Postoperative Glaucoma Decompensation Following Spine Surgery: The Importance of Proper Patient Positioning.

Transforaminal lumbar interbody fusion (TLIF) is a widely utilized surgical procedure for the treatment of degenerative lumbar spinal conditions, including lumbar disc herniations, spinal stenosis, and spondylolisthesis. One such rare and underreported complication is vision loss following spinal surgery. Postoperative vision loss (POVL) is an extremely uncommon complication, occurring in approximately 0.002% to 0.2% of all non-ocular surgeries, including spinal procedures. We presented a 70-year-old male with complaints of left-sided cruralgia, lumbar radicular pain (sciatica-type L5), and ri

The learning curve in minimally invasive pituitary surgery.

BACKGROUND: Minimally invasive pituitary surgery (MIPS) is performed via an endoscopic transnasal transsphenoidal approach. This provides excellent illumination, visualization, and magnification of the operative field, in addition to avoiding complications associated with other approaches. In this study we examined the first 45 cases of MIPS to determine if a learning curve exists for this technique. METHODS: A retrospective chart review was performed of the first 45 cases of MIPS at a major academic medical center. Cases were divided into three groups of 15 patients each. Group characteristi

Arterial complications following anterior lumbar interbody fusion: report of eight cases.

The objective of this study is to report eight cases of arterial complication following anterior lumbar interbody fusion (ALIF) and to analyze the data in order to identify possible risk factors. The authors have encountered six cases of common iliac artery occlusion and two cases of acute vasospasm as a complication of ALIF using two different approaches to spine: hypogastric-midline-transperitoneal approach at one center and minimally invasive muscle-sparing retroperitoneal approach at the other. All cases involved surgery at the L4-L5 level. All patients were smokers, and three had an exist

Short-term complications of pubovaginal sling procedure for genuine stress incontinence in women.

OBJECTIVES: To evaluate the complication rate intraoperatively and within the first 30 postoperative days of the pubovaginal sling procedure. METHODS: From January 1992 to September 1996, we prospectively analyzed 90 women with type II and III genuine stress incontinence (age 38 to 84 years, average Valsalva leak point pressure 57.5 cm H2O) who underwent the pubovaginal sling procedure at our institute. Sixty percent of patients had no previous surgical treatment for their incontinence. Thirty-three percent of our patients have significant comorbidity (chronic obstructive lung disease, diabet

Neuraxial Anesthesia in Parturients With Intracranial Arteriovenous Malformations: A Case Report.

A cerebral arteriovenous malformation (AVM) is an abnormal nidus of blood vessels that directly connects the arteries and veins in the brain, bypassing the normal capillary network. Due to the rarity of this condition, specific anesthetic management guidelines for patients with intracranial AVMs are limited. In patients with cerebral AVMs, increased intracranial pressure is a concern in the setting of recent hemorrhage, significant mass effect, or venous congestion. In these types of cases, anesthetic management becomes more complex. The risk of AVM rupture during pregnancy, though rare, is a

Incidence of vertebral artery injury in patients undergoing cervical spine trauma surgery in correlation with surgical approach: A review.

Spinal cord injuries at the cervical spine level represent the most consequential of the related injuries at all levels of the spine. They can trigger permanent unilateral or bilateral damage with conspicuous disability. Regarding unstable injuries, the gold standard approach is open reduction and osteosynthesis, which can select between anterior and posterior surgical access. Each of the aforementioned approaches demonstrates both advantages and disadvantages; thus, it is up to the surgeon to determine the optimal option concerning the patient's safety. Diligent intraoperative control of anat

Managing Antral Wall Discontinuities in Sinus Elevation Surgery.

INTRODUCTION: Schneiderian membrane perforation (SMP), which is usually readily manageable, is the most common intraoperative complication of sinus elevation surgery. Some evidence suggests that SMP is associated with increased risk for postoperative complications, including maxillary sinusitis. Antral wall discontinuity (AWD) is an acquired condition that may increase SMP likelihood and lead to larger, less-manageable perforations. CASE PRESENTATION: A generally healthy patient receiving sinus elevation surgery exhibited an AWD in the palatine process of the maxilla. The osseous defect was i

Anesthetic consideration for neuromuscular diseases.

PURPOSE OF REVIEW: The aim of this review is to examine data relating to perioperative management of the patient with neuromuscular disorders RECENT FINDINGS: Patients with pre-existing neuromuscular disorders are at risk for a number of postoperative complications that are related to anesthetic drugs that are administered intraoperatively. Careful preoperative assessment is necessary to reduce morbidity and mortality. In particular, the risk of postoperative respiratory failure and need for long-term ventilation should be reviewed with patients. The use of succinylcholine should be avoided in

The Prevention of Periprosthetic Joint Infections.

Periprosthetic joint infection (PJI) following total joint arthroplasty (TJA) adversely affects patient quality of life and health status, and places a huge financial burden on the health care. The first step in combating this complication is prevention, which may include implementation of strategies during the preoperative, intraoperative, or postoperative period. Optimization of the patient with appreciation of the modifiable and non-modifiable factors is crucial. Preoperative optimization involves medical optimization of patients with comorbidities such as diabetes, anemia, malnutrition and

Aortic bleeding one week after removal of an intraoperative epicardial temporary pacing wire.

A 56-year-old man had left nephrectomy and resection of a cavoatrial tumor thrombus under a cardiopulmonary bypass assist for left renal cell carcinoma. An intraoperative bipolar temporary epicardial atrial pacing wire was removed on postoperative day 8. The patient collapsed on postoperative day 15. Emergent transthoracic echocardiography and computed tomography scanning with contrast media detected cardiac tamponade. The three-dimensional volume-rendering images from the multislice computed tomography scan demonstrated bleeding from the aortic root. Upon emergency operation, active arterial

Experimental study of the morphological and electromyographic characteristics of the most frequent iatrogenic lesions of the recurrent laryngeal nerves in thyroid surgery.

INTRODUCTION: Iatrogenic lesions of the recurrent laryngeal nerves remain one of the most severe and frequent complications in thyroid surgery. According to Eisele (Laryngoscope 1996;106:443) their incidence varies between 1% and 12% depending on the size of surgery. AIM: The aim of the study was to examine experimentally the type of neural lesion and the efficiency of stimulation electromyography for immediate evaluation of the recurrent laryngeal nerve function in the most common surgical traumas. MATERIAL AND METHODS: Ten clinically healthy and parasite-free male dogs of mixed breed weigh

[Dysphagia as the leading symptom: presentation of a rare course in chronic pancreatitis with formation of pseudocysts].

Pancreatic pseudocysts are a common complication of pancreatitis. Although 20% regress spontaneously, persistent pseudocysts may lead to various complications such as infection, bleeding or compression of adjacent structures or organs. Due to the barrier function of the diaphragm, infection of pancreatic pseudocysts with posterior mediastinal extension represents a rare entity. Because of the rareness and untypical symptoms of such events, a mediastinal extension of an infected pancreatic pseudocyst often ist not recognized. In our case the expansion of the infection to the mediastinum had led

A comparison between loop diathermy conization and cold-knife conization for management of cervical dysplasia associated with unsatisfactory colposcopy.

Eighty-six women with cervical dysplasia and unsatisfactory colposcopy were managed with excisional conization--43 with outpatient loop diathermy conization under local anesthesia and 43 matched controls with cold-knife conization as inpatients under general anesthesia. Both groups were similar in terms of age, parity, and severity of dyskaryosis on initial cytology, treatment success rates, and completeness of excision. However, loop diathermy conization was significantly quicker (2.8 +/- 2.9 min vs 14 +/- 18.6 min) and associated with less intraoperative blood loss (3.3 +/- 2.8 ml vs 79.1 +/

Prolonged Neuromuscular Paralysis Following Succinylcholine Induction Leading to a Trans-Kambin Oblique Lateral Lumbar Interbody Fusion (OLLIF) Procedure Performed Without Neuromonitoring.

Intraoperative neuromonitoring (IONM) is widely used during minimally invasive spine procedures, including trans-Kambin oblique lateral lumbar interbody fusion, to reduce the risk of neural injury. We present a unique case in which prolonged neuromuscular paralysis following succinylcholine induction resulted in the absence of effective neuromonitoring. After careful multidisciplinary consideration of case cancellation and a detailed risk-benefit assessment, the procedure was completed because it involved a single level without complicating anatomical factors. The patient experienced an excell

Epidural Pneumorrhachis Causing Intraoperative Loss of Motor Potentials During Instrumented Fusion for Scoliosis: A Case Report.

CASE: In a patient who underwent a thoracoscopic anterior release combined with a posterior spinal fusion for juvenile idiopathic scoliosis, unilateral loss of neuromonitoring signals was noted during the posterior instrumentation, and epidural pneumorrhachis was identified by intraoperative O-arm imaging. An immediate laminectomy and decompression of epidural fat and air were performed, resulting in return of the neuromonitoring signals. The patient had no clinical motor or neurological deficits postoperatively, and the posterior spinal fusion was completed successfully 3 days later. CONCLUS

Laser-assisted indocyanine green imaging to assess perfusion of scalp closure in an infant.

The current treatment of craniosynostosis is open surgical excision of the prematurely fused suture and cranial vault remodeling. Due to the change in skull morphology and the increase in volume, some tension on the skin flaps is noted with closure. Although complete wound breakdown is rare, it can be a devastating complication. We present our experience with the use of the SPY imaging system (Lifecell Corporation, Branchburg, NJ, USA) to visualize and record blood flow within the flaps of a 1-year-old patient with anterior plagiocephaly. Intraoperative indocyanine green angiography has the po

[Intraoperative three-dimensional imaging to monitor selective decompression in lumbar spinal stenosis].

BACKGROUND: We are looking for a possibility to examine the amount of decompression in operations for spinal stenosis with an Iso-C(3D) fluoroscope (Siemens). METHODS: In ten patients operated on because of spinal stenosis from one side with undercutting, we performed a scan with the Iso-C(3D) fluoroscope during the operation. In five patients we additionally performed intraoperative myelography. RESULTS: In all cases we were able to make useful scans. Two times we changed our surgical procedure because of the scans. There were no complications because of the investigation with the Iso-C(3D)

Intraoperative Healon5 injection into blebs for small conjunctival breaks created during trabeculectomy.

Leakage from a conjunctival break created during trabeculectomy may cause serious complications such as shallow anterior chamber, hypotony, and a flat bleb in the early postoperative period. Therefore, careful handling of the conjunctiva with a nontoothed forceps is essential. However, thin conjunctiva is susceptible to mechanical injury and is difficult to repair with suturing, especially when the point of leakage is close to the scleral flap. In 3 consecutive cases in which a small break in the thin conjunctiva adjacent to the scleral flap was created during trabeculectomy, sodium hyaluronat
