainage of the endolymphatic sac is a surgical procedure which was developed to relieve vertigo and hopefully to maintain or improve hearing in patients with Meniere's disease. In this study, 16 medically diagnosed Meniere's disease patients who underwent endolymphatic sac surgery were followed audiometrically for a period of up to three years. Pre- and postoperative audiometric and other symptomatic findings were reported. Results indicated that in the greatest majority of cases hearing remained essentially unchanged or was improved. Poorer hearing was found in 26.3% of the
