Mycobacterium tuberculosis infection in renal transplant recipients is associated with significant morbidity and mortality. Genitourinary tuberculosis is a less frequent presentation and a high level of suspicion is needed to avoid treatment delay. This report describes the clinical presentation, diagnosis, and medical management of a 2-year-old spayed female dwarf rabbit (Oryctolagus cuniculus) suffering from an ulcerative keratitis and a stromal abscess with a rapid evolution. The diagnosis of fungal keratitis was based on fungal culture identifying the pathogen as Aspergillus fumigatus. Treatment by topical terbinafine was efficacious in resolving the keratomycosis within 8 weeks. To our knowledge, this is the first reported case of spontaneous keratomycosis in a rabbit and the first description of successful therapy by terbinafine.

We report a case of a 65-year-old female diagnosed with sever dengue fever. She started showing recovery from dengue fever with medical management. On day 6 of admission, she had leukocytosis, altered mental sensorium, and hemoptysis. Chest tomography showed air space consolidation with multiple nodules in the left upper and middle lobe sputum and bronchoalveolar lavage cultures were positive for Aspergillus flavus. The patient showed improvement with voriconazole and therapy was continued for 6 weeks.

Postural orthostatic tachycardia syndrome (POTS) refers to the presence of orthostatic intolerance with a heart rate (HR) increment of 30 beats per minute (bpm) or an absolute HR of 120 bpm or more. Hair loss has a high prevalence in the general population and can have significant medical and psychological sequelae. Pattern hair loss and alopecia areata represent the major reasons patients present to dermatologists in relation to hair loss. Because conventional treatment options are generally incompletely effective, novel methods for hair grown induction are being developed. The role of using electromagnetic radiation, including low-level laser therapy for the management of hair loss through phototrichogenesis, is reviewed in this article.

Otophyma is the term used for sebaceous gland hypertrophy and hyperplasia of the ear. It is usually the end stage of rosacea--a common, chronic, cutaneous disorder of unknown etiology. We hereby present a rare case of otophyma with its clinical features, its etiology, diagnosis, and review of medical and surgical therapy. This patient remained undiagnosed and mismanaged for many years before she was finally diagnosed with this rare condition and successfully managed by surgical technique. To our knowledge, this is the second case of otophyma presented in the otolaryngological literature.

Sodium hypochlorite (NaOCl) is a well documented and widely accepted intracanal irrigant used to disinfect the pulp space in endodontic therapy. NaOCl can be an extremely cytotoxic material; therefore, care must be taken with its use. The authors present a case of periradicular extravasation of NaOCl with sequelae of pain, swelling, ecchymosis and parasthesia consistent with a chemical burn. The patient was treated with a nonsurgical medical management approach. She experienced a complete resolution of signs and symptoms, aside from a persistent residual focal parasthesia.

Menorrhagia, also known as heavy menstrual bleeding, is defined as a menstrual blood loss is greater than 80 mL. The objective measurement of menorrhagia is not easy in clinical practice, but often, the subjective complaint correlates well with the measured loss. Four types of treatments - hysterectomy, endometrial resection or ablation, levonorgestrel-releasing intrauterine system (LNG-IUS) and oral medical therapy, are available for management of menorrhagia. The article reviews the efficacy of LNG-IUS on menorrhagia and compares it to other treatment methods.

Acute purulent pericarditis is a rare entity in the neonatal age group. The most common isolated organisms are Staphylococcus aureus, Haemophilus influenzae, and Streptococcus pneumoniae. Other organisms, like Pseudomonas aeruginosa, have been seldom implicated with only one case of Pseudomonas pericarditis reported in the neonatal period. The prognosis is often considered very poor in this age group. This article describes Pseudomonas pericarditis in a 1-week-old immunocompetent female newborn who was successfully managed with combined medical and surgical therapy.

Mycobacterium tuberculosis is a rare cause of mastoiditis, but diagnosis is often delayed, with potentially serious results. We present the case of a 7-year-old child who failed to improve even once the diagnosis was made and appropriate medical treatment initiated. At mastoidectomy, a bony sequestrum was found which had not been evident on CT scanning. We review the diagnosis and management of this condition and suggest that failure to respond to drug therapy even in the absence of demonstrable complications be added to the list of indications for surgical intervention.

We describe a case of bilateral vocal process lesions in a 65-year-old man. His history was strongly suggestive of vocal process granulomas: previous gastroesophageal reflux, intubation, smoking, and oral squamous cell carcinoma. Medical management with a proton-pump inhibitor, reflux precautions, voice therapy, and adequate hydration yielded no results. Subsequent surgical intervention revealed that he had squamous papillomas. We also provide a brief review of vocal process granulomas and squamous papillomas.

Epidural lipomatosis (EL) is characterized by abnormal accumulation of unencapsulated fat in the epidural space, and usually occurs as a complication of longterm steroid therapy. This condition, which may result in devastating neurologic complications, has also been reported without exogenous steroid intake. We describe a case of nonsteroid induced symptomatic EL associated with obesity, and emphasize the possibility of effective medical management of this entity with weight reduction instead of decompressive laminectomy whenever neurologic symptoms are mild and stable.

The management of chronic wounds remains a challenge for various medical and surgical specialties. General strategies include local wound care, surgery, skin grafting, debridement, and vascular reconstruction. However, many wounds remain resistant to conventional therapies. Because platelet factors are important in the normal mechanism of wound healing, we report on the topical application of platelet-derived growth factor. Our initial results suggest that the topical application may stimulate and promote healing of chronic wounds.

Management of atrial fibrillation includes assessing the need for rate control, identifying underlying conditions, and performing cardioversion or instituting long-term medical therapy. Elective cardioversion should be strongly considered for every patient, chiefly to decrease the incidence of embolic stroke. Patients who remain in chronic atrial fibrillation require attention to rate control; digoxin (Lanoxin) alone may be a poor choice if they are vigorous and active. Many subgroups of patients benefit from long-term anticoagulation.

The diagnosis of Mallory-Weiss syndrome was made in 142 patients with upper gastrointestinal haemorrhage by early endoscopy. These patients represent 5.16% of all patients with haemorrhage treated in our unit over a period of ten years. Emergency surgery was needed in 13 due to persistent haemorrhage. In the remaining 129, conservative management sufficed to stop the bleeding. All patients receiving medical therapy survived while one of the surgically treated patients died of pulmonary thromboembolism in the early postoperative period.

A 16-years-old male with a history of Job's syndrome and a recent diagnosis of Crohn's disease was admitted to the hospital because of abdominal pain, nonbloody diarrhea, weakness, and fever. Due to failure to respond to medical therapy it was decided that an operation was indicated, and resection of the terminal ileum and right colon was performed. Cecum and ileum were inflamed, and pathologic studies revealed the presence of budding yeasts which on special stain were diagnostic of Histoplasma species. Successful management was accomplished with oral ketoconazole.
