Rehabilitation of High-Level Dynamic Balance Skills in an Individual With Significant Nonprogressive Cerebellar Atrophy: Case Report.

A 46-year-old man received a course of quinolone-family antibiotics for puncture wound-related cellulitis. Cerebellar symptoms developed within 3 weeks of treatment, progressed over 6 months, and then became static. Seven years later, he received an individualized 12-week-long course of physical therapy to challenge his balance with high-level fall-inducing but safe dynamic activities. Outcome measures included the International Cooperative Ataxia Rating Scale, modified Dynamic Gait Index, instrumented gait analysis, and posturography. The total modified dynamic gait index score improved 4 poi…

The application of the Lyapunov Exponent to analyse human performance: A systematic review.

Variability is a normal component of human movement, allowing one to adapt to environmental perturbations. It can be analysed from linear or non-linear perspectives. The Lyapunov Exponent (LyE) is a commonly used non-linear technique, which quantifies local dynamic stability. It has been applied primarily to walking gait and appears to be limited application in other movements. Therefore, this systematic review aims to summarise research methodologies applying the LyE to movements, excluding walking gait. Four databases were searched using keywords related to movement variability, dynamic stab…

A procedure and model for the identification of uni- and biarticular structures passive contribution to inter-segmental dynamics.

Inter-segmental moments come from muscles contractions, but also from passive moments, resulting from the resistance of the periarticular structures. To quantify the passive contribution of uni- and biarticular structures during gait, we propose an innovative procedure and model. 12 typically developed (TD) children and 17 with cerebral palsy (CP) participated in a passive testing protocol. The relaxed lower limb joints were manipulated through full ranges of motion while kinematics and applied forces were simultaneously measured. The relationships between uni-/biarticular passive moments/forc…

How Paretic and Non-Paretic Ankle Muscles Contract during Walking in Stroke Survivors: New Insight Using Novel Wearable Ultrasound Imaging and Sensing Technology.

Abnormal muscle tone and muscle weakness are related to gait asymmetry in stroke survivors. However, the internal muscle morphological changes that occur during walking remain unclear. To address this issue, this study investigated the muscle activity of the tibialis anterior (TA) and medial gastrocnemius (MG) of both the paretic and non-paretic sides during walking in nine stroke survivors, by simultaneously capturing electromyography (EMG), mechanomyography (MMG), and ultrasound images, and using a validated novel wearable ultrasound imaging and sensing system. Statistical analysis was perfo…

Location of ischemia and ischemic pain intensity affect spatiotemporal parameters and leg muscles activity during walking in patients with intermittent claudication.

The ways in which locations of ischemia and ischemic pain affect spatiotemporal gait parameters and leg electromyographic activity during walking have never been investigated in patients with peripheral arterial disease presenting intermittent claudication. Two groups were classified according to unilateral location of ischemia (distal, n = 10, or proximo-distal, n = 12). Patients described pain and three gait phases-initial pain-free, onset of pain and maximum pain-were analyzed. Patients with proximo-distal ischemia walked less (230 ± 111 m vs 384 ± 220 m), with increased step length, step t…

Chronic subacromial impingement leads to supraspinatus muscle functional and morphological changes: Evaluation in a murine model.

Muscle atrophy and fatty infiltration have been directly correlated with higher rates of incomplete or failed healing following surgical repair of the rotator cuff. The purpose of this study was to evaluate clinically relevant functional and morphological changes in the supraspinatus muscle at various time points in this model of rotator cuff tendinopathy. Subacromial impingement was induced in 47, male C57BL/6 mice (total 94 limbs) by implantation of a metal clip in the subacromial space. Specimens were evaluated at 4, 6, and 12 weeks postoperatively. Gait analysis was used to measure various…

Short-term functional advantages after medial unicompartmental versus total knee arthroplasty.

BACKGROUND: There are many uncertainties about the advantages and disadvantages of using unicompartmental (UKA) versus total knee arthroplasty (TKA) to treat patients with knee osteoarthritis. It is important to have sufficient early postoperative quadriceps strength for long-term, self-reported and gait-related outcomes after knee arthroplasty, but very limited comparative data exist regarding UKA and TKA patients. METHODS: This study assessed isometric quadriceps strength, spatio-temporal gait parameters (walking speed, step length, single-limb support phase) and self-reported outcomes (pai…

Pathological trunk motion during walking in children with amyoplasia: is it caused by muscular weakness or joint contractures?

The aim was to investigate the causes for pathological trunk movements during gait in children with Amyoplasia. Eighteen children with Amyoplasia were compared with 18 typically developed children. Three-dimensional motions of pelvis, thorax and spine during gait were analyzed. Excessive trunk movements were defined as being above 4 standard deviations of those of typically developed children. Clinical examination of active strength and passive range of motion of the hip, knee and ankle joints were correlated to the parameter that showed the greatest prevalence of pathological trunk motion. Th…

Rapidly progressive spinal muscular atrophy in an ambulatory 2-year-old male.

A 2-year 9-month-old male was referred for gait disturbances. Main complaints were abnormal gait with frequent falls observed as soon as he began to walk unaided, at 18 months of age. The first neurologic examination revealed symmetric and proximal weakness in the lower limbs with difficulty running and walking upstairs. Deep tendon reflexes were decreased, and generalized hypotonia was observed. Three months later, at 3 years of age, he had lost independent gait, and 1 month later he could not stand unaided. DNA analysis revealed homozygous deletion in exons 7 and 8 of SMN1 gene, confirming t…

Evaluation of the biomechanics of lower limbs in normal, overweight, and obese people using the SMART F-IMU system.

This study provided a pilot evaluation and proof of concept for the proposed SMART F-IMU system, designed to investigate gait characteristics in normal-weight (NW) and overweight/obese (OW/OB) participants. This system combined modified inertial measurement units (IMUs) and flexible force sensors to provide estimates of ground reaction force (GRF), joint angles, and spatiotemporal gait parameters. The SMART F-IMU appeared to address the limitations of traditional gait analysis methods, enabling convenient and affordable assessments in real-world settings. The OW/OB group exhibited significantl…

Effects of an ICT-Based Wearable Intervention on Physical Function in Arteriosclerosis Obliterans: A 12-Week Study.

Arteriosclerosis obliterans (ASO) is associated with impaired walking function and claudication. However, the effects of information and communication technology (ICT)-based wearable interventions on objectively measured gait outcomes in this population have not been determined. In this 12-week intervention, 52 patients with ASO were randomly assigned to an ICT-based wearable-assisted exercise intervention (n = 30) or a control (n = 22) group. All participants wore a triaxial accelerometer-based device on the non-dominant wrist to monitor moderate-to-vigorous physical activity (MVPA), expresse…

Augmenting a ResNet + BiLSTM Deep Learning Model with Clinical Mobility Data Helps Outperform a Heuristic Frequency-Based Model for Walking Bout Segmentation.

Wearable sensors have become valuable tools for assessing gait in both laboratory and free-living environments. However, detection of walking in free-living environments remains challenging, especially in clinical populations. Machine learning models may offer more robust gait identification, but most are trained on healthy participants, limiting their generalizability to other populations. To extend a previously validated machine learning model, an updated model was trained using an open dataset (PAMAP2), before progressively including training datasets with additional healthy participants an…

The Effects of Decreasing Foot Strike Angle on Lower Extremity Shock Attenuation Measured with Wearable Sensors.

Shock attenuation may be a clinically feasible method to assess changes in lower extremity joint loading induced by gait modifications, such as decreasing foot strike angle (forefoot striking). The purpose of this study was to identify changes in lower extremity shock attenuation between habitual and forefoot strike gait conditions. Eighteen participants ran on a treadmill with their habitual gait and an instructed forefoot strike gait. Shock attenuation was measured with inertial measurement units as the ratio of proximal to distal peak resultant/vertical accelerations, with three sensor comb…
