INTRODUCTION: Cephalomedullary nails (CMNs) are commonly used to treat proximal femur fractures. The Trochanteric Fixation Nail Advanced (TFNA, DePuy Synthes) is a CMN system, introduced in 2015, featuring significant design modifications compared to its predecessors. These include a new Ti-15Mo alloy and a fenestrated lag screw that permits cement augmentation of the femoral head. Nevertheless, device-specific failures have emerged. We report a unique mode of failure: breakage of the lag screw through a cement fenestration.

Background/Objectives: Orbital fractures are a very serious problem due to the close location of the eyeball and a direct path to brain injuries, which is associated with serious consequences. This study aims to assess the usefulness of the Orbital Destruction Intensity (ODI) scale. Additionally, this article includes elements of an epidemiological study. Methods: A retrospective study of 160 patients admitted to the Department of Maxillofacial Surgery in Łódź (Poland) between January 2021 and June 2024 was conducted. In this study, general patient information (gender, age), details about the

People with chronic disability and uncontrollable long-term complications following hip fracture have characterist.ics that may predispose them to social death. Continuous physical disability can have negative physical, psychological, and social consequences in these patients. To design care interventions for preventing and controlling social death, it is essential to identify the dimensions and characteristics of this process. Therefore, the present study aimed to explain the process of social death in hip fracture patients. In this study, which was conducted using a grounded theory approach,

The transition of traditionally hospital-based orthopedic procedures to the ambulatory surgery center setting provides many benefits from a patient care and financial perspective. Specifically, closed ankle fractures can potentially be managed at such centers without needing hospitalization. Adding to the paucity of data, this study describes the safety, cost, and outcomes of patients undergoing ankle fracture repair in an ambulatory surgery center. A retrospective chart review of 100 patients who underwent ankle fracture open reduction and internal fixation from a single ambulatory surgery ce

INTRODUCTION: Even though the dislocation rate is lower in the hemiarthroplasty (HA) than total hip arthroplasty, it has still developed as one of serious complications in elderly patients. We have used short external rotators (SER) preserving posterolateral approach to reduce dislocation after hip arthroplasty, especially in elderly patients. The present study was conducted to introduce SER preserving posterolateral approach and report the dislocation rate after HA via this approach in elderly patients with femoral neck fractures.
METHODS: Between January 2015 and July 2019, 307 consecutive

Nonunion describes bone fractures that fail to heal, resulting in the fracture callus failing to fully ossify or, in atrophic cases, not forming altogether. Fracture healing is regulated, in part, by the balance of proinflammatory and anti-inflammatory processes occurring within the bone marrow and surface cell populations. We sought to further understand the role of osteoimmunology (i.e., study of the close relationship between the immune system and bone) by examining immune cell gene expression via single-cell RNA sequencing of intramedullary canal tissue obtained from human patients with fe

PURPOSE: To investigate, if patients with complex proximal tibial fracture have realistic expectations on open reduction and internal fixation.
METHODS: 114 patients (mean 49 years, SD ± 13) with closed AO-type B and C proximal tibial fracture were grouped (group B, respectively C). Prior to surgery expectations concerning knee function, pain, return to work/sports, and the risk for osteoarthritis was assessed with the Hospital for Special Surgery-Knee Surgery Expectations Survey (HFSS-KSE) and a non-validated ten-item survey.
RESULTS: 92% of patients expected at least an almost natural knee

BACKGROUND: The exact incidence of postoperative periprosthetic humeral fractures (PPHF) months or years after fracture-related implantation of a hemiprosthesis is unknown. The currently available literature is predominantly concerned with operative treatment approaches. As a rule, these involved older patients and severe fracture conditions so that severe complications and unsatisfactory healing results were described. This article presents an alternative conservative treatment approach which is discussed based on the results of treatment.
PATIENTS AND METHODS: Between 2011 and 2016 a conser

Rare cases of primary cerebral fat embolism as a postoperative complication have been reported. In addition, cerebral fat embolism occurring before operative repair without shunt lesion are more rarely reported. We report a patient with a posttraumatic cerebral fat embolism resulting in severe neurologic dysfunction without right to left shunt. A 25-year-old man was brought to the hospital immediately after a traffic accident. He sustained a right segmental femoral shaft fracture and a left tibial shaft fracture. The patient was alert with no neurological deficits. Approximately 13 hours after

The aim of the study was to document soft tissue vascularization of the lateral heel on cadavers and, therefore, to indirectly demonstrate the importance to perform perfectly precise incisions, in order to prevent ischemic complications. Sections of 8 human lower limb cadavers were performed in cooperation with the Anatomical Institute of the MU Medical Faculty in Brno, according to current common standards for anatomical preparations. In successive steps, cutaneous and subcutanous covers of the lateral ankle, malleolar and heel regions were preparated. Final branches of the individual arterie

BACKGROUND: Distal radius fractures (DRFs) are the most common upper-extremity fractures in older adults. Frailty may modulate postoperative risk, but the comparative prognostic value of widely used frailty indices in DRF surgery is unclear.
OBJECTIVE: To compare the 5-item modified Frailty Index (mFI-5), Clinical Frailty Scale (CFS), and Charlson Comorbidity Index (CCI) for predicting complications and functional outcomes after DRF surgery in patients ≥65 years, and to assess whether machine learning (ML) enhances risk stratification.
METHODS: We retrospectively analyzed 562 patients (mean

BACKGROUND: Fracture-related infections (FRI) are estimated to complicate approximately 1% of closed fractures and 30% of open fractures globally. These statistics, however, can vary significantly by region, with low-income and middle-income countries experiencing a disproportionately high burden. The studies in these regions are often underpowered to evaluate the local prevalence and contributing factors of FRI. The objectives of this study were (1) to assess the prevalence of FRI in open long-bone injuries and (2) investigate the association between FRI and various risk factors identified in

INTRODUCTION: The aim of this study was to compare two surgical procedures for the treatment of tibial pilon fractures, i.e. minimally invasive plate osteosynthesis (MIPO) vs. open reduction and internal fixation (ORIF), in terms of wound healing time, ankle swelling, change in pain, functional outcomes and complications.
HYPOTHESIS: We hypothesized that MIPO treatment would induce a faster healing process and lower complications rates.
PATIENTS AND METHODS: This monocentric, comparative, prospective and randomized study was based on the enrollment of 54 patients between November 2017 and Ma
