Epithelial sensing of vitamin A shapes intestinal antimicrobial defense.

Vitamin A is a central regulator of intestinal adaptive immunity, but its role in innate immunity is less defined. Antimicrobial proteins form a chemical barrier that protects the intestinal epithelium from microbial invasion. Among these, REG3 family lectins are induced by the microbiota, yet how nutritional cues intersect with microbial signals to control their expression remains unclear. Here, we show that dietary vitamin A promotes expression of REG3 antimicrobial lectins, including REG3G, in intestinal epithelial cells from both mice and humans. This induction is mediated by retinoic acid

Phytobezoar-induced intestinal obstruction relieved via enteroscopy.

Phytobezoar-induced intestinal obstruction is a rare condition caused by previous gastric surgery and consumption of tannin-containing foods. Laparoscopy used to be a common treatment to relieve intestinal obstruction due to phytobezoars. We reported a rare case of phytobezoar-induced intestinal obstruction caused by over-consuming fresh hawthorns, and successfully treated with enteroscopy. This case report confirmed the feasibility of enteroscopy to treat phytobezoar-induced intestinal obstruction.

Dietary vitamin A restricts intestinal tuft cell differentiation.

Despite evidence highlighting immunoregulatory roles of tuft cells, it is not known whether vitamin A or retinoic acid affects tuft cell homeostasis. Here, we find that epithelial-intrinsic retinoic acid receptor regulates tuft cell numbers in mice. However, loss of retinoic acid signaling in tuft cells does not replicate this effect, suggesting altered differentiation. Consistent with this, retinoic acid administration to stem cell-derived organoids reduces tuft cell numbers, and epithelial-intrinsic retinoic acid is sufficient to alter tuft cell responses. In addition, dietary vitamin A dyna

Role of PANoptosis in intestinal diseases and its therapeutic implications.

PANoptosis is a novel programmed inflammatory cell death pathway. It integrates pyroptosis, apoptosis, and necroptosis components via a unified molecular platform termed PANoptosome. Unlike single classical cell death pathway, PANoptosis is tightly regulated by key regulators such as ZBP1, RIPK3, caspase-8, and ASC, resulting in a highly inflammatory and immunogenic form of cell death. While its pathological relevance has been widely investigated in cancer, microbial infections, and systemic inflammatory syndromes, its role in intestinal diseases has not been comprehensively explored. In this

Murine Model for Selective Transection of Intestinal Autonomic Nerves.

Autonomic nerves comprise the sympathetic and parasympathetic systems, which act in a largely antagonistic manner to regulate diverse physiological functions. With growing interest in the roles of autonomic innervation in intestinal physiology and pathology, there is an increasing need for experimental models that enable precise manipulation of gut-directed sympathetic and parasympathetic inputs. Here, we present a surgical protocol to selectively transect the sympathetic and parasympathetic nerves innervating the intestine. Using the superior mesenteric artery (SMA) as a reproducible anatomic

Diagnosis of intestinal tuberculosis: a systematic review and meta-analysis.

BACKGROUND/AIMS: Diagnosis of intestinal tuberculosis (ITB) is challenging. Histopathology and microbiological examination remain the gold standard, but previous studies show varied diagnostic performance. We aimed to systematically evaluate the accuracy of tests to diagnose ITB in both conventional and novel methods. METHODS: We searched MEDLINE and EMBASE from inception to October 2023. All studies enrolling at least 10 patients with reported information regarding the diagnosis of ITB based on endoscopic biopsy specimens, stool tests, and blood tests were included. We performed a meta-analy

Progress in the Application of Hydrogel Materials for the Treatment of Intestinal Fistula.

Intestinal fistula refers to an abnormal anatomical channel between the intestines or between the intestine and another organ, commonly resulting from factors such as intestinal incision leakage, anastomotic leak, inflammatory bowel disease, and radiation enteropathy. Following its occurrence, various complications may arise, including abdominal infection, sepsis, intra-abdominal hypertension, massive abdominal hemorrhage, necrotizing fasciitis of the abdominal wall, chronic critical illness, and multiple organ dysfunction syndrome. In most cases, surgical resection of the intestinal fistula c

OPTN/SRTR 2024 Annual Data Report: Intestine.

Intestine transplant encompasses multiple surgical procedures for a variety of indications that affect health and quality of life. Intestine transplants are performed with or without liver transplant, sometimes along with concomitant transplant of the stomach, pancreas, and colon in either scenario. In 2024, there were 128 individuals added to the waiting list of candidates who can benefit from an intestine transplant in the United States. Although most candidates had short-gut syndrome, an increasing number of candidates had diagnoses of motility disorders, extensive mesenteric thromboses, or

KIT supports small intestinal tuft cell hyperplasia.

The small intestine balances the competing tasks of nutrient absorption, immune tolerance, and defense through dynamic differentiation of short-lived epithelial cells. During helminth infection, interleukin-13 (IL-13) or IL-4 drive a 10-fold expansion of tuft cells to promote helminth clearance. While IL-4/13 signaling in epithelial cells is required for tuft cell hyperplasia, few signals that support this process have been identified. Here, we show that tuft cells across all tissues express the receptor tyrosine kinase KIT and that IL-4/13 is necessary and sufficient to up-regulate KIT on sma

HAT1 Regulates Intestinal Stem Cell Proliferation and Differentiation.

Stem cells are critical for the development and maintenance of tissue integrity. An important example is intestinal stem cells (ISCs) that generate all epithelial cell types necessary for formation of the intestinal lining. HAT1, a histone acetyltransferase that acetylates newly synthesized histone H4 molecules on lysine residues 5 and 12 during replication-coupled chromatin assembly, is specifically expressed in intestinal stem and progenitor cells located in intestinal crypts. To determine if HAT1 is important for intestinal stem and progenitor cell function, we generated an inducible deleti

Eosinophils drive intestinal remodelling and innate defence in reproduction.

Mammalian reproduction requires substantial immune adaptations to safeguard reproductive success and to ultimately shape the evolutionary trajectories of a species. Systemic and placental immunity shift towards tolerance during pregnancy1,2; however, how maternal immunity adapts in barrier tissues-which are sites of frequent infection and inflammation-from pregnancy until the postpartum lactation period remains poorly understood. Here we report a previously unrecognized role for eosinophils, a type of granulocyte typically associated with allergies and helminth infections3,4, in remodelling th

Impact of intestinal propionate metabolism on systemic metabolic homeostasis.

Propionate is an abundant short-chain fatty acid largely derived from gut microbiota in mammals. Propionate metabolism is essential to maintain systemic homeostasis, and inborn errors of essential metabolic enzymes in this pathway cause severe illness. The juxtaposition of high concentrations of propionate with the intestinal epithelium suggests a need for propionate catabolism. To understand the requirement of propionate metabolism in intestinal epithelium, we generated mice with a conditional knockout of propionyl-CoA carboxylase A (Pcca) specifically in the intestine (PccaVil-Cre). Male and

Epithelial fucosylation regulates intestinal stem cell differentiation.

The potential role of human milk oligosaccharides (HMOs) in regulating intestinal epithelial cell proliferation and differentiation in mammals has attracted considerable attention. Fucosylated HMO can provide fucosylated glycans to the intestine of newborns, an outcome similar to genetic overexpression of fucosyltransferase. However, how HMOs, such as 2'-fucosyllactose (2'-FL), may impact small intestinal fucosylation to regulate intestinal stem cell (ISC)-mediated intestinal epithelial development remains unknown. In the present study, we employed a model of small intestinal fucosylation inhi

Circadian and sympathetic regulation of intestinal eosinophil abundance in mice.

Eosinophils are abundant residents of the mouse small intestine under steady-state conditions. Their survival depends on the local cytokine milieu, including GM-CSF, and these cells contribute to the generation of IgA+ plasma cells. Here, we demonstrate that the abundance of small-intestinal eosinophils exhibits time-of-day variation accompanied by rhythmic GM-CSF expression. These oscillations were abolished in Clock-mutant mice, under disrupted light/dark cycles, and following pharmacological blockade of sympathetic nerve activity. Fecal IgA levels displayed similar temporal variation, which

When Tuberculosis Bleeds: Intestinal Tuberculosis Presenting With Hematochezia.

Intestinal tuberculosis is an uncommon but clinically significant manifestation of tuberculosis that often presents with nonspecific gastrointestinal symptoms. Although the ileocecal region is most frequently involved, overt gastrointestinal bleeding is an unusual presentation, particularly in elderly patients who may lack classic constitutional symptoms. An elderly man presented to a community hospital in Guam with hematochezia one week after the diagnosis of pulmonary tuberculosis and initiation of rifampin, isoniazid, pyrazinamide, and ethambutol. Colonoscopy revealed a large semi-circumfer

Small intestinal microbiome, the underrated maestro of SIMO disease.

Small intestinal microbial overgrowth (SIMO) results from a breakdown in the delicate equilibrium between luminal environment, gut motility, and microbial ecology. Despite extensive research, these factors have largely been investigated as separate entities, with limited integrative insights into their interplay. This review is the first comprehensive synthesis of physicochemical, mechanical, and microbial parameters shaping SIMO pathogenesis. By reviewing both clinical and experimental data, we reveal how alterations in pH, transit time, digestive secretion dynamics, bile acid composition and
