Advances in drug allergy, urticaria, angioedema, and anaphylaxis in 2018.

Many notable advances in drug allergy, urticaria, angioedema, and anaphylaxis were reported in 2018. Broad-spectrum antibiotic use and, consequently, antibiotic resistance are widespread, and algorithms to clarify β-lactam allergy and optimize antibiotic use were described. Meaningful data emerged on the pathogenesis of delayed drug hypersensitivity reactions. Progress not only in defining biomarkers but also in understanding the effect on quality of life and developing better treatments has been made for patients with chronic idiopathic urticaria. Patients with hereditary angioedema (HAE) hav…

Recent advances in understanding non-celiac gluten sensitivity.

Non-celiac gluten sensitivity (NCGS) is a condition characterized by intestinal and extra-intestinal symptoms related to the ingestion of gluten-containing foods in the absence of celiac disease and wheat allergy. The diagnosis is cumbersome and currently confirmed only by gluten withdrawal and double-blind placebo challenge protocols. There is great overlap in symptoms between NCGS and other functional gastrointestinal disorders, making a differential diagnosis difficult. The pathophysiology of NCGS is largely unclear, and there are contrasting data on the trigger of this condition. This revi

Non coeliac gluten sensitivity - A new disease with gluten intolerance.

Until recently gluten intolerance has been believed to be typical of celiac disease (CD) and wheat allergy (WA). In the last few years, however, several study results have been published that have proved that gluten intolerance can also affect people who do not suffer from any of the above mentioned diseases. The new syndrome has been named non-celiac gluten sensitivity (NCGS) or gluten sensitivity (GS). It has been included in the new list of gluten-related disorders published in 2012. Researchers believe that NCGS is the most common syndrome of gluten intolerance. This review discusses many

Strategies for distinguishing asymptomatic latex sensitization from true occupational allergy or asthma.

OBJECTIVE: To evaluate strategies for distinguishing those individuals with true occupational asthma caused by exposure to natural rubber latex from those individuals with allergic disease related to other allergens. DATA SOURCES: Article published between January 1, 1981, and December 31, 2001, were identified via a MEDLINE search with the following keywords: latex, allergy, asthma, occupation asthma, and adverse reactions. STUDY SELECTION: English-language reports concerning diagnostic methods in latex and other forms of occupational asthma. RESULTS: Many methods have been evaluated for t

Adverse human health effects associated with molds in the indoor environment.

Molds are common and important allergens. About 5% of individuals are predicted to have some allergic airway symptoms from molds over their lifetime. However, it should be remembered that molds are not dominant allergens and that the outdoor molds, rather than indoor ones, are the most important. For almost all allergic individuals, the reactions will be limited to rhinitis or asthma; sinusitis may occur secondarily due to obstruction. Rarely do sensitized individuals develop uncommon conditions such as ABPA or AFS. To reduce the risk of developing or exacerbating allergies, mold should not be

Anaphylactic reaction to lychee in a 12-year-old girl: cross-reactivity to latex?

There are very few case reports on allergic reactions to lychee in the literature - so far only in adults. We report on a 12-year-old girl who developed swelling of lips, pruritus, generalized urticaria and dyspnea 30 min after eating a raw lychee. A second event occurred after eating a piece of cake covered with a fruit cocktail. All other foods were well tolerated. In infancy the girl had suffered from atopic dermatitis, which disappeared in childhood; for the previous 2 yr she had presented with seasonal allergic rhinoconjunctivitis. Upon oral provocation, she developed restlessness, flush,

Latex allergy: historical perspective.

The widespread use of powdered latex gloves, following the institution of universal precautions in 1987, increased the occurrence of a host of adverse reactions to natural rubber latex (NRL). Although the most common reactions are irritant or allergic dermatitis reactions to manufacturing chemicals, a spectrum of type I IgE-mediated reactions to NRL proteins are of great concern. IgE-mediated reactions range from contact urticaria to occupational asthma and anaphylaxis, resulting in significant morbidity and potential mortality. At-risk populations include highly exposed groups like health car

Late adverse reactions to intravascular iodinated contrast media.

Late adverse reactions to intravascular iodinated contrast media are defined as reactions occurring 1 h to 1 week after contrast medium injection. They have received increasing interest over the past decade, but their prevalence remains uncertain and their pathophysiology is not fully understood. The Contrast Media Safety Committee of the European Society of Urogenital Radiology decided to review the literature and to issue guidelines. An extensive literature search was carried out and summarized in a report. Based on the available information, simple guidelines have been drawn up. The report

Similar prevalence of respiratory symptoms and atopy in Estonian schoolchildren with changing lifestyle over 4 yrs.

The prevalence of allergic sensitization and clinical manifestations is low in Eastern Europe, despite a continuous increase in industrialized countries with a market economy. The aim of the present study was to study changes in the prevalence of respiratory symptoms and atopic sensitization over time among schoolchildren in Estonia, in relation to environmental changes as the country transformed into a market economy. A cross-sectional study of 10-yr-old children was carried out between October 1996 and April 1997, employing a questionnaire regarding the prevalence of wheezing, rhinitis and i

[Food allergies and pseudo-allergies--mechanism, clinical aspects and diagnosis].

Allergic and pseudoallergic reactions caused by foods respectively food-additives present cutaneous (urticaria, erythrodermia), gastrointestinal (nausea, vomiting, diarrhoea) and respiratory symptoms (allergic bronchial asthma). The anaphylactic shock is the most severe manifestation. Exact diagnosis is based on anamnesis, skin-tests, laboratory investigations, dietetic test procedures and oral provocation. In allergic and pseudoallergic reactions the adequate therapy is the avoidance of the causative agent (diet).

An unusual local reaction to continuous subcutaneous infused terbutaline in unstable asthmatics.

Three out of nine unstable asthmatics whose asthma is subjectively and objectively better controlled on continuous subcutaneous terbutaline infusions (6-12 mg/24 hours) developed tender indurated subcutaneous swellings at the infusion site. In the two cases reported here in detail, histologically the lesions were areas of panniculitis and deep dermal collagen necrosis. Further investigation of the possible causative mechanism for these lesions--drug solution pH, osmolality, or impurities--suggests that impurities in the solution are the most likely cause.

Delayed reactivation of sunburn by methotrexate: sparing of chronically sun-exposed skin.

A 60-year-old man with metastatic transitional cell carcinoma of the bladder was treated with weekly methotrexate injections, each followed by leucovorin rescue. After two to three hours of sun exposure, he experienced a mild sunburn erythema on his lower extremities in areas not chronically sun exposed. He received methotrexate the next day. More than one week later, following another dose of methotrexate, he experienced a second-degree burn in the areas of the prior sunburn, with sparing of chronically exposed areas on his face, neck, and arms. We discuss here the reactivation and exacerbati

Contact urticaria to O-phenylphenate.

Contact urticaria is a form of immediate hypersensitivity reaction that is being noted with increasing frequency. This report illustrates an unusual presentation of contact urticaria in the form of an immediate reaction to a component of plaster cast material. The substance involved was ortho-phenylphenate, a preservative. It has not been previously implicated in a contact urticaria reaction.

Streptomycin drug fever during treatment of bilateral Meniere's disease.

The use of intramuscular injections of streptomycin sulfate for the ablation of vestibular function in patients with bilateral Meniere's disease is well established. Although hypersensitivity reactions to streptomycin are documented, frank drug fevers are rare. Our patient, with no previous history of drug allergies, was treated with parenteral streptomycin for disabling bilateral Meniere's disease. The patient's hospital course was complicated by severe hypotension and spiking fevers, which were temporally related to the streptomycin injections, and which resolved on cessation of the drug the…

[Allergy and anaphylaxis].

Acute undesirable drug reactions following the application of one of the modern amiglycoside antibiotics only occur in some exceptional cases. Those compounds reveal low sensitizing capacity, in contrast to streptomycin. Following topical applications, allergies of delayed type have been recorded, especially in the case of neomycin, a widely used topical antibiotic. Aminoglycosides exhibit anaphylactoid activity; they cause a degranulation of mast cells. Via this mechanism, histamine reactions are provoked. Such "anaphylactoid" reactions are clinically indistinguishable from true allergic (ana
