Neurologic complications of immunization.

In the United States and many other developed countries, active immunization of children has virtually eliminated poliomyelitis, measles, rubella, tetanus, and other diseases, such as disease due to Haemophilus influenzae type b. Individual vaccines can produce systemic or neurologic reactions ranging from minor events, such as pain and erythema at the injection site, to major complications, such as seizures, shock, encephalopathy, or death. Immunization programs have also generated considerable controversy, as witnessed by recent concerns regarding the relationship between vaccines or their c…

[Bilateral retinitis: early manifestation of subacute sclerosing panencephalitis. A case report].

Subacute sclerosing panencephalitis (SSPE) is a progressive encephalitis caused by persistent measles infection. The disease is almost always fatal and mainly affects children and young adults. Ophthalmological signs are frequently observed and can be the presenting manifestations. We report the case of a 22-year-old young man who presented with bilateral posterior retinal necrosis confirmed by fluorescein angiography and normal laboratory tests. The appearance of peripheral retinal tears and stereotyped myoclonic movements were signs of the evolving condition. Diagnosis of SSPE was confirmed …

[Secondary effects of vaccinations].

The adverse effects of vaccines include local reactions and systemic symptoms or illnesses. Local reactions are frequent, most often presenting as transient pain, redness, edema and/or nodule. Fever of short duration is the main systemic symptom, generally occurring within 24-48 hours following vaccination. Some vaccines have recognized specific adverse effects such as thrombocytopenic purpura for the measles-mumps-rubella vaccine, and febrile convulsions for the pertussis vaccine. Hepatitis B vaccine and Haemophilus influenzae type b vaccine have been respectively suspected to be responsible …

Immunizations: recommendations and resources for active patients.

Although childhood vaccination rates are at an all-time high, those for adolescents and adults are suboptimal. All adolescents and adults should be immunized against measles, mumps, rubella, varicella, tetanus, and diphtheria, and many should also receive hepatitis A, hepatitis B, influenza, and pneumococcal vaccines. In addition, active patients who engage in outdoor activities may benefit from vaccination against Lyme and meningococcal disease. Regular, strenuous exercise and foreign travel may increase the risk of some infectious diseases. Athletes often see a physician only for sports phys…

[Mumps in 1997].

In 1997 83,558 cases of mumps were reported and attack rate of 216.3/100,000 was two times higher than in the previous year. Four percent of cases were admitted to hospitals (3,327) and the percentage of hospitalised children ranged from 0.3% in Chełm voivodeship to 12.9% in Konin voivodeship. Mostly affected were children in the age group 5 to 9 years old in which attack rates ranged from 1,602 to 2,293 per 100,000. Patients in this age group constituted 59% of total number of mumps. MMR vaccine is still not included into the national programme of immunization; instead monovalent measles vacc…

[Strategies to control vitamin A deficiency].

Vitamin A deficiency is a major public health problem in the countries of the Sahel. It causes xerophthalmia and high rates of child mortality and it occurs mostly in underdeveloped regions. People of all ages may suffer from vitamin A deficiency but it is a particular problem in pre-school-age children. Each year, about 250,000 children throughout the world become blind due to vitamin A deficiency. Measles, pneumonia and diarrhea reduce the child's reserves of retinol and increase the dietary requirement for vitamin A. Improvement of social conditions is a radical approach to preventing vitam…

[Epidemiologic surveillance in the Army and Public Health: as exemplified by rubella].

Since 1993, several outbreaks of rubella are annually reported from the French military. This trend reveals a transfer of the reservoir of wild virus towards a non-immune population of male adolescents and young adults, as a result of the rubella control programmes. This paradoxical phenomenon should disappear within the next decade, when the new vaccinal schedule including a combined measles, mumps and rubella vaccine for the children of both sexes 11 to 13 years old will have been implemented. Meanwhile, the military system of weekly epidemiological report will be usable as a national observ…

Selected pediatric viral infections.

Viral infections of the central nervous system in infants and children are uncommon but potentially serious illnesses. Common causes have included the enteroviruses (particularly polioviruses and coxsackieviruses), herpes viruses (type 1 and type 2 herpes simplex, varicella, and cytomegalovirus), arboviruses, rubella, mumps, measles (including subacute sclerosing panencephalitis), and human immunodeficiency virus. Several of these viruses, such as cytomegalovirus, herpes simplex, and vertically transmitted human immunodeficiency virus, may cause congenital infections. Others are acquired later…

[The epidemic process in infections before and after active immunization. I. Comparison of long-term monitoring of the disease with a simple mathematical model].

In the first part of this paper the authors used for demographic data and morbidity data concerning measles, parotitis, rubella and chickenpox in the Sokolov district a model of exponential distribution, in particular its distribution function. The authors introduced the concept of mean population year. They tested the theoretical assumption that in different infections in long-term average the annual number of infected subjects is equal and equals the mean population year. They found that the delta parameter which determines the shape of the exponential regression curve and is the measure of …

Vaccination campaigns for common childhood diseases.

Mathematical models exist for vaccination programs for common childhood diseases such as measles, chickenpox, polio, rubella, and mumps. In compartmental models, the population is divided into compartments containing susceptible, infected, and immune individuals, and the model also takes account of the age structure in the population. An equilibrium analysis is performed on the resulting partial differential equations to determine the vaccination program that just eliminates the disease. The stability of the resulting equilibrium solutions is also examined. Particular attention is paid to mult…

A computer network for the surveillance of communicable diseases: the French experiment.

The description and first results of the French Communicable diseases Network are reported. The network, initiated in November 1984, currently includes the National Department of Health, the local health offices and various clinical, biological, and epidemiological partners. Surveillance of influenza, viral hepatitis, acute urethritis, measles, and mumps is based upon reports from sentinel general practitioners throughout France who are equipped with terminals and can communicate their data on a 24-hour basis. The network distributes electronic bulletins summarizing the surveillance data, the …

[Retrospective study on the mortality of children under 5 in a rural district of the region of Brazzaville (People's Republic of Congo). I. Rate and causes of mortality].

A retrospective study of 1,003 children permits us to identify precisely rates and causes of mortality in children under 5 years old in a rural holoendemic malaria area of the People's Republic of the Congo. The mortality rates are distinctly lower than those generally observed in tropical Africa. Infant mortality (0-1 year) was found to be 71%, mortality 13%, early neonatal mortality (0-7 days) 41% and mortality between 1-5 years, 49%. Main causes of mortality are those related to pregnancy (prematurity, obstetrical pathology) and with infectious diseases particularly measles. On the other ha…

Persistent or slow viral infections and related diseases.

The discovery of persistent transmissible agents by veterinarians has led to striking advances in the infectious cause of neuropathies of human beings. There is evidence for persisting infection in congenital rubella and the herpes group of viruses including cytomegalovirus infections. Hepatitis types A and B are candidates for inclusion in the category of persisting viral infections. The rubeola or measles virus is established as a persistent virus which causes elevated antibodies in the serum and cerebrospinal fluid of many patients with severe demyelinating disease such as subacute sclerosi…

[New assumptions in the concept of vaccine prophylaxis of infantile infections].

An essential reduction in the incidence rate of six infantile infections within the Expanded Immunization Program was accompanied by occurrence of new problems in vaccine prophylaxis. 1. Vaccine-dependence--the reoccurrence of vaccine-prevented infections in those cases when vaccination was stopped. 2. A choice of strategy in the immune prophylaxis of poliomyelitis during the post-liquidation period bearing in mind the elimination of vaccine strains from the human population and a possible global refusal from vaccination. 3. Elaboration of regional programs for eliminating the measles in big t…

Toxic epidermal necrolysis following morbilli-parotitis-rubella vaccination.

We present the first reported case of toxic epidermal necrolysis (TEN) caused by morbilli-parotitis-rubella (MPR) vaccine. A 13-year-old girl developed TEN 7 days after she received live, attenuated, triple MPR vaccine. The history of drug intake and any illness was negative. At admission the patient was acutely ill with high fever. The whole body was erythematous. The epidermis was wrinkled and the Nikolsky sign was positive. Numerous erosions were present on the lips and genital region. On the seventh day of illness, the eruption involved 80% of the skin. Systemic corticosteroid therapy was
