What is the Relationship between the Localization of Maxillary Fungal Balls and Intranasal Anatomic Variations?

OBJECTIVES: Although the mechanisms underlying the initiation and maintenance of inflammation in unilateral maxillary fungal balls (FBs) are poorly understood, the relationship between intranasal anatomy and maxillary FB is thought to play an important role. The aim of this study was to investigate the relationship between anatomic variations and FB. METHODS: We enrolled 140 patients who were composed of 56 patients with FB, 56 patients with unilateral chronic rhinosinusitis (CRS), and 28 patients with no sinus disease. Computed tomography scans were retrospectively analyzed to identify and c

Evaluation of the microbiology of chronic maxillary sinusitis.

Chronic paranasal sinusitis is a disease that afflicts a significant percentage of the population and causes considerable long-term morbidity. With the common use of multiple courses of broad-spectrum oral antibiotics to treat this condition, there is a possibility of an alteration in the pathogens that promote a persistence of chronic sinusitis. One hundred seventy-four consecutive patients with a diagnosis of chronic maxillary sinusitis requiring an endoscopic surgical procedure were prospectively evaluated. At the time of surgery, cultures of mucopus in the maxillary sinus were aseptically …

Intrasinus versus extrasinus zygomatic implant placement: A 5-year retrospective observational analysis of survival and complications.

BACKGROUND: Edentulism, a common condition, may be congenital or acquired later in life as a result of dental caries, periodontitis, aging, craniofacial trauma, or tumor resection. The rehabilitation of missing teeth can be achieved with dental implants. In cases of insufficient alveolar bone, several treatment options have been described in the literature, including maxillary sinus augmentation with bone grafting, the Le Fort I osteotomy, the angulated placement of conventional implants, and the use of specialized implants, such as zygomatic implants, which are anchored in the zygomatic bone.…

A new CBCT-based classification of posterior extramural ethmoid cells.

PURPOSE: The study aimed to evaluate the prevalence of extramural expansion of posterior ethmoid cells using Cone beam computed tomography ( … nd into the sphenoid sinus) and posterior ethmomaxillary cells (characterized by extension into the maxillary sinus). Among sphenoethmoidal cells, the following subtypes were identified: Onodi cells (PE cell extension toward the optic canal), inferolateral sphenoethmoidal cells (inferolateral expansion of PE cells relative to the sphenoid sinus without reaching t …

Examination and Relationship of Posterior Superior Alveolar Artery and Canalis Sinuosus Using Cone Beam CT.

In this study, we investigated the anatomical location, dimensions, and relationships of the posterior superior alveolar artery (PSAA) and canalis sinuosus (CS) within the maxilla, aiming to enhance the safety and efficacy of surgical procedures. A retrospective analysis was performed on 323 individual cone beam computed tomography scans. The diameter of the PSAA and CS, the distance of the PSAA from the sinus floor, the distance of the PSAA and CS from the alveolar crest, the distance of the PSAA and CS from the nasal septum, and the distance from CS to the nasal cavity floor were measured. Learning Curve of a Minimally Invasive Technique for Transcrestal Sinus Floor Elevation: A Split-Group Analysis in a Prospective Case Series With Multiple Clinicians.

AIMS: To assess the learning curve of a minimally invasive procedure for maxillary sinus floor elevation with a transcrestal approach (tSFE) and evaluate the influence of clinician's experience in implant surgery on its outcomes. MATERIALS AND METHODS: Patients were treated by clinicians with different levels of experience in implant surgery and inexperienced with respect to the investigated tSFE technique. The initial (n = 13) and final (n = 13) groups treated by the expert clinician were compared for tSFE outcomes. Additionally, the high, moderate, and low groups (n = 20 each) treated by th…

OBJECTIVE: To determine the influence of allergic rhinitis (AR) on the expression of matrix metalloproteinase (MMP)-9, MMP-2, and tissue inhibitor-1 of metalloproteinase (TIMP-1) in nasal polyposis. STUDY DESIGN: A case-control study. SETTING: A tertiary referral center. SUBJECTS AND METHODS: The expression of MMP-9, MMP-2, and TIMP-1 was investigated in the nasal polyp tissue (NP) and maxillary sinus mucosa (MM) samples from 20 AR patients and 20 nonallergic patients undergoing endoscopic sinus surgery. Trichrome, periodic acid-Schiff, and hematoxylin-eosin staining were also performed and…

Congenital Upper Lip Sinus.

Congenital upper lip sinus (CULS) is an extremely rare anomaly. The aim of the study is to present a new case and compile the present knowledge of CULS to help foster a deeper understanding of this disease. A 9-year-old girl with a midline CULS, a prolonged maxillary labial frenulum, and a palatal epulis is presented. Thirty-nine papers met the eligibility criteria and 45 cases were included in the systematic review. CULS shows typical clinical and pathologic characteristics. It tends to occur at the midline region horizontally (n=35; 78%) and the upper part of the white lip (n=23; 51%) vertic

SMARCB1 deficient sinonasal carcinoma: An emerging entity with a diagnostic challenge.

SMARCB1 deficient sinonasal carcinomas are rare neoplasms, classified under sinonasal undifferentiated carcinomas by the fourth edition of the World Health Organization (WHO) classification of head and neck tumors. It is characterized immunohistochemically by loss of SMARCB1(INI1) expression. We are reporting the case of a 63-year-old man who was evaluated for nasal stuffiness of 3 months duration in another hospital where a radiological evaluation showed a polypoidal soft tissue lesion in the right maxillary sinus extending to the right nasal cavity and spheno-ethmoidal sinus. He underwent ex…

Severe Pneumonia and Sepsis Caused by Dialister pneumosintes in an Adolescent.

BACKGROUND: Dialister pneumosintes (D. pneumosintes) is known to cause dental, periodontal or sinus infections. To date, the pathogen has only been described in a small number of cases with a severe infection. CASE REPORT: We describe the clinical case of a 13-year-old, obese female patient that presented with acute respiratory failure and sepsis. A CT-scan showed extensive bilateral patchy areas, subpleural and peribronchovascular consolidations with surrounding ground-glass opacity, extensive consolidations in the lower lobes of both lungs matching to a severe pneumonia and clinically emerg

Management of chronic sinusitis in cystic fibrosis.

Chronic rhinosinusitis is extremely common in patients with cystic fibrosis. It causes numerous problems in these patients and can put them at risk for life-threatening illness. Potential problems include nasal obstruction, congestion, sinus pain and pressure, infection (usually with Pseudomonas organisms), hyposmia or anosmia, and the seeding of bacteria into the lower respiratory tract. Cystic fibrosis patients with chronically infected sinuses are at increased risk for pneumonia following lung transplantation. A prophylactic protocol has been developed for the management of chronic sinusiti

Regenerative Endodontic Treatment in Permanent Incisors: Two Case Reports with 6 Years of Follow-Up.

BACKGROUND: Regenerative endodontic treatment (RET) has emerged as a biologically based alternative to traditional apexification for managing immature permanent teeth with pulp necrosis. By promoting tissue ingrowth and continued root development, RET aims not only to eliminate infection but also to reinforce structurally compromised roots. Although its clinical use has expanded, evidence regarding the long-term predictability and durability of RET remains limited, as most published studies provide only short- or mid-term follow-up. CASE PRESENTATION: This report describes two pediatric cases
