[Occult spinal dysraphia].

One of the most complex and most difficult congenital anomalies is spina bifida. Peter Van Forest was the first one who noticed this anomaly in 1587, and Recklinghausen, in 1886, classified spina bifida to types and suggested surgical procedures for its management. Earlier name, spina bifida, is currently more and more replacing with a term "defect of neural tube" (NTDs), or even more, "spinal dysraphia". Anomaly can appear at any level of spinal cord (cervical, thoracal, lumbar and sacral) and posterior localization is more often than the anterior one. Contrary to the open spinal dysraphism t

Pathophysiology of Parkinson's disease: from clinical neurology to basic neuroscience and back.

Parkinson's disease (PD) is characterized by motor and nonmotor (cognitive and limbic) deficits. The motor signs of PD include hypokinetic s … sms and by other modulator neurotransmitter systems entrain the abnormal synchronized oscillations. Neurosurgical procedures, such as lesions or high-frequency stimulation of different parts of the loop, might resume the normal unsynchronized activity of the basal ganglia circuitry, and, therefore, ameliorate the clinical symptoms of Parkinson's disease.

Studies on cyclic AMP in different compartments of cerebrospinal fluid.

Adenosine 3', 5'-monophosphate (cAMP) was measured in the CSF of 42 patients undergoing radiological investigation, neurosurgical procedures, or investigation of hepatic coma. The concentration of cAMP was significantly higher in ventricular CSF than in lumbar CSF. Premedication with pentobarbitone plus promethazine increased cAMP in lumbar CSF. There was no difference in cAMP concentration in lumbar CSF obtained before or after injection of air or after the administration of diazepam during lumbar pneumoencephalography. Lumbar CSF cAMP concentration was significantly increased in patients in …

Association between perioperative factors and the occurrence of postoperative cerebellar mutism syndrome in children.

PURPOSE: Postoperative cerebellar mutism syndrome (pCMS) is a complex condition involving neurological impairment including mutism, hypotonia and dysphagia, that occurs after resection of infratentorial lesions predominantly in children. While the literature has reported several preoperative risk factors, there is little knowledge on the surgeon's role and perioperative factors. Thus, we aimed to examine the association between perioperative features and pCMS. METHODS: This single-center study included 83 patients who underwent microsurgical resection of a cerebellar lesion in 2013-2022. Cont

Clinical utility of maximal and minimal intensity projections in T2-weighted MRI for neurosurgical planning.

BACKGROUND: T2- and FLAIR-weighted MRI are standard modalities for visualizing brain parenchyma in neurosurgical practice but often fail to adequately depict critical structures such as sulci, ventricles, and vessels. We aimed to address this limitation by applying maximal intensity projection (MIP) and minimal intensity projection (MinIP) techniques to conventional T2-weighted MRI (T2-MIP and T2-MinIP, respectively) to improve anatomical understanding. METHODS: We retrospectively reviewed patients with gliomas imaged between 2023 and 2024 who underwent three-dimensional T2-weighted imaging (

Combining 3D printing and elastographic characterization: A novel sphenoid wing meningioma simulation model for neurosurgical training.

High-fidelity simulation models are crucial for advancing neurosurgical training, particularly for complex skull base pathologies such as sphenoid wing meningiomas. This study introduces and validates a novel, cost-effective 3D-printed simulator specifically designed for sphenoid wing meningioma resection. A key innovation of this model is the integration of shear wave elastography (SWE) to enable objective biomechanical validation of tumor-mimicking materials. A patient-specific skull model was created using fused deposition modeling (FDM) 3D printing and paired with custom-molded tumor repli

Extent of resection and perioperative neurological status as prognostic markers in glioblastoma: a population-based study.

PURPOSE: Maximal safe resection is the suggested standard for surgical treatment for patients with IDH-wildtype glioblastoma. Surgical safety is prioritized over radical extent of resection (EOR) because new neurological deficits are considered detrimental for survival. The aim of this study was to explore the combined impact of extent of resection and neurological status on survival in glioblastoma patients, using standardized tools for neurological function (neurological assessment in neuro-oncology (NANO scale) and extent of resection (RANO resect classification). METHODS: Population-based

Estimating post-operative complication rates in patients with primary brain tumours from routine administrative data: A national cohort study.

INTRODUCTION: Neurosurgery is an important element of brain tumour treatment but carries with it the risk of complications. Previous work has defined a narrow set of general post-operative complications which are used as Patient Safety Indicators (PSIs), but these are not brain tumour specific and do not capture the full range of complications. As a result, there is no way of measuring post-operative complications in neurosurgery at a national level. METHODS: We conducted a retrospective, observational cohort study using a comprehensive national administrative dataset from England on adult br

Surgical management of a lenticulostriate artery aneurysm and an associated initially less-appreciated arteriovenous malformation: two-dimensional video.

Surgical management of the basal ganglia vascular lesions needs a personalized surgical strategy due to intricate neurovascular anatomy and eloquent surrounding structures. This report describes the successful resection of a perforator aneurysm and an initially less-appreciated arteriovenous malformation (AVM) at the basal ganglia in a 15-year-old male. Preoperative digital subtraction angiography (DSA) and high-resolution magnetic resonance imaging (MRI) showed a deep-seated basal ganglia aneurysm on a lenticulostriate artery (LSA) perforator with a distal suspicious early drainage vein adjac
