Health Education Barriers for Ghanaian Parents of Children with Cleft Lip and Palate: Qualitative Study.

INTRODUCTION: Health education plays a critical role in managing congenital anomalies like cleft lip and palate (CL/P). It equips parents with the knowledge and confidence needed for effective care, addressing medical, nutritional, and psychosocial challenges. In Ghana, where CL/P prevalence exceeds the global average and no national health education guidelines exist, initiatives often overlook culturally tailored approaches. OBJECTIVE: To explore health education perceptions and barriers faced by Ghanaian parents of children with CL/P, guided by the social-ecological model at Komfo Anokye Te

Mechanical Support Escalation to Bridge Anemic Jehovah's Witness to Cardiac Transplantation.

Background: Jehovah's Witness (JW) patients pose a unique challenge to cardiac surgery due to their refusal of blood products, typically precluding them from becoming candidates for orthotopic heart transplantation (OHT). While "bloodless" cardiac surgery has been described in ideal candidates, anemic or other hematologic-risk patients are typically excluded. We describe a successful "bloodless" OHT in a non-ideal JW patient with anemia and cardiogenic shock, with ventricular assist through a transvalvular pump to bridge and optimize hematologic status prior to operation. Case Presentation: A

The impact of living arrangements and deinstitutionalisation in the health status of persons with intellectual disability in Europe.

BACKGROUND: Despite progress in the process of deinstitutionalisation, very little is known about the health conditions of people with intellectual disability (PWID) who live in large institutions and PWID living in small residential services, family homes or independent living within the community. Furthermore, there are no international comparison studies at European level of the health status and health risk factors of PWID living in fully staffed residential services with formal support and care compared with those living in unstaffed family homes or independent houses with no formal suppo

Propofol-induced hypertriglyceridemia in critically ill trauma patients requiring nutrition therapy: A descriptive cohort study.

BACKGROUND: This study aimed to assess the incidence and identify risk factors associated with the development of propofol-induced hypertriglyceridemia in critically ill trauma patients who required nutrition therapy. METHODS: Adult patients (aged >17 years) admitted to the trauma intensive care unit who received propofol for >24 h and had a serum triglyceride concentration determination during the infusion were retrospectively evaluated. Hypertriglyceridemia was defined as a serum triglyceride concentration > 400 mg/dL. Potential risk factors for hypertriglyceridemia as well as linear and no

Living alone, social networks in neighbourhoods, and daily fruit and vegetable consumption among middle-aged and older adults in the USA.

OBJECTIVE: A social network is a valuable resource in later life. Therefore, the current study aims to investigate whether social networks within homes and neighbourhoods are associated with older adults' daily fruit and vegetable consumption. DESIGN: Cross-sectional secondary data analysis. SETTING: USA. PARTICIPANTS: A nationally representative sample of 6865 community-dwelling older adults over age 53 in the Health and Retirement Study - Health Care and Nutrition Survey. RESULTS: Older adults who lived alone with no children or friends nearby had the lowest fruit and vegetable consumpti

Proportionate Postnatal Growth in Preterm Neonates on Expressed Breast Milk Feeding With Selected Fortification.

BACKGROUND: Preterm neonates not fed an exclusive human-milk diet in the neonatal intensive care unit (NICU) show disproportionate postnatal growth. There are scant data on postnatal growth in neonates from India fed an exclusive expressed breast milk (EBM) diet. This study describes the postnatal changes in weight, length, and head circumference in preterm neonates given EBM with selected fortification. METHODS: The study had a prospective observational design. Exclusive EBM feeding, early initiation, and standardized progression of feeds was followed. Fortification of breast milk with human

Chronic diseases and social risk factors in relation to specific symptoms of depression: Evidence from the U.S. national health and nutrition examination surveys.

BACKGROUND: Depression is a heterogeneous mental disorder with multiple symptoms, but only few studies have examined whether associations of risk factors with depression are symptom-specific. We examined whether chronic diseases and social risk factors (poverty, divorce, and perceived lack of emotional support) are differently associated with somatic and cognitive/affective symptoms of depression. METHODS: Cross-sectional analyses were based on individual-level data from the 31,191 participants of six cross-sectional U.S. National Health and Nutrition Examination Surveys (NHANES) carried out

Outcome of Very Preterm Infants With Early Optimal Nutrition Strategy: A Comparative Cohort Study.

BACKGROUND: Aggressive nutrition may benefit early growth; nevertheless, effects on neurodevelopmental outcomes are unclear. We planned a descriptive analytical study to compare survival without neurodevelopment disability (NDD) at 1 year in 2 groups during 2 time epochs-before and after implementation of early optimal nutrition strategies. NDD was defined as any one of the following: mental and/or motor development quotient < 85 at 12 months of age, corrected for prematurity; Denver Developmental Screening Test abnormal/suspect in even 1 domain out of the 4 domains; seizures; requirement of h

Global paediatric advanced life support: improving child survival in limited-resource settings.

Nearly all global mortality in children younger than 5 years (99%) occurs in developing countries. The leading causes of mortality in children younger than 5 years worldwide, pneumonia and diarrhoeal illness, account for 1·396 and 0·801 million annual deaths, respectively. Although important advances in prevention are being made, advanced life support management in children in developing countries is often incomplete because of limited resources. Existing advanced life support management guidelines for children in limited-resource settings are mainly empirical, rather than evidence-based, writ

Hypervitaminosis a in pediatric hematopoietic stem cell patients requiring renal replacement therapy.

BACKGROUND: Chronic renal failure patients have been known to develop vitamin A toxicity, but a descriptive study of hypervitaminosis A in patients with acute renal failure (ARF) has not yet been published. The authors observed hypervitaminosis A in pediatric hematopoietic stem cell transplant (HSCT) patients. METHODS: All HSCT patients admitted between January 2001 and May 2006 who experienced ARF, received renal replacement therapy (RRT), and had a vitamin A level drawn were included in this retrospective, descriptive study. Molar ratios of vitamin A and retinol-binding protein (RBP) were c

Survival rate and prognostic factors in patients with intestinal failure.

BACKGROUND: Intestinal failure impairs nutritional status and survival expectance. Though intestinal adaptation and enteral independence may be achieved, artificial nutrition is needed in about half of the patients. AIMS: This study is aimed at assessing the causes of death, survival rate, enteral independence in time, and factors affecting the clinical outcome in a group of patients with intestinal insufficiency. PATIENTS: Sixty-eight patients with intestinal insufficiency, due to major intestinal resection in 60 cases (short bowel syndrome) (remnant intestine length 101-150 cm in 31 cases,

Tube-feeding decisions in the elderly.

Tube-feeding decisions in the elderly can be most challenging. We begin our decision making with the premise that artificial nutrition is a medical therapy and therefore that its prescription is based on a calculation of net benefits over burdens for the patient to whom it is given. When the burdens of this therapy outweigh the benefits of prolonged life, tube feeding may be ethically withheld or withdrawn. For the cognitively impaired and in the absence of known patient preferences, the ratio of benefits to burdens may best be figured after a time trial of therapy. If restraints are needed to

Bundibugyo Virus Disease in 2026 - Clinical and Public Health Responses.

Bundibugyo virus is a relatively rare orthoebolavirus that has caused only two previously recognized disease outbreaks but remains capable of producing severe epidemic disease with substantial mortality. The 2026 outbreak of Bundibugyo virus disease in the Democratic Republic of Congo has highlighted persistent challenges in the detection of filovirus disease outbreaks, as well as in diagnosis, clinical management, and the public health response, particularly in resource-limited settings. As with other filovirus infections, effective control of the Bundibugyo virus disease outbreak depends on

Scurvy presenting as lower limb ecchymoses in the setting of metastatic colorectal cancer.

A 58-year-old woman presented with a 1-week history of lower limb bruising. She had a medical history of recurrent metastatic colon cancer with a sigmoid colectomy and complete pelvic exenteration leading to colostomy and urostomy formation. She had malignant sacral mass encroaching on the spinal cord. This caused a left-sided foot drop for which she used an ankle-foot orthosis. She was on cetuximab and had received radiotherapy to the sacral mass 1 month ago. On examination, there were macular ecchymoses with petechiae on the lower limbs. There was sparing of areas that had been compressed by

Update on traumatic acute spinal cord injury. Part 2.

The aim of treatment in acute traumatic spinal cord injury is to preserve residual neurologic function, avoid secondary injury, and restore spinal alignment and stability. In this second part of the review, we describe the management of spinal cord injury focusing on issues related to short-term respiratory management, where the preservation of diaphragmatic function is a priority, with prediction of the duration of mechanical ventilation and the need for tracheostomy. Surgical assessment of spinal injuries based on updated criteria is discussed, taking into account that although the type of i

[Pharmacists as part of an outpatient palliative care team].

Pharmacists are an integral factor in palliative medical care, especially in the context of specialised outpatient palliative care (in Germany „SAPV“). As part of a multi professional team, pharmacists take care of patients especially in complex supply situations. The drug therapy is always checked for side-effects and interactions, and the medication is adjusted to application methods which are still possible (e. g. subcutaneous administration or administration via gastro-intestinal probes). A 24/7 supply of urgently needed medicines needs to be ensured and clinical nutrition must be suited t
