Beyond compliance: Patient risk, ERAS adherence, and postoperative outcomes through explainable machine learning.

BACKGROUND: Enhanced Recovery After Surgery (ERAS) protocols improve outcomes after colorectal surgery, but adherence remains variable and may interact with patient risk. Traditional compliance scores lack granularity to explore these dynamics. We aimed to use interpretable machine learning to quantify the contribution of individual ERAS items and clinical features to postoperative complications, and to identify data-driven ERAS phenotypes. METHODS: This was a secondary analysis of the EuroPOWER cohort (NCT04889798), a prospective European study including 2841 adults undergoing elective color

Age-related differences in the intake of heart failure therapy among participants from the MyoVasc study.

AIM: To investigate whether older individuals (≥70 years) with heart failure receive guideline-recommended heartfailure medications less frequently than younger individuals. FINDINGS: Older individuals had a higher prevalence of heart failure and greater polypharmacy but were treated lessoften with guideline-based therapies, including aldosterone antagonists and beta-blockers, even after adjustment forrenal function and comorbidities. In contrast, calcium channel blockers were prescribed more frequently in geriatricpatients. MESSAGE: Older individuals with heart failure appear to be undertre

Multimodal intervention to decrease central line-associated bloodstream infection rate in pediatric intensive care unit.

BACKGROUND: Central line-associated bloodstream infections (CLABSIs) are the most common healthcare-associated infections, leading to increased mortality, hospital stays, and costs. Despite evidence-based prevention bundles, adherence is often suboptimal. This study aimed to evaluate the impact of a multimodal intervention on CLABSI rates in the pediatric intensive care unit (PICU), particularly in tertiary care settings with high infection rates. METHODS: A quasi-experimental study was conducted from September 2022 to March 2024, involving pediatric patients under 15 years with central venou

Comfort rules for face masks among healthcare workers during COVID-19 spread.

BACKGROUND: The outbreak of Coronavirus disease 2019 (COVID-19) made imperative the use of protective devices as a source control tool. As there is no definite antiviral treatment and effective vaccine, the only efficient means of protecting and mitigating infectious contagion has been the use of personal protective equipment, especially by healthcare workers. However, masks affect the humidification process of inhaled air, possibly leading to a basal inflammatory state of the upper airways. STUDY DESIGN: This is a single-center observational study conducted at the University Hospital of Cata

Barriers inhibiting effective detection and management of postpartum hemorrhage during facility-based births in Madagascar: findings from a qualitative study using a behavioral science lens.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading cause of maternal mortality in low-income countries, and is the most common direct cause of maternal deaths in Madagascar. Studies in Madagascar and other low-income countries observe low provider adherence to recommended practices for PPH prevention and treatment. Our study addresses gaps in the literature by applying a behavioral science lens to identify barriers inhibiting facility-based providers' consistent following of PPH best practices in Madagascar. METHODS: In June 2019, we undertook a cross-sectional qualitative research study

A "preamputation care pathway" reduces mortality for lower limb amputation.

Lower limb amputation due to peripheral arterial disease is common. The comorbid nature of this patient group makes management challenging. The aims of the study were 1) to introduce a novel "preamputation care pathway" to facilitate perioperative care and 2) to evaluate whether such a pathway is able to reduce morbidity and mortality. All patients undergoing lower limb amputation over 12 months were prospectively identified. Patient demographics were recorded before statistical analysis was performed. Twelve limbs were amputated (mean age, 69 years; mean American Association of Anesthesiologi

Viral hepatitis B and C outbreak related to parenteral treatment at an oncological department in Slovakia.

The Regional Public Health Authority was notified about four cases of viral hepatitis (B and C) among patients with pre-event history of hospitalization at an oncological ward. An outbreak investigation was commenced in response. Forty-seven cases of viral hepatitis B and C in 39 patients hospitalized during the period from September 2009 to July 2010 were registered. We found an association between parenteral treatment and disease (hepatitis B: χ(2) = 49.53; P < 0.001; hepatitis C: χ(2) = 22.42; P < 0.001). Despite using disposable materials, there remains a risk of transmission of bloodborne
