Duplication of the foramen rotundum: A rare case report.

Variations at the skull base can result in misinterpretation of radiological imaging and occasionally, iatrogenic injury. Here, we describe, … oid approaches to the middle cranial fossa as well as various transfacial treatments for trigeminal neuralgia which rely on observing the foramina around the foramen ovale on fluoroscopy for correct positioning of needles and catheters.

Stevens-Johnson Syndrome following Failure of Genetic Screening prior to Carbamazepine Prescription.

Failure to screen susceptible individuals for human leucocyte allele B∗1502 leads to the onset of Stevens-Johnson syndrome (SJS). We report a case of a 27-year-old Malay female who was treated with carbamazepine following the diagnosis of trigeminal neuralgia without a genetic screening. She was prescribed 150 mg of carbamazepine initially and the dose was increased to 300 mg following the initial dose. A sudden development of skin and mucous membrane ulcers was observed and this warranted immediate hospitalization. A diagnosis of SJS was made and she was treated immediately with intravenous c…

Posttraumatic saphenous neuroma after open tibial fracture.

Neuralgia and neuroma secondary to iatrogenic saphenous nerve injury have been described in the setting of orthopedic surgical interventions. However, postoperative neuropathic pain caused by saphenous neuroma in the setting of orthopedic trauma has not been reported. We present a case of a 43-year-old woman with a symptomatic saphenous neuroma after an open, laterally angulated tibial fracture. The diagnosis was confirmed in the clinical setting, and treatment consisted of neurolysis and partial neurectomy, resulting in complete pain relief and functional recovery. This case demonstrates the …

Parry-romberg syndrome: a linear variant of scleroderma with discoid lupus erythematosus on scalp - an association.

Parry-Romberg syndrome is a rare neurocutaneous syndrome characterized by progressive shrinkage and degeneration of the tissues beneath the skin, usually on one side of the face. It is associated with neurological disorders such as trigeminal neuralgia, facial paresthesia, headache, and focal epilepsy. Concomitant occurrence of discoid lupus erythematosus (DLE) and morphea in the same skin lesion is exceptional, defined as overlap syndrome with two or more different connective tissue disease concurrently or consecutively. A 32-year-old female developed DLE on a long-standing lesion of sclerode…

Complete response to steroids in dural inflammatory pseudotumor associated with Still's disease.

We report a unique case of a dural-based inflammatory pseudotumor (IPT) arising in the left cavernous sinus of a patient with a history of juvenile Still's disease. The patient presented with hemi-facial paresthesias, dull, constant headaches, and transient episodes of sharp pain along the temporalis region. Treatment with oral steroid therapy resulted in complete regression of the lesion and accompanied neuralgia symptoms. Because intracranial IPT can mimic meningiomas both clinically and radiographically and can be associated with systemic arthritic diseases, neurosurgeons should be familiar…

Herpes zoster in older adults.

Herpes zoster (HZ) is a common condition among older adults, manifested by pain and the classic presentation of a unilateral rash that follows a dermatomal distribution and does not cross the midline of the body. It is caused by reactivation of the virus that caused chickenpox during an earlier infection. In many cases, acute HZ is followed by a severe and disabling complication known as postherpetic neuralgia (PHN), characterized by pain that persists for months or even years after the HZ rash heals. Using an individual example, this article provides information on the clinical manifestations…

Cavernous haemangioma in the interpeduncular cistern: case report and review of literature.

A rare case of a cavernous haemangioma in the interpeduncular cistern is reported. The patient, forty-five year old male presented with excruciating left sided trigeminal neuralgia and diplopia for the past one year. Examination revealed left third and fifth nerve paresis. Magnetic resonance imaging showed a well-defined, lobulated tumour in the interpeduncular cistern. The tumour was totally excised through a subtemporal route. Histology of the tumour revealed a cavernous haemangioma. Extracerebral location for a cavernous haemangioma is rare. An interpeduncular cavernous haemangioma has neve…

Unusual presentations of middle fossa encephaloceles: report of two cases.

Basal encephaloceles are rare occurrences with occult and often varied presentations. Frequently, the need for surgical treatment is not clear to the patient or the physician, leading to potentially fatal complications. We report the case of a 44-year-old woman with an 8-year history of nonspecific complaints who presented to us with trigeminal neuralgia. Also, an unsual case is presented of a 60-year-old man with bilateral middle fossa encephaloceles who suffered acutely from widespread pneumocephalus acting as a mess lesion. Both are cases of nontraomatic middle fossa encephaloceles, which a…

[Neurovascular compression as a reason for neurogenic hypertension].

30 years ago Janetta proposed neurovascular compression as an etiological factor of trigeminal neuralgia. Neuroanatomical, neurophysiological studies and observations during neurosurgical operations seem to confirm Janetta's suggestion. The conception of neurovascular compression is accepted also as an etiological factor in hemifacial spasm. In literature a neurovascular compression is suspected as an etiological factor in many diseases of unknown etiology. The results of neurovascular decompression performed as initial treatment of these diseases were presented. The methods of contemporary di…

Chronic paroxysmal hemicrania-tic syndrome.

We report the coexistence of both chronic paroxysmal hemicrania (CPH) and trigeminal neuralgia (tic douloureux) in a female patient. The clinical features combined to make a configuration of CPH-tic syndrome. The two components of the syndrome appeared synchronously in the same orbital region--first branch of the trigeminal nerve--with a latency of several years after the onset of isolated tic attacks of the second and third trigeminal divisions. The concurrence of both types of pain in the same symptomatic area may have some significance for pathogenic, clinical, and pharmacological aspects o…

Amyloidoma of the gasserian ganglion.

We report an unusual case of an amyloidoma of the gasserian ganglion associated with trigeminal neuralgia. MR imaging showed a mass in Meckel's cave, which was isointense with surrounding tissue on T1-weighted images and hypointense on T2-weighted images. After contrast administration, the mass enhanced homogeneously, and thin cuts through the region showed involvement in the expected location of the gasserian ganglion and, more laterally and inferiorly, in the proximal part of V1. A review of the literature revealed that only one similar case has been reported since the advent of modern neuro…

Upper lumbar disk herniations.

Specific features of upper lumbar disk herniations are reviewed based on data from the literature and from a retrospective study of 24 cases treated surgically between 1982 and 1994 (seven at L1-L2 and 17 at L2-L3). Clinical manifestations are polymorphic, misleading (abdominogenital pain suggestive of a visceral or psychogenic condition, meralgia paresthetica, isolated sciatica; femoral neuralgia is uncommon) and sometimes severe (five cases of cauda equina syndrome in our study group). The diagnostic usefulness of imaging studies (radiography, myelography, computed tomography, magnetic reson…

Results of reoperation for failed microvascular decompression.

Among 64 patients with hemifacial spasm (HFS) and 60 with trigeminal neuralgia (TN) treated by microvascular decompression (MVD), repeated MVD performed on 3 cases with HFS resulted in the absence of spasm in all cases. In 7 cases with TN, this technique resulted in complete remission in 2, recurrence in 3, and no pain relief in 2 cases. MVD was more effective on HFS than on TN in repeated procedures as well as for initial treatment. The cause of recurrence of HFS was attributed to the inadequate cushion effect of muscle as a prosthesis, while that for TN was suspected to be related more to po…

Pressure monitoring inside Meckel's cave during percutaneous microcompression of gasserian ganglion.

During percutaneous microcompression of the gasserian ganglion for the relief of trigeminal neuralgia, a computerized technique for monitoring the pressure inside Meckel's cave was employed in 22 patients. A dedicated transducer connected to a computer records the balloon inflation pressure. Its variations are discernible within tenths of a bar and are plotted in relation to time. The intraoperative pressure inside Meckel's cave is from 0.9 to 2.4 bars. When pressure was low, there was recurrence of pain. The highest values of pressure (1.9-2.4 bars) were observed in most of the patients suffe…

Relation between sensory disturbance and outcome after retrogasserian glycerol rhizotomy.

The relation between postoperative sensory deterioration and surgical outcome in 54 patients treated by retrogasserian glycerol rhizotomy for trigeminal neuralgia was studied. The facial sensibility was assessed one day and three months postoperatively. Thresholds for perception and pain were determined quantitatively using transcutaneous electrical stimulation and clinically by light touch and pinprick tests. At a follow-up one year after surgery there was no significant difference in pain relief between patients who did show and patients who did not show sensory deterioration at the one day …

Evoked taste thresholds in a normal population and the application of electrogustometry to trigeminal nerve disease.

No standardised method for taste threshold measurement is available and therefore comparison between clinical studies is difficult. An electrogustometer was evaluated in normal subjects. No sex difference in taste threshold was noted; however, there was a significant elevation in detection threshold with age and smoking. Electrogustometric values both in patients before and after surgery for trigeminal neuralgia and in patients with trigeminal sensory neuropathy were determined. Many patients with trigeminal nerve disorders had abnormal electrogustometric detection thresholds suggesting that t…

Complications of herpes zoster ophthalmicus.

Of 86 patients with herpes zoster ophthalmicus seen at the Mayo Clinic, Rochester, Minn, from 1975 to 1980, 61 had some form of ocular involvement. Corneal disease was seen in 47, uveitis in 37, postherpetic neuralgia in 15, scleritis in three, and ocular motor palsies in three. No case of optic nerve or retinal involvement was found. Of serious concern were four patients with neurologic complications, including two with contralateral hemiplegia and two with segmental cerebral arteritis. Because the neurologic complications occur several months after the episode of herpes zoster ophthalmicus, …

Investigation of a novel 2.5 MV sintered diamond target beam for intracranial linac-based stereotactic treatments.

Purpose. This work investigates the small-field dosimetric characteristics of a 2.5 MV sintered diamond target beam and its feasibility for use in linac-based intracranial stereotactic treatments. Due to the increased proportion of low energy photons in the low-Z beam, it was hypothesized that this novel beam would provide sharper dose fall-off compared to the 6 MV beam owing to the reduced energy, and therefore range, of secondary electrons.Methods. Stereotactic treatments of ocular melanoma and trigeminal neuralgia were simulated for 2.5 MV low-Z and 6 MV beams using Monte Carlo to calculate…
