A phase 2 open-label pilot study of Remimazolam for sedation in critically ill patients.

INTRODUCTION: Remimazolam is a novel benzodiazepine with an ultra-short half-life. It is a potentially interesting alternative for sedation in the Intensive Care Unit, but there is limited data regarding its use in critically ill patients. METHODS: Phase 2, investigator-initiated, single-center, non-randomized, open-label study. Patients with an expected duration of sedation ≥ 24 h were eligible and received a maximum 48-h infusion of Remimazolam, with a dose ranging from 0.1 to 1 mg/min. The primary endpoint was a composite of the ability to reach a targeted sedation level without the use of

Measuring the IMPACT: Pilot Validation of a Novel Patient-Reported Outcome Measure for Craniomaxillofacial Trauma.

BACKGROUND: There is a paucity of validated, patient-reported outcome measures for quality-of-life (QOL) in craniomaxillofacial (CMF) trauma. OBJECTIVE: To determine whether the Integrated Modular Patient-Reported Outcome Assessment for Craniomaxillofacial Trauma (IMPACT) independently measures QOL in patients presenting for routine follow-up within 3 months of injury by comparing with the 15-Dimension QOL survey (15D). METHODS: A mixed-methods, pilot validation was performed via literature review, expert opinion, patient interviews, and prospective survey collection. Criterion validity was

SAHRANG: Subarachnoid Hemorrhage Recovery and Galantamine - A pilot multicenter randomized placebo-controlled trial.

Background Subarachnoid hemorrhage (SAH) causes life-long neurologic dysfunctions. Peripheral inflammatory processes as a reaction to brain injury has been shown to worsen outcomes after SAH. Galantamine has been shown to reduce proinflammatory microglial activities and improve synaptic connections. We hypothesize that galantamine treatment after SAH mitigates inflammation-mediated neuronal injury and improve outcomes. We conducted a pilot clinical trial to examine the tolerability and safety of galantamine in SAH patients. Methods This prospective, multicenter, double-blind, randomized, place

A dose-ranging pilot trial of dexmedetomidine-propofol in children undergoing magnetic resonance imaging.

The optimal sedation/anesthesia technique for magnetic resonance imaging (MRI) scans has not been established. The combination of propofol with dexmedetomidine has been reported, but without systematic dosing data. Our primary aim was to determine the propofol-sparing effect of dexmedetomidine (DEX) when added to propofol for MRI scan sedation/anesthesia utilizing a dose-ranging protocol for four distinct regimens (Propofol-Only, DEX-High, DEX-Low, DEX-Bolus). Our secondary aims were to document adverse events, scan interruptions due to patient movements, and determine recovery time. Seventy-n

Psychological mechanisms of prenatal mindfulness training on antenatal blood pressure reduction: A pilot study.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are a significant cause of perinatal morbidity and mortality. Mindfulness training (MT) shows promise for reducing blood pressure during pregnancy; however, the underlying mechanisms by which MT exerts this effect remain unclear. OBJECTIVES: The objectives of this pilot randomized clinical trial were to study psychological mechanisms through which MT may reduce blood pressure in pregnancy. STUDY DESIGN: Pregnant participants were randomized to an 8-week phone-delivered mindfulness intervention or usual care. Antenatal blood pressure value

Hip Fracture Intervention Study for Prevention of Hypotension Trial: a Pilot Randomized Controlled Trial.

BACKGROUND: Hypotension during anesthesia for surgery for hip fracture is common and associated with myocardial injury, stroke, acute kidney injury, and delirium. We hypothesized that maintaining intraoperative blood pressure close to patients' preoperative values would reduce these complications compared to usual care. METHODS: A pilot feasibility patient- and assessor-blinded parallel group randomized controlled trial. People with unilateral hip fracture aged ≥70 years with capacity to give consent before surgery were eligible. Participants were allocated at random before surgery to either

NK3 receptor antagonist alters the centrally controlled perception of menopausal flushing - a pilot study.

OBJECTIVE: Kisspeptin/neurokinin/dynorphin (KNDy) signaling links reproductive and thermoregulatory systems, and improvements in menopausal flushing are reported with neurokinin 3 receptor (NK3R) antagonists. A rise in brainstem activity preceding a flush has been proposed as its functional origin, with subsequent activity in the insula and prefrontal cortices reflecting individual perception. Using functional magnetic resonance imaging (fMRI), this study investigated the central effect of the NK3R antagonist MLE4901 during a flush, particularly functional connectivity changes in the salience

Slow Wave Sleep and Emotion Regulation in Adolescents With Depressive Symptoms: An Experimental Pilot Study.

Emotion regulation deficits are a hallmark of adolescent depression, and sleep greatly impacts emotion regulation. Initial data indicate acute mood benefits of slow-wave sleep deprivation (SWSD) in depressed adults, but it is unclear whether this may occur through improvement in emotion regulation. In addition, this has not been tested experimentally in adolescent depression. In this pilot study, we tested the effect of SWSD on emotion regulation in adolescents with elevated depressive symptoms. Fifteen adolescents (mean age [SD] = 17.47 [1.55] years, 12 female) completed three consecutive nig

Urinary biomarkers of acute kidney injury in neonates < 25 weeks' gestation: a pilot study.

BACKGROUND: Acute kidney injury (AKI) occurs in 30% of premature neonates and is associated with adverse outcomes that may be mitigated with early detection. Diagnosis of AKI relies on serum creatinine (SCr) which has several limitations, including lack of sensitivity and specificity. There are limited data on the utility of alternative urine biomarkers to predict AKI in neonates born at < 25 weeks' gestation. METHODS: Urine was collected from neonates born at < 25 weeks' gestation during the first postnatal week. Two urine AKI biomarkers, neutrophil gelatinase-associated lipocalin (NGAL) and

A pilot prospective study of arterial stiffness during weight restoration in adolescents with anorexia nervosa.

PURPOSE: Carotid-femoral pulse wave velocity (cfPWV), an index of arterial stiffness, is one of the earliest indicators of cardiovascular risk. Studies of adolescents with anorexia nervosa have demonstrated increased arterial stiffness compared to healthy controls. Little information is available on the effect of weight restoration on arterial stiffness in adolescents with anorexia nervosa. METHODS: This pilot longitudinal study examined changes in arterial stiffness during weight restoration in adolescent females admitted to an inpatient eating disorder unit. Female adolescents aged 15-19 ye

Predicting exercise pulmonary hypertension: the right-net machine learning model a pilot study.

BACKGROUND: Exercise-transthoracic Doppler echocardiography (Ex-TTE) determination of mean pulmonary arterial pressure (mPAP)/cardiac output (CO) slope may offer key diagnostic and prognostic information in cardiorespiratory diseases. However, its applicability and reliability in routine clinical practice remain to be established. Herein, the aim of the present study was to apply a machine learning (ML) model to predict abnormal exercise TTE-derived mPAP/CO slope (>3 mmHg/L·min) in individuals at risk of pulmonary hypertension (PH), based only on clinical and resting TTE parameters. METHODS:

MicroRNA Expression Profiles in Very Preterm Infants with Patent Ductus Arteriosus: A Pilot Study.

INTRODUCTION: Very preterm infants are at risk for developing hemodynamically significant patent ductus arteriosus (hsPDA), which contributes to increased morbidity. The optimal management of hsPDA remains controversial, and treatment options are associated with complications. Developing accurate prediction tools for hsPDA closure is essential to guide management strategies. The aim of the present pilot study was to investigate microRNA expression profiles in very preterm infants with and without hsPDA and to assess their potential as biomarkers for hsPDA. METHODS: We prospectively enrolled p

Tai Chi Easy for Opioid Use Disorder: Pilot Feasibility Outcomes and Lessons Learned.

Background: Opioid use disorder (OUD) remains a global crisis, with chronic pain (CP), anxiety, and opioid cravings contributing to treatment discontinuation and return to opioid use. Despite the effectiveness of medications for OUD (MOUD) such as buprenorphine and methadone, they are underutilized, with nearly 50% of individuals discontinuing prematurely or returning to opioid use. Mind-body therapies (e.g., mindfulness, acupuncture, Tai Chi) have shown promise in reducing stress, drug cravings, and pain while improving well-being. However, Tai Chi has not been systematically tested for indiv

Pilot Retrospective Evaluation of a Balancing and Optimizing Injection Pattern for the Frontalis Muscle Using LetibotulinumtoxinA.

Signs of aging in the upper face arise from multimodal changes in facial anatomy, contributing to concerns such as eyebrow ptosis and forehead lines. While neurotoxin injections are widely used to address these lines, the anatomical variability of the frontalis muscle presents procedural challenges. This retrospective analysis aimed to introduce and preliminarily evaluate a structured injection pattern for forehead treatment, developed with attention to the biomechanics of upper facial musculature. A total of 24 patients (mean age 42.5 ± 9.1 years) treated with a standardized injection scheme

Metagenomic whole genome shotgun analysis of the airway microbiome in laryngotracheal stenosis: a pilot study.

The airway microbiome has been implicated in the pathogenesis of laryngotracheal stenosis (LTS), yet prior studies using 16 S rRNA sequencing have limited sub-genus level resolution. Metagenomic whole genome shotgun sequencing (mWGS) allows for strain-level taxonomic and functional genomic analysis, providing detailed insights into specific organisms and pathways. A pilot study was conducted to explore the advantages and challenges of mWGS in investigating the airway metagenome in LTS. mWGS was conducted on 12 intraoperative swab samples from 8 LTS patients, divided into tracheostomy-dependent
