Metastatic malignant thymoma developed in a 64-year-old man after a 9-year history of myasthenia gravis. Following good response to irradiation of the mediastinal mass and osseous metastasis, pure red cell aplasia developed.

The authors report on a personal series of 27 patients with pulmonary metastases; seventeen were operated, 3 for cure. Nine hundred and eighty six operated cases from the litterature are then analysed yielding the following notions. Where the primary tumor is controlled at the time of metastatic resection, when there is no systemic or mediastinal dissemination, and when the growth of metastases was slow, these secondary resection of lung metastases were followed by a 16 to 63% 5- year survival rate. The authors conclude it is necessary to control the hypothesis that 5 to 10% of operated cancer

The authors report a case of granulomatous hepatitis after treatment by B.C.G. in a patient with malignant melanoma. The patient was treated for one year by an association of chemotherapy and lyophilized B.C.G. (Pasteur Institute). Aside from the rarity of the localization the importance of this observation resides in the discovery of B.C.G. after auramine staining in the liver suggesting that the hepatic lesions were linked to the in situ presence of mycobacteria.

The syndrome of inappropriate secretion of antidiuretic hormone was recognized in a sixty-year-old man with a poorly differentiated metastatic adenocarcinoma of the prostate gland. Elevated levels of antidiuretic hormone were found in the patient's serum and in the prostatic tumor but not in the cerebrospinal fluid. The patient's clinical course is detailed, and the pathophysiology of this syndrome is discussed.

Obstruction of the extrahepatic bile ducts by primary or metastatic carcinoma can sometimes be diffiult to relieve. Frequently there is an inadequate length of normal duct proximal to the obstruction to permit an anastomosis. A by-pass anastomosis between a dilated right hepatic biliary radicle and the gallbladder is a simple and satisfactory method of relieving the jaundice. This can be accomplished by using a T-tube as a stent; a vein graft over the T-tube limb in the liver seems to improve the result.
