Spontaneous ovarian hyperstimulation syndrome - understanding the dilemma.

Ovarian hyper-stimulation syndrome (OHSS) is an uncommon identity due to variable number of causes, gestation and gestational disease being more common than thyroiditis and other causes. The role of radiology and biochemical markers are of utmost importance in not only diagnosing spontaneous ovarian hyper-stimulation syndrome (sOHSS) but also ruling out other cystic ovarian diseases and to determine the underlying aetiology and course of the disease. Understanding its pathophysiology and genetics holds the key to unravel the mysteries of this condition.

Integration of pharmacometabolomic and pharmacogenomic approaches reveals novel insights into antiplatelet therapy.

Interindividual variability in response to antiplatelet therapy results in higher platelet reactivity as well as higher rates of cardiovascular events. Despite substantial effort, the genetic and nongenetic determinants of antiplatelet variability remain poorly understood. Emerging pharmacometabolomic paradigms that integrate systems approaches such as pharmacogenomics have the potential to unveil novel biology regarding disease pathogenesis, reveal the effect of drugs on pathways, and allow better understanding of response variability. Such approaches offer great potential for personalized an

[Effective epidural anesthesia for cesarean section in parturient woman with type I mucopolysaccharidosis (Hurler's syndrome)].

Mucopolysaccharidosis is genetically determined disease caused by the deficiency of one of the lysosomal enzymes involved in the glycosaminoglycan (GAG) breakdown pathway. This metabolic block leads to the accumulation of GAG in various organs and tissues and affects nerve system, eyes, internal organs and musculoskeletal system. The reviewer of various articles on pubmed showed only one article about epidural anesthesia in patient with type I mucopolysaccharidosis. Our observation allows to consider that epidural anesthesia with ropivacaine is possible and safe in patients with type I mucopol

Molecular epidemiology of rabies virus in Mongolia, 2005-2008.

The objective of this study was to determine the genetic diversity of rabies virus (RABV) in Mongolia based on the nucleotide sequences of viral N gene. A total of 24 rabies-positive samples from seven different domestic and wild animal species collected in western and central Mongolia between 2005 and 2008 were examined for their N gene sequences. The results showed that the endemic Mongolian RABVs could be divided into two different groups closely related to the Steppe-type and Arctic-like viruses isolated in Russia.

[Genetic markers of predisposition to infectious complications in neonatal infants with respiratory distress syndrome].

Neonatal respiratory distress syndrome (NRDS) is the commonest cause of death and morbidity of the newborn. A genetic risk for NRDS is currently recognized. The aim of this study was to determine whether there was an association of the polymorphisms of the SP-B, SP-D genes and the interleukins (IL)-1alpha, IL-10 genes with the development of infectious complications in neonatal infants with the respiratory distress syndrome. It was found that the certain genotypes by the polymorphic loci of surfactant proteins and interleukins might be associated with infectious diseases in the newborn with re

[Autoimmune thyroiditis: diagnosis and treatment].

Autoimmune thyroiditis (AIT) is an autoimmune disease with genetic background. Women are more likely to develop AIT. In adulthood, AIT is the most frequent cause for acquired hypothyroidism. Cytotoxic T-lymphocyte antigen (CTL-4) may play a role in the etiology of the disease. Diagnosis is made by determination of elevated antibodies against thyroid peroxidase and thyroglobulin, and a hypoechoic pattern in ultrasound. In case of hypothyroidism a substitutive therapy with levothyroxine is necessary. AIT may occur as postpartum thyroiditis or as a special entity as consequence of therapy with cy

[Phylogenetic analysis of complete nucleotide sequence of a snowshoe hare (Lepus americanus) virus genome].

The complete nucleotide consequence of snowshoe hare (Lepus americanus) virus (SSHV) genome was first determined. Phylogenetic analysis indicated the identity of the study strain in the complete S segment and an L-segment with a length of 145 amino acid residues with SSHV sequences from the database GenBank (J02390, AF393325). The homology of an M segment of the study segment was 99.7% with the consequences of the same from the database GenBank (K02539). La Crosse virus is most genetically close to SHV among the representatives of the California encephalitis serocomplex.

[Approaches to species-specific typing of double mixed cultures comprising pseudotuberculosis bacteria and atypical plague bacillus strains].

The species relevance of atypical Yersinia strains was determined by various microbiological, immunological, and genetic (including polymerase chain reaction) tests. These strains were shown to represent mixed cultures of Y. pseudotuberculosis serovariant O1b and Y. pestis var antiqua. Identification-resistant cells with atypical properties and plasmid segregation were found in the populations of Y. pestis strains. Analysis of different diagnostic tests revealed the most reliable ones selected for the identification of atypical Y. pestis strains with unstable genome.

Processing facial emotions in adults with velo-cardio-facial syndrome: functional magnetic resonance imaging.

We studied the functional neuroanatomy of social behaviour in velo-cardio-facial syndrome (VCFS) using a facial emotional processing task and functional magnetic resonance imaging in adults with this syndrome and controls matched for age and IQ. The VCFS group had less activation in the right insula and frontal brain regions and more activation in occipital regions. Genetically determined abnormalities in pathways including those involved in emotional processing may underlie deficits in social cognition in people with VCFS.

Ectopia or concomitant hypohyperdontia? A case report.

This report describes the unusual appearance seen on a panoramic radiograph of an orthodontic patient which the authors argue may represent ectopia or concomitant hypohyperdontia of the mandibular premolar teeth. A literature review describes the frequency of such anomalies in this area from previous studies. The presenting features of the patient and the differential diagnoses are explored. Treatment planning is discussed and treatment carried out in this particular case is detailed. The unusual symmetrical bilateral anomalies in this patient may point to a genetic determinant of tooth germ p

Treatment of aggression in Tourette syndrome.

The largely non-specific and/or multiply-determined etiologies of aggressive symptoms in TS pose significant impediments to effective clinical management. At this time, treatment requires comprehensive neuropsychiatric assessment with a systematic prioritization of the different psychiatric co-morbidities that require intervention. Medication side effects, psychosocial stressors and environmental triggers must also be identified and addressed. The future elucidation of meaningful clinical and genetic subtypes of TS will have important consequences for the more specific prevention and treatment

[A case of Edwards' syndrome in pregnancy complicated by serologic incompatibility and preeclampsia].

A case of Edwards' syndrome (trisomy 18) diagnosed in the third pregnancy trimester is described. The diagnosis was based on sonographic examination and cytogenetic amniocentesis. Lethal genetic fetal malformation determined the medical indication to preterm delivery. Additionally, serologic incompatibility during pregnancy was observed, as well as pregnancy induced hypertension turning into preeclampsia after the labour action was evoked. A caesarean section due to obstetric indications was done. Phenotype and lethal congenital malformations in the newborn have confirmed of the chromosome abe

[Pathophysiology of postmenopausal osteoporosis].

Osteoporosis is a huge public health problem because it is directly related with fractures. A number of genetic and environmental factors are interweaving in the complex pathophysiology. These factors determine peak bone mass and the degree (rapidity) of bone loss later on. Understanding of the molecular mechanism of bone formation and remodeling, as well as understanding of factors responsible for bone quality (strength) have been significantly improved over the last few years. Thorough knowledge of postmenopausal osteoporosis pathophysiology is the prerequisite for effective prevention and t

The pathophysiology and mechanisms of NP-C disease.

The molecular isolation of NPC1 and NPC2, the genes defective in patients with Niemann-Pick disease type C (NP-C), has heralded in an exponential increase in our understanding of this syndrome and thus of human intracellular sterol transport. Despite this, neither the mechanisms of action nor the substrates for these putative transporters have been defined. In this overview, we describe our perspectives on the current awareness of the genetic determination and cellular biology of this syndrome, with emphasis on the underlying events that lead to neurodegeneration and the manner in which they m

[Thyroid-associated ophthalmopathy: physiopathology, endocrine status].

The pathogenesis of Graves' ophthalmopathy remains uncertain. An inflammatory reaction is well established in orbital tissues in Graves' thyrotoxicosis. This inflammation is associated with antithyroid antibody action, but is not systematic in autoimmune hyperthyroidism patients. If ocular manifestations are pathognomonic for Graves' disease they can present in association with euthyroidism or hypothyroidism. Ophthalmopathy factors could be genetically determined but environmental parameters such as smoking and stress may also be involved, supporting the hypothesis that the autoimmune process

Gene expression studies of osteoporosis: implications for microarray research.

Osteoporosis is one of the most common bone-related disorders in the elderly. It is a complex disease, and largely determined genetically. Traditional gene expression studies have shown that osteoporosis has complex regulating mechanisms which are controlled by multiple inherent factors, such as hormones, cytokines, various receptors, etc. The complex nature of osteoporosis, and the large number of genes involved in its onset and development, suggest the use of the state of the art microarray technique as a powerful tool to unravel mechanisms underlying etiology of osteoporosis.

The alveolar macrophage: the Trojan horse of Bacillus anthracis.

Bacillus anthracis, the causative agent of anthrax, has a particular strategy for invading the host and crossing the alveolar barrier. B. anthracis survives within alveolar macrophages, after germination within the phagolysosome, then enters the external medium where it proliferates. Recent data have shown that edema toxin and lethal toxin are the major genetic determinants mediating the survival of germinated spores within macrophages. Here, recent advances in the analysis of B. anthracis pathogenesis are summarized and future challenges discussed.

Absence of correlation between BMP-4 polymorphism and postmenopausal osteoporosis in Italian women.

Several studies have demonstrated that bone has the power of regeneration and repair. BMPs (bone morphogenetic proteins) are involved in the determination of osteoblast phenotype and bone turnover, therefore genes coding for these proteins, like BMP-4, could be considered potential candidate genes for osteoporosis. We investigated the association of BMP-4 gene polymorphism with osteoporosis in a cohort of 72 osteoporotic, postmenopausal women and 82 unrelated controls. We failed to detect any significant association between this genetic marker and the disease.

Familial cluster of ovarian small cell carcinoma: a new mendelian entity?

We report a pedigree in which three sisters had a particular type of ovarian cancer, small cell carcinoma of the hypercalcaemic type. This rare type of ovarian carcinoma is now well characterised by clinical and pathological findings and is well distinguished from other ovarian epithelial tumours and ovarian germ cell tumours. The occurrence of this rare type of cancer in several members of the same family and the existence of four other similar published observations raises the question of the genetic determination of this kind of tumour.

Management of rhesus isoimmunization by preimplantation genetic diagnosis.

A genetic assay by single blastomere analysis was developed for rhesus (RhD) blood group typing of early cleavage stage embryos. The method, which is based on the simultaneous amplification of an RhD-specific sequence and an internal control in single cells, was applied for the selective transfer of RhD-negative embryos in a family of an RhD sensitized woman and a heterozygote partner. The RhD status of two out of three biopsied embryos was determined. According to their amplified products, both were typed as RhD-negative and transferred to the uterus. Pregnancy was not achieved.
