Surfactant replacement therapy--economic impact.

Surfactant replacement is an effective treatment for neonatal respiratory distress syndrome. (RDS). As widespread use of surfactant is becoming a reality, it is important to assess the economic implications of this new form of therapy. A comparison study was carried out at the Neonatal Intensive Care Unit (NICU) of Northwest Armed Forces Hospital, Saudi Arabia. Among 75 infants who received surfactant for RDS and similar number who were managed during time period just before the surfactant was available, but by set criteria would have made them eligible for surfactant. All other management mod

Causes of death and infant mortality rates among full-term births in the United States between 2010 and 2012: An observational study.

BACKGROUND: While the high prevalence of preterm births and its impact on infant mortality in the US have been widely acknowledged, recent data suggest that even full-term births in the US face substantially higher mortality risks compared to European countries with low infant mortality rates. In this paper, we use the most recent birth records in the US to more closely analyze the primary causes underlying mortality rates among full-term births. METHODS AND FINDINGS: Linked birth and death records for the period 2010-2012 were used to identify the state- and cause-specific burden of infant m

Physical Activity: An Accessible Prescription for Perinatal Mental Health.

Perinatal mood and anxiety disorders (PMADs) affect up to 20% of perinatal patients and can lead to adverse maternal and infant outcomes. Moderate-to-vigorous physical activity (MVPA) is a safe, cost-effective, and underutilized intervention that reduces the risk and severity of PMADs, with benefits comparable to psychotherapy. Despite this evidence, fewer than 15% of perinatal individuals meet MVPA recommendations due to safety concerns, limited guidance, and social barriers. Obstetrical care providers can reduce this gap by offering evidence-based counselling and using tools like the Get Act

Small kid, giant aneurysms: anakinra treatment in IVIG-resistant Kawasaki disease.

Our report details an atypical presentation of Kawasaki disease (KD) in an infant with a late initial presentation. The patient presented with large coronary artery aneurysms and was refractory to traditional KD treatment, intravenous immunoglobulin and aspirin. The patient was treated with anakinra, an interleukin-1 receptor antagonist, which improved inflammatory markers and prevented the progression of the patient's coronary artery aneurysms. There are no clear guidelines for refractory KD and limited reports of anakinra use in KD. Emerging treatment options for patient at high risk for ref

Interstitial Laser Ablation of Feeding Vessels to a Large Placental Chorioangioma.

BACKGROUND: Chorioangioma is a vascular neoplasm of the placenta with the potential to cause heart failure, hydrops, and even death. CASE: A 30-year-old patient was referred owing to a large placental chorioangioma and fetal hydrops at 28 weeks of gestation. The patient underwent ultrasound-guided interstitial laser ablation. Ten days later, fetal blood transfusion was performed and at 31 weeks of gestation, and the patient delivered a female infant by cesarean section. The newborn was discharged from the neonatal intensive care unit without any complication. CONCLUSION: According to our cas

Let's Not Miss the Treatable Ones: Two Cases of Neonatal Sepsis Due to Malaria.

Congenital malaria is the direct infection of an infant with a malarial parasite from the mother either during pregnancy or at birth. Neonatal malaria occurs due to an infective mosquito bite after birth. Neonatal and congenital malaria (NCM) can occasionally present with life-threatening neonatal sepsis and rarely with neonatal jaundice. These conditions are typically managed by general pediatricians, especially in remote areas without access to specialized care. A high clinical index of suspicion is required to diagnose neonatal and congenital malaria, given that their presentation can mimic

Recent Advances in Pathophysiology and Management of Transient Tachypnea of Newborn.

Transient tachypnea of newborn (TTN) results from failure of the newborn to effectively clear the fetal lung fluid soon after birth. TTN represents the most common etiology of respiratory distress in term gestation newborns and sometimes requires admission to the neonatal intensive care unit. TTN can lead to maternal-infant separation, the need for respiratory support, extended unnecessary exposure to antibiotics and prolonged hospital stays. Recent evidence also suggests that TTN may be associated with wheezing syndromes later in childhood. New imaging modalities such as lung ultrasound can h

Sustained remission with azacitidine monotherapy and an aberrant precursor B-lymphoblast population in juvenile myelomonocytic leukemia.

Juvenile myelomonocytic leukemia (JMML) has a poor prognosis in general, with hematopoietic stem cell transplant (HSCT) remaining the standard of care for cure. The hypomethylating agent, azacitidine, has been used as a bridging therapy to transplant. However, no patients have been treated with azacitidine without an HSCT post azacitidine. We report on an infant with JMML with somatic KRAS G12A mutation and monosomy 7 who achieved sustained remission following azacitidine monotherapy. He also developed an aberrant B-lymphoblast population which declined with similar kinetics as his JMML-associ

ACOG Committee Opinion No. 748 Summary: The Importance of Vital Records and Statistics for the Obstetrician-Gynecologist.

Information from vital records is critical to identify and quantify health-related issues and to measure progress toward quality improvement and public health goals. In particular, maternal and infant mortality serve as important indicators of the nation's health, thereby influencing policy development, funding of programs and research, and measures of health care quality. Accurate and timely documentation of births and deaths is essential to high-quality vital statistics. This Committee Opinion describes the process by which births, maternal deaths, and fetal deaths are registered; the challe

Congenital Trismus From Brainstem Dysgenesis: Case Report and Review of Literature.

Trismus refers to any condition inducing limited mouth opening and may present as a result of acquired or congenital pathology. We present the case of a newborn who presented with severe, congenital trismus due to brainstem dysgenesis. We describe the course of his investigations, and a multidisciplinary approach to the management of his care and follow-up. To our knowledge, this is one of the earliest reported cases of congenital trismus attributable to brainstem dysgenesis. A literature review was conducted to provide an overview of the differential pathogenesis as it presents in congenital

Birthing and Family Medicine: More Than Obstetrics.

The term birth injury usually applies to an injury sustained by either a woman or her infant in the course of giving birth. In this case the injury was sustained by the delivering family physician. What initially seemed like a minor injury turned out to require a significant career modification. The writer reflects on the personal impact of his choice to provide maternity care as a family physician. The unexpected end to this part of his practice, brought on by the injury, allows an opportunity to assess the effect of other "birth" experiences that are inherent to long-term patient-doctor rela

A case report of severe panhypopituitarism in a newborn delivered by a women with Turner syndrome.

Turner syndrome (TS) is a congenital disease caused by absence or structural abnormalities of sex chromosomes resulting in gonadal dysgenesis. Spontaneous pregnancies occur in 2-8% of patients, especially with mosaic kariotypes, however they are associated with increased risk of poor outcome both for mother and fetus. We report a 4-day-old male infant delivered by women with mosaic TS who was admitted to the pediatric intensive care unit and presented with severe panhypopituitarism as the early manifestation of pituitary stalk interruption syndrome (PSIS). To the best of our knowledge this is

Natural history of newborn with borderline small left heart without interatrial communication.

A patient with a borderline small left heart requires difficult clinical decision making during prenatal and neonatal periods. This report describes the unusual natural history of a newborn with prenatally diagnosed hypoplastic left heart structures and early spontaneous postnatal closure of the interatrial communication. Initial deterioration from pulmonary edema and low-cardiac output improved with postnatal intensive care. During the seventh week of life, aortic coarctation was diagnosed and the infant underwent uneventful surgical repair. Despite transient clinical deterioration, early spo

Tarsometatarsal injury in a child.

Tarsometatarsal joint injuries are well described in adults. In children, these injuries have been less described and have been reported only as small case series. They frequently go unrecognized in children because of skeletal immaturity and lack of awareness among health care providers. However, these injuries if untreated can result in significant pain and deformity in children. It is important that treating physicians recognize the symptoms and signs of these injuries and initiate further diagnostic workup, especially when there is persistent foot pain in the absence of radiographic signs

Candidal arthritis after complete treatment of systemic candidiasis.

Over the last few decades, the incidence of invasive candidal infections in neonatal intensive care units has increased dramatically. Various complications, such as arthritis, endocarditis, meningitis, and endophthalmitis, have been reviewed. We present the case of a premature infant with systemic candidemia. Arthritis was discovered 6 months after completion of amphotericin B therapy, and was successfully treated with oral fluconazole for 6 weeks. We conclude that long-term follow-up is particularly important in patients with treated candidemia. To prevent complications, prolonged treatment w

Cognitive and behavioral deficits in premature graduates of intensive care.

A substantial number of VLBW graduates of intensive care develop cognitive and behavioral problems, even in the absence of neuroimaging abnormalities. Although this article has highlighted the potential, important, contributing role of medical and stressful, neonatal, environmental conditions to the development of these deficits, it is not all-encompassing, and there are additional prenatal (ie, in utero stress, drug exposure) and neonatal (ie, infectious) contributing factors. The long-term, outcome data presented in this article are pertinent to the more mature, VLBW infant, and it remains u

Primary aldosteronism in pregnancy.

Aldosteronism is a rare complication of pregnancy. We report a case of a 26-year-old woman who became pregnant soon after a diagnosis of primary aldosteronism due to left adrenal adenoma was made. Only oral potassium supplementation was required in addition to routine prenatal care until 36 weeks' gestation. Subsequently, antihypertensive medication was needed to control elevated blood pressure. A healthy male infant was delivered by cesarean section because of abruptio placentae. The postoperative course was uneventful. Left adrenalectomy was conducted eight months after delivery under laparo
