Catastrophic Health Expenditure in Mental Health as Coercion in Post-Suicide Attempt Care: A Population-Based Study in Latin America.

BACKGROUND: Despite an increase in mental health problems, there is low implementation of mental health services, high catastrophic health spending, and a high incidence of coercive mental health practices. This could hinder recovery after a suicide attempt. AIMS: To analyze catastrophic spending and coercive practices, and their relationship with variables associated with recovery (depression and well-being) and with the functioning of health services (continuity of care), in people who attempted suicide during 2024 and 2025 in Caldas, Colombia. METHOD: A cross-sectional and population-base

Non-Communicable Diseases, Longevity, and Health Span: A Hong Kong Perspective.

The health of different countries evolves in parallel with their economic development. Communicable diseases play a more prominent role in low-income countries compared with high-income countries, while non-communicable diseases (NCDs) have become dominant in developing and developed economies. This transition has been accompanied by public health efforts to prevent NCDs, resulting in improvements in total life expectancy at birth (TLE). It is recognized that health determinants are not confined to healthcare systems, but that social determinants play a key role in health inequalities. The rap

Pediatrics in the year 2020 and beyond: preparing for plausible futures.

Although the future of pediatrics is uncertain, the organizations that lead pediatrics, and the professionals who practice within it, have embraced the notion that the pediatric community must anticipate and lead change to ultimately improve the health of children and adolescents. In an attempt to proactively prepare for a variety of conceivable futures, the board of directors of the American Academy of Pediatrics established the Vision of Pediatrics 2020 Task Force in 2008. This group was charged to think broadly about the future of pediatrics, to gather input on key trends that are influenci

The use of 'tie down' in New Zealand prisons-what is the role of the health sector?

We draw upon two recent reports from the Chief Ombudsman that describe the prison management of people assessed at risk of self-harm or suicide, as cruel, inhuman or degrading treatment or punishment. People were mechanically restrained on 'tie-down' beds by their legs, arms and chest or placed in waist restraints with their hands cuffed behind their backs over prolonged periods. These practices occurred at the direction of, or were approved by, health professionals. We highlight ethical issues for health professionals party to 'tie down' and examine the current guidance and regulatory framewo

Crafting veteran to baccalaureate nursing programs.

Upon leaving the military, veteran health care personnel have difficulty finding commensurate civilian employment, yet their military transcripts document a strong record of health care education and experience. Vigorous Veterans' Bachelor of Science in Nursing (VBSN) programs can provide military personnel with opportunities to become registered nurses. This article describes the incremental planning, curriculum design, prior-learning evaluation option implementation, and student veteran results from one VBSN program with a ten-year history of successful, sustainable delivery. Congenital anomalies of the upper digestive tract: a review of esophageal and gastric embryology and malformations.

Congenital anomalies of the digestive system, particularly in the esophagus and stomach, present significant challenges to neonatal health due to their complex management and associated risks of morbidity and mortality. This narrative review explores esophageal and gastric anomalies, addressing the embryology, prevalence, diagnostic methods, and treatment approaches, with an emphasis on early detection and improved outcomes. Because of recent advances in prenatal imaging, such as ultrasonography and magnetic resonance, diagnostic accuracy has increased, allowing better clinical planning and ma

Felt Stigma in Youth With Disorders of Gut-Brain Interaction: Implications for Abdominal Pain, Sleep, and Mood.

Children with painful disorders of gut-brain interactions (DGBIs) endorse pain-related stigmatization. However, little is known about how stigma may be associated with pain intensity. Consequently, we conducted a cross-sectional study to identify a potential pathway between felt stigma and abdominal pain in youth with DGBIs. 120 youth completed measures of health-related stigma, depressive symptoms, sleep disturbances, and pain severity. As expected, greater felt stigma was significantly correlated with greater depressive symptoms, sleep-related impairments, and abdominal pain. Furthermore, a

Socioeconomic and geographic inequalities in hospital-level ADRD burden: implications for hospital-to-home transitions and dementia care coordination.

Alzheimer's disease and related dementias (ADRD) create challenges for hospital-to-home transitions, caregiver preparation, and home health care coordination. This cross-sectional study examined socioeconomic and geographic inequalities in hospital-level ADRD burden among 3,027 U.S. hospitals serving Medicare fee-for-service beneficiaries. Multivariable quasibinomial regression assessed associations between ADRD prevalence and hospital characteristics, rurality, census region, and patient composition. An age-adjusted sensitivity analysis was conducted among 3,003 hospitals with complete age-co

Badge of Courage: Pain and Suffering After Military Service in a Nonveteran.

Feigning and fabrication by a patient can evoke anger and distress in staff and generate diagnosis and management challenges. When faced with difficulties that can lead to moral injury among health care providers, and even risk compromised patient care, clinical teams must balance empathy with appropriate boundaries. This case report examines an individual with a history of polysubstance use and acute-on-chronic pain whose persistent patterns of fabrication-spanning exaggerated military service, terminal illness, family trauma, and academic accomplishments-posed substantial risk-assessment and

The Effect of Diabetes Mellitus on Mandibular Growth.

Diabetes mellitus is a chronic condition characterized by insufficient insulin production or utilization. Affecting approximately 8.5% of adults globally, diabetes is categorized primarily into Type 1, Type 2, and gestational diabetes. Diabetes markedly impacts bone health, particularly affecting the growth and development of the mandible. Key alterations include impaired bone metabolism leading to diminished bone density and strength. Additionally, diabetes impairs bone healing processes, often exacerbated by deficiencies in vitamin D, thus increasing fracture risks. Understanding the interpl

[County differences in incidence and mortality of malignant neoplasms in Hungary between 2005 and 2019].

The objective of our study was to map county differences in incidence and mortality by cancers and examine their changes over time. Based on the database of National Cancer Registry and Central Statistical Office, age-standardized incidence and mortality rates per 100,000 person-years were calculated for each county for 15 cancer types and 3 time periods. East-West divide was apparent in incidence and mortality of lung cancer, with larger weight in East (Borsod-Abaúj-Zemplén, Heves, Jász-Nagykun-Szolnok, Békés counties). Concentration of lip and oral cavity malignancies was identified in the n

Dr. Fe Del Mundo: The Pioneer Who Transformed Pediatrics and Child Healthcare in the Philippines.

Dr. Fe del Mundo was a female Filipino pediatrician, humanitarian, and advocate of Filipino children, dedicating her life to seven decades of service. Her programs on maternal and child health, preventive medicine, nutrition, vaccination, and family planning transformed the healthcare landscape in the Philippines. Overcoming adversity, she pursued a road less traveled for women of her time. After training in the United States of America, she returned to the Philippines to take care of sick children during the Second World War and later opened the first pediatric hospital in the country. Her re
