Multidisciplinary Management in Psoriasis: A Structured Approach for Early Comorbidity Detection and Referral.

INTRODUCTION: Psoriasis is a systemic disease with a high burden of underdiagnosed and undertreated comorbidities, partly due to fragmented care. These comorbidities significantly worsen outcomes and quality of life, highlighting the need for structured and coordinated multidisciplinary management to reduce diagnostic delays. METHODS: This project aimed to develop screening algorithms and multidisciplinary referral criteria to promote early detection and coordinated management of psoriatic comorbidities. A three-phase methodology was applied: a literature review, focus groups and a nominal gr

Endocrine disorders after aneurysmal subarachnoid hemorrhage: Acute neuroendocrine responses and long-term sequelae. A narrative review.

Aneurysmal subarachnoid hemorrhage (aSAH) is a life-threatening cerebrovascular event associated with profound neurological and systemic disturbances. Among these, alterations in hypothalamic-pituitary function, water-electrolyte balance, and glucose metabolism are frequently observed but remain difficult to interpret, particularly in the setting of critical illness. Many endocrine abnormalities detected during the acute phase reflect adaptive neuroendocrine stress responses rather than true hormonal failure, whereas persistent deficiencies may emerge during recovery and contribute to long-ter

Severe dengue fever with life-threatening gastrointestinal hemorrhage in a liver transplant recipient undergoing adjuvant chemotherapy for hepatocellular carcinoma.

Dengue fever poses a significant diagnostic and therapeutic challenge in immunocompromised hosts, particularly in solid organ transplant recipients. We report a severe case in a patient during the early post-operative period following liver transplantation for hepatocellular carcinoma, who was concurrently receiving adjuvant chemotherapy with oxaliplatin and fluorouracil. This unique confluence of iatrogenic immunosuppression and chemotherapy-induced myelosuppression resulted in a life-threatening presentation characterized by grade 3 bone marrow suppression, severe anemia, overt gastrointesti

Cardiac Hypertrophy: A Comprehensive Review from Prenatal Life to Young Adulthood.

Myocardial hypertrophy (MH) represents a complex and heterogeneous condition in the pediatric and young adult population. While rare in children, MH encompasses a wide spectrum of physiological and pathological entities, ranging from transient hypertrophy in the infants of diabetic mothers to progressive genetic hypertrophic cardiomyopathies (HCM) with significant morbidity and mortality. Differential diagnosis is critical, as many phenocopies-including metabolic, mitochondrial, and syndromic diseases-can mimic HCM. Echocardiography remains the first-line imaging modality, with cardiac magneti

ECMO in Pregnancy: Analysis of Indications, Management and Outcomes.

Extracorporeal membrane oxygenation (ECMO) has become a critical rescue therapy for pregnant and peripartum patients experiencing life-threatening cardiac or respiratory failure that is refractory to standard treatments. Pregnancy induces unique physiologic changes that influence ECMO initiation, cannulation, and management, necessitating specialized approaches to optimize outcomes for both mother and fetus. Pharmacologic challenges include altered drug kinetics and the risks associated with placental drug transfer, while anticoagulation strategies require a careful balance between thrombosis

Impaired muscle metabolism in the ICU: interrogating the underlying pathophysiology.

PURPOSE OF REVIEW: Accelerated muscle wasting in critically ill patients contributes to poor recovery outcomes. Critical care guidelines recommend delivering higher protein doses; yet, increasing evidence suggests harm from higher protein doses. RECENT FINDINGS: Definitive randomised controlled trials in critically ill adults have reported signals of harm from higher protein administration compared to lower protein doses or standard of care, with significant results pertaining to reduced health-related quality of life and worse outcomes in sub-groups of acute kidney injury and higher illness

Perceptions of physical activity for health promotion among people living with HIV in Uganda: A qualitative study.

BackgroundPromoting health and quality of life for people living with HIV (PLHIV) in low-resource settings is vital. Understanding perceptions of physical activity (PA) and factors influencing participation can guide culturally appropriate interventions. This study explored how PLHIV in Central Uganda perceive PA, factors affecting its acceptability, and views on integrating PA into antiretroviral treatment (ART) services.MethodsA qualitative exploratory design was used. Semi-structured interviews were conducted with 12 adults (7 women, 5 men) on ART for at least 1 year. Data were analysed usi

The impact of social isolation and loneliness on the well-being of carers of a person with dementia in aotearoa New Zealand.

Dementia is a leading cause of disability, and as the population ages, there will be a greater need for friends and family to care for people with Dementia. Unfortunately, informal care for a person with dementia is associated with poor psychological and physical health and lower quality of life of the caregiver. The aim of the present study was to understand how to best support caregivers within their communities by examining their experience of loneliness, isolation, and their relationship with well-being. The study used a representative sample of the New Zealand population in terms of ethni

Nutritional care in older adults: are we doing everything? An expert opinion review.

Malnutrition is a prevalent, yet often underdiagnosed and undertreated, condition in older adults. It is characterized by weight loss and/or reduced muscle mass due to diminished caloric intake, inflammation, and/or disease burden. In return, malnutrition can lead to diminished skeletal muscle functionality and disability, among others. Malnutrition plays a crucial role in the pathogenesis of two prevalent geriatric syndromes, namely sarcopenia and frailty. The complex interplay between malnutrition, sarcopenia, and frailty significantly impacts the older population, leading to increased morbi

Successful treatment of toxic epidermal necrolysis using plasmapheresis: A prospective observational study.

Toxic epidermal necrolysis (TEN) is a rare, severe, life-threatening skin disease and it requires urgent critical care, including admission to the intensive care unit (ICU). It is characterized by fatal sequelae and high mortality. Currently, insufficient evidence exists to support the use of any systemic adjuvant therapy, such as cyclophosphamide, intravenous immunoglobulin (IVIg), or corticosteroids. However, plasmapheresis has been increasingly valued by clinicians due to its significant efficacy and little adverse side effects. To assess the efficacy of such treatment, 28 patients who were

Reshaping policy to deliver holistic care for adolescents with Crohn's disease.

The number of adolescents with Crohn's disease (CD) is rising, with one third of cases diagnosed before turning 21 years old. Evidence shows that long-term medical intervention, school absence and the physical toll of CD on the growing adolescent also have a psychological effect on patients. In addition, poorly defined transition pathways are extending these problems into adulthood. The National Institute for Health and Care Excellence (NICE) guidelines are important in shaping service delivery and distribution. However, analysis of the full 2012 NICE CD management guidelines indicates they fa

Osteoporosis prevention, screening, and treatment: a review.

Osteoporosis, defined as low bone mass leading to increased fracture risk, is a major health problem that affects approximately 10 million Americans. The aging U.S. population is predicted to contribute to as much as a 50% increase in prevalence by 2025. Although common, osteoporosis can be clinically silent, and without prevention and screening, the costs of osteoporotic fracture-related morbidity and mortality will burden the U.S. healthcare system. This is a particularly relevant concern in the context of diminishing health care resources. Dual-energy X-ray absorptiometry is the most widely

[Specialist community social services as a form of community social support].

Art. 9 of the Polish Mental Health Act provides two forms of community-based social support--specialist social help services and community self-help houses for persons who, due a to serious mental illness or severe mental retardation, face considerable difficulties in their daily life, especially with respect to interpersonal relations, employment and welfare matters. The first form could only be implemented after coming into force of the suitable regulation of the Ministry of Labour and Social Policy of December 18, 1996 (Dz. U. z 1997 r., nr. 2, poz. 12). Hence, it may be understood that inf

Social phobia in disabled workers with chronic musculoskeletal pain.

The present study assessed the prevalence and impact of social phobia and other anxiety disorders in disabled workers with chronic musculoskeletal pain. Potential participants were 200 disabled workers consecutively referred to an interdisciplinary tertiary care centre. A two stage screening process was used in which: (a) a self-report battery was given during a pre-admission visit, and (b) pre-planned selection criteria were applied to the self-report instruments to select patients for a structured diagnostic interview. Fifty-four of the 146 patients who provided complete responses on the sel

Augmentation mammaplasty: neither simple nor safe.

Elective aesthetic cosmetic surgery of the female breast appears not to warrant the same attention to informed consent or authorisation as other surgical procedures. Arguments for benefits do not need to be persuasive since the prospective patient is usually 'sold on the idea'. This paper argues that more rather than less information is necessary where the surgery will not achieve what is claimed for it and where there are significant risks of harm. Evidence is presented to show that augmentation mammaplasty does not provide a solution to problems of body image, self-esteem and sexuality; it m

Acute kidney injury due to hemoglobin cast nephropathy secondary to autoimmune hemolytic anemia: a case report.

BACKGROUND: Autoimmune hemolytic anemia (AIHA) is an uncommon acquired disorder that can lead to life-threatening intravascular hemolysis and acute kidney injury (AKI). Hemoglobin cast nephropathy is a rare but severe complication of massive hemolysis, often underrecognized in clinical practice. CASE PRESENTATIONS: We describe a 57-year-old female with no prior chronic kidney disease who presented with nausea, vomiting, oliguria, edema, and rapidly progressive renal dysfunction after a presumed preceding infectious-like episode. Laboratory investigations revealed severe anemia, elevated hemol
