Density of spinal anaesthetic solutions of bupivacaine, levobupivacaine, and ropivacaine with and without dextrose.

BACKGROUND: Spread of intrathecal local anaesthetics is determined principally by baricity and position of the patient. Hypobaric solutions of bupivacaine are characterized by an unpredictable spread of sensory block whereas addition of dextrose 80 g ml(-1) provides a predictable spread but to high thoracic levels. In contrast, dextrose concentrations between 8 and 30 g ml(-1) have shown reliable and consistent spread for surgery. Hence, the aim of this study was to determine the density of bupivacaine, levobupivacaine, and ropivacaine with and without dextrose at both 23 and 37 degrees C befo

[Spinal anesthesia in a patient with hemiparesis after poliomyelitis].

A seventy-year-old man who underwent transurethral resection of the prostate (TUR-P), had carried over the palsy after poliomyelitis inflicted at one year of age. Spinal anesthesia using with 7 mg of hyperbaric tetracaine with dextrose solution was performed for this surgery. The running pressure of D-sorbitol solution was 50 cmH2O during the operation. The operation time was 41 minutes and the balance of running water was -200 ml. After this TUR-P, there was no progression of his palsy, muscle atrophy or no aggravation of autonomic system immediately after the operation, 3 months later and 6

Enantioseparations in nonaqueous capillary liquid chromatography and capillary electrochromatography using cellulose tris(3,5-dimethylphenylcarbamate) as chiral stationary phase.

The potential of the widely used chiral stationary phase for high-performance liquid chromatography (HPLC) enantioseparations, cellulose tris(3,5-dimethylphenylcarbamate) (CDMPC, sold under the trade name Chiralcel OD) was evaluated under the conditions of nonaqueous capillary electrochromatography (CEC). The effect of the particle size of the silica gel, the loading of CDMPC on the silica gel and nature of the organic solvent, as well as electrolyte salts on the separation characteristics were investigated. This study illustrates the applicability of CDMPC for obtaining highly efficient enant

Systemic diseases associated with disorders of water homeostasis.

Disorders of AVP secretion and action sometimes present as the first manifestation of a variety of different systemic diseases. It is prudent for the clinician to consider these causes in the differential diagnosis of hypoosmolar hyponatremia, polyuria and polydipsia, since recognizing the underlying disorder may affect treatment decisions, and intervention directed at the primary disorder often can reverse the abnormal water metabolism in these patients. Although much of the pathophysiology of these disorders is not understood completely, great progress has been made toward appreciating the c

Improving cycling performance: how should we spend our time and money.

Cycling performance is dependent on physiological factors which influence mechanical power production and mechanical and environmental factors that affect power demand. The purpose of this review was to summarize these factors and to rank them in order of importance. We used a model by Martin et al. to express all performance changes as changes in 40 km time trial performance. We modelled the performance of riders with different ability ranging from novice to elite cyclists. Training is a first and most obvious way to improve power production and was predicted to have the potential to improve

Microcapsulated parathyroid tissue in vitro.

Ideally, microcapsules will make it possible to transplant parathyroid tissue for allo- and xenotransplantation in hypoparathyroid patients. In this study, parathyroid tissues capsulated by polyelectrolyte complex were evaluated in vitro. Hormone secretion ability was significantly (P < 0.001) greater in sediment than in supernatant. There was no difference in hormone secretion ability between the parathyroid tissue thawed at room temperature and the tissue thawed in a bath at 37 degrees C. Both non-capsulated and capsulated parathyroid tissues were incubated for three weeks. The ability of ca

Diuretic-induced hyponatremia.

Diuretics are one of the most common causes of severe hyponatremia. Yet, despite several relevant studies and years of clinical experience, the mechanism and optimal treatment of diuretic-induced hyponatremia remain unclear. What is clear is that most cases are caused by thiazide rather than loop diuretics and that severe hyponatremia can develop very rapidly in susceptible patients. In this review, I will discuss the pathogenesis, clinical features, prevention, and treatment of diuretic-induced hyponatremia in the hope that increased awareness and understanding will reduce the incidence and c

Fluid therapy in the cardiac patient.

Correcting fluid deficits and preventing fluid overload in patients with cardiac disease can be challenging. The purpose of the fluid therapy, the projected duration of the therapy, and the type of heart disease affecting the patient play important roles in the decision-making process. In addition, the distinction between patients who are not symptomatic (NYHA Class I) and those who are symptomatic (NYHA Classes II to IV) for their cardiac disease should be made early in fluid therapy planning.

Volume repletion after exercise-induced volume depletion in humans: replacement of water and sodium losses.

Sodium and water loss during, and replacement after, exercise-induced volume depletion was investigated in six volunteers volume depleted by 1.89 +/- 0.17% (SD) of body mass by intermittent exercise in a warm, humid environment. Subjects exercised in a large, open plastic bag, allowing collection of all sweat secreted during exercise. For over 60 min beginning 40 min after the end of exercise, subjects ingested drinks containing 0, 25, 50, or 100 mmol/l sodium (trials 0, 25, 50, and 100) in a volume (ml) equivalent to 150% of the mass lost (g) by volume depletion. Body mass loss and sweat elec

Thermoregulatory adaptations associated with training and heat acclimation.

The large metabolic heat load generated as a consequence of muscular work requires activation of thermoregulatory mechanisms in order to prevent an excessive and potentially dangerous rise in body temperature during exercise. Although the horse has highly efficient heat dissipatory mechanisms, there are a number of circumstances in which the thermoregulatory system may be overwhelmed, resulting in the development of critical hyperthermia. The risk for development of life-threatening hyperthermia is greatest when (1) the horse is inadequately conditioned for the required level of physical perfo

Microdisperse form of immobilized hemoglobin modelling the erythrocytes.

Disperse systems based on hemoglobin sorption immobilization in reticular carboxylic polyelectrolytes are proposed as model of erythrocytes. The efficiency of oxygen transport of these systems is much higher than that of native hemoglobin and is comparable with the gas transport of erythrocytes. This is believed to be due to highly selective sorption of hemoglobin in microdisperse forms of permeable polyelectrolytes. Microparticles of immobilized hemoglobin exhibit high local concentration of hemoglobin: the protein mass being one order of magnitude than that of polymer-carrier. Besides, it is…

Hyponatremia caused by a reset osmostat in a neonate with cleft lip and palate and panhypopituitarism.

A neonate with cleft lip and palate and hypopituitarism had persistent hyponatremia despite treatment with hydrocortisone, L-thyroxine, and growth hormone. Serum sodium concentration and urinary osmolality increased and decreased appropriately and concurrently with alterations in sodium and water intake. The ability to regulate serum concentrations of antidiuretic hormone at subnormal serum sodium concentrations indicated a reset osmostat.

Dose-response relationships and mineralocorticoid activity in cortisol-induced hypertension in humans.

OBJECTIVE: This study was designed to define the dose-response relationships for cortisol-induced hypertension in humans and to test the hypothesis that cortisol-induced hypertension is a consequence of classical mineralocorticoid actions using the mineralocorticoid antagonist spironolactone. METHODS: In study 1, six normal men were given cortisol orally every 6 h for 5 days at doses of 40, 80 and 200 mg per day. In study 2, six normal men were given spironolactone at 400 mg/day for 6 days and cortisol at 80 mg/day for 5 days, commencing on the second day of spironolactone treatment. RESULTS

["Small volume resuscitation"--does it open new possibilities in the treatment of hypovolemic shock?].

Hypertonic-hyperoncotic solutions are a supplementation to conventional fluid regimens in the management of hypovolemia due to trauma, hemorrhage and shock. In this review the possible modes of action of these solutions are discussed and their efficacy both in experimental and clinical settings is presented. Possible side effects, such as hypernatremia and possible problems in the presence of increased intracranial pressure, following administration of hypertonic-hyperoncotic solutions are discussed, as well as the reaction of normovolemic patients to such infusions.

Successful use of continuous veno-venous haemofiltration to treat profound fluid retention in severe peripartum cardiomyopathy.

The use of continuous veno-venous haemofiltration after failure of conventional treatment in a patient with severe peripartum cardiomyopathy is described. Treatment with inotropes and diuretics failed to produce a diuresis despite the presence of severe fluid overload. Haemofiltration over a 9-day period allowed removal of 171 of fluid with a concomitant improvement in haemodynamic function and a spontaneous diuresis.

Reversible vasopressin deficiency in severe hypernatremia.

Studies to assess the relationship between plasma arginine vasopressin concentration (Pavp) and plasma osmolality (Posm) were performed on an elderly patient with dementia who developed severe hypernatremia due to inadequate water intake following a debilitating hip fracture. Serum sodium concentrations were 174 and 196 mEq/L on two consecutive hospital admissions. During the second of these admissions, sequential measurements of Pavp and Posm were obtained as hypernatremia was gradually corrected. Pavp during this period was correlated with Posm (r = 0.887, P < 0.01), but was low despite the

Reversal of diabetes insipidus in Brattleboro rats: intrahypothalamic injection of vasopressin mRNA.

Messenger RNAs occur within the axons of magnocellular hypothalamic neurons known to secrete oxytocin and vasopressin. In Brattleboro rats, which have a genetic mutation that renders them incapable of vasopressin expression and secretion and thus causes diabetes insipidus, injection into the hypothalamus of purified mRNAs from normal rat hypothalami or of synthetic copies of the vasopressin mRNA leads to selective uptake, retrograde transport, and expression of vasopressin exclusively in the magnocellular neurons. Temporary reversal of their diabetes insipidus (for up to 5 days) can be observe
