Molecular Diagnosis and Phenotypic Variability of Noonan Syndrome: Experience from a Romanian Multicenter Study.

Background: RASopathies represent a clinically and genetically diverse group of syndromes resulting from germline mutations in genes regulating the RAS/mitogen-activated protein kinase (MAPK) signaling cascade. Methods: The aim of this study was to describe the clinical features and genetic variants identified in patients with genetically confirmed Noonan syndrome (NS) in a limited cohort from Romania. A total of 25 patients with positive genetic testing for NS-associated genes were included. Genetic testing was performed primarily using next-generation sequencing. Results: A total of twenty-s

Clinical, Histological, and Genetic Characterization of a Large Cohort of 49 Patients With Nebulin-Related Congenital Myopathy.

Congenital nemaline myopathies are rare genetic disorders that typically manifest at birth or in childhood, with muscle weakness and respiratory distress. They are characterized by the presence of rod-like structures on the muscle biopsy, or a mix of rods with cores, focal areas with disorganization of oxidative activity. Pathogenic variants in the NEB gene represent a main cause of these conditions. However, significant phenotypic variability exists, hampering disease prognosis, healthcare, and genetic counseling. This study is aimed at further characterizing the clinical and genetic features…

On the use of familial aggregation in population-based case probands for calculating penetrance.

BACKGROUND: Estimating the lifetime risk associated with (i.e., the penetrance of) genetic abnormalities that predispose individuals to cancer is important for genetic counseling. (Penetrance may be estimated from the degree of familial aggregation of cancer, that is, the extent to which cancers cluster in families.) Early penetrance studies of BRCA1 and BRCA2 mutations used high-risk families with multiple cases of breast cancer, a study design that led to very high penetrance estimates. However, such studies were subject to potential ascertainment biases. To offset such biases, recent studie…

The clinical introduction of genetic testing for Alzheimer disease. An ethical perspective.

OBJECTIVE: Primary caregivers should be aware of recent progress in the genetics of Alzheimer disease (AD) and of the clinical and ethical considerations raised regarding the introduction of genetic testing for purposes of disease prediction and susceptibility (risk) analysis in asymptomatic individuals and diagnosis in patients who present clinically with dementia. This statement addresses arguments for and against clinical genetic testing. PARTICIPANTS: The 20 participants were selected by the investigators (S.G.P., T.H.M., A.B.Z., and P.J.W.) to achieve balance in the areas of genetics, co

Non-invasive prenatal diagnosis (NIPD) for single gene disorders: cost analysis of NIPD and invasive testing pathways.

OBJECTIVE: Evaluate the costs of offering non-invasive prenatal diagnosis (NIPD) for single gene disorders compared to traditional invasive testing to inform NIPD implementation into clinical practice. METHOD: Total costs of diagnosis using NIPD or invasive testing pathways were compared for a representative set of single gene disorders. RESULTS: For autosomal dominant conditions, where NIPD molecular techniques are straightforward, NIPD cost £314 less than invasive testing. NIPD for autosomal recessive and X-linked conditions requires more complicated technical approaches and total costs we

Wilson v. Kuenzi, 17 May 1988.

The parents of a child born with Down's Syndrome brought a wrongful birth action on their own behalf and a wrongful life action on the behalf of their child against their physician, claiming that he had failed to advise the mother of the availability of amniocentesis, thus precluding her from having an abortion to prevent birth. The Court dismissed the actions, ruling that no such actions were recognized in Missouri in 1983, the time that the negligence of the physician was alleged to have occurred. In 1988, other US courts held as follows with respect to wrongful birth, wrongful life, and wro

Universal newborn screening for familial hypercholesterolaemia-insights from healthcare professionals in Queensland, Australia.

Background Familial hypercholesterolaemia is a common disorder leading to premature atherosclerotic cardiovascular disease. Despite available treatment options and guideline recommendations for early screening and intervention, fewer than 10% (1 in 10) of individuals with familial hypercholesterolaemia have been identified. Objective To assess if healthcare providers in Queensland, Australia, support introduction of newborn screening for familial hypercholesterolaemia with a focus on benefits, concerns, barriers, and solutions. Methods A survey consisting of 16 items was conducted. Healthcare

A Rare Case of Coexisting 22q11.2 Deletion Syndrome and Cornelia De Lange Syndrome: A Case Report and Review of the Literature.

The coexistence of multiple genetic syndromes in a single individual is rare but clinically significant, as it can lead to delays in diagnosis and management. To ensure high-quality, patient-centred care, clinicians must remain alert to the possibility of dual diagnoses. This case report describes a rare presentation encountered in a clinical genetics setting and highlights its key learning points, complemented by a review of the relevant literature. We report the case of a 15-year-old girl born prematurely with a complex medical history including developmental delay, feeding difficulties, joi

Genetic basis of channelopathies and cardiomyopathies in Hong Kong Chinese patients: a 10-year regional laboratory experience.

INTRODUCTION: Hereditary channelopathies and cardiomyopathies are potentially lethal and are clinically and genetically heterogeneous, involving at least 90 genes. Genetic testing can provide an accurate diagnosis, guide treatment, and enable cascade screening. The genetic basis among the Hong Kong Chinese population is largely unknown. We aimed to report on 28 unrelated patients with positive genetic findings detected from January 2006 to December 2015. METHODS: Sanger sequencing was performed for 28 unrelated patients with a clinical diagnosis of channelopathies or cardiomyopathies, testing
